Forty patients with bladder outflow obstruction due to benign prostatic hypertrophy were assessed by means of symptom analysis, rectal examination, intravenous urography and/or cystoscopy, medium-fill water cystometry and pressure flow study. No significant correlation was found between the severity of obstruction and the degree of instability, but the occurrence of instability did correlate with the degree of obstruction. This latter conclusion, though logical, had not been proved before, and was achieved by means of a new urodynamic parameter.

Between 1978 and 1982, 88 patients were treated with radiotherapy at the University of the Witwatersrand Hospitals for carcinoma of the bladder. They were followed up for evidence of cystitis (i.e. frequency, urgency, nocturia, dysuria, haematuria). Those with persistent symptoms underwent cystoscopy. Biopsies were taken of areas suspicious of residual or recurrent tumour. The total incidence of late bladder complications was 11 (12,5%). Of these 11 patients, 6 had persistent or recurrent tumour. The implications of this are discussed.

In a 48-year-old man with dysuria and microscopic haematuria, pyelography, cystoscopy and bimanual bladder palpation showed no definite abnormality. Because of continued symptoms, ultrasound scanning of the abdomen was done three months later. The examination revealed a tumour in close relation to the top of the bladder. Malignant cells were aspirated at ultrasonically guided puncture, and a urachus tumour was suspected. For early diagnosis of urachus tumours, ultrasound investigation of vague bladder symptoms is required.

A case report of vaginal calculus formation in a nine-year-old girl with myelodysplasia is presented. Etiologic factors in the formation of such calculi include fistulous communication between the vagina and the bladder, neuropathic urethrovesical dysfunction, anatomic conditions causing vaginal outlet obstruction, and/or vaginal pooling of urine and urease-producing bacterial infection. A correct preoperative diagnosis can be made by using oblique x-ray films and cystoscopy. Surgical treatment is simple and successful.

A review of the symptoms and signs as well as preoperative investigations was made in 12 patients with proved colovesical fistulas. The symptoms were predominantly those of the lower urinary tract. The value of barium enema, cystogram, cystoscopy and sigmoidoscopy was pointed out. Treatment consisted of a combined treatment with colostomy and indwelling bladder catheter, primary closure of the fistula only and various staged procedures with bowel resection. We emphasize the value of the one-stage procedure in the uncomplicated case.

Endometrial cancer following a supra-cervical hysterectomy is a rare event and a correct diagnosis requires high clinical suspicion. Here we report a case of a woman who developed endometrial cancer years after supracervical hysterectomy was performed for placenta accreta spectrum (PAS) with bladder invasion. The patient presented with urinary bleeding and was diagnosed on cystoscopy; her pathology and unusual history led to the ultimate diagnosis of a primary endometrial cancer in the remaining uterine stump.

Penile fracture is a urologic injury with an etiology that varies based on the cultural milieu. Diagnosis can be made based on history and physical examination alone. Patients should be evaluated with RUG or cystoscopy when urethral injury is suspected. Ultrasound or MRI is a helpful adjunct when the diagnosis is unclear, and can assist in identifying the location of the rupture. Surgical management is favored over conservative measures to improve outcomes. Delayed surgical repair may not be inferior to immediate intervention.

A 22-year-old woman met with road traffic accident 6 months back following which she underwent exploratory laparotomy with intraperitoneal bladder rupture repair. She presented with urethrovaginal fistula due to a fragment of fractured pubic bone impinging into the anterior vaginal wall. The findings were confirmed on CT scan and cystoscopy. The patient was managed with removal of the bony spicule and transvaginal repair of urethrovaginal fistula with Martius fat pad interposition.

We report an extremely rare case of a dermoid cyst of the urinary bladder in a 30-year-old female who presented with pain in the left flank and dysuria since 9 months. On imaging (ultrasound and computed tomography), a relatively well-defined mass lesion with areas of fat and calcification was seen arising from the bladder wall. Cystoscopy showed presence of hair on the surface of the lesion. Histopathological findings were consistent with dermoid cyst in the urinary bladder.

We report three cases of stone formation at the prostatic urethra after 80 W potassium-titanyl-phosphate laser ablation of the prostate for benign prostatic hyperplasia. These complications occurred several months after the procedure. The diagnosis was made by ultrasonography, plain radiography of the kidneys, ureters, and bladder, and cystoscopy in patients who were being evaluated after presenting with irritative voiding symptoms. The patients were treated by cystolithotripsy and resection of the calcified prostatic tissue.

The case of a 69-year-old female patient come to our observation with the diagnosis of non-Hodgkin's lymphoma on histology and cystoscopy is discussed. Symptoms were characterized by dysuria, vaginal spotting and abdominal pain. CT and NMR for staging and control CT during chemotherapy were performed. They documented marked reduction of the neoformation protruding into the bladder and concentric urethral thickening associated with volumetric stability of intercavoaortic lymph node involvement and stable condition of the other localizations.

The use of anticoagulants, for whatever indication, may carry a high risk of hematoma formation following surgery. Obstructive uropathy is a very rare but possible example of complication secondary to an extensive pelvic hematoma. We describe a case of a patient with rheumatic heart disease and aortic valve replacement, who developed a massive postoperative pelvic hematoma following bilateral tubal ligation, resulting in bilateral ureteric obstructions. This was treated with bilateral ureteric stent through cystoscopy.

A 3-year-old boy was referred to us with a diagnosis of bladder tumor. On cystoscopy, a yellowish-white pedunculated tumor was found at the right trigone and biopsy revealed an inflammatory change. Computed tomographic (CT) scan showed a soft tissue density from the lower ureter to the bladder. Open surgery confirmed a ureteral polyp originating from the lower ureter prolapsing into the bladder, which mimicked bladder tumor. The need of open surgery in such cases is discussed.

A 63-year-old man presented with haematuria and suprapubic pain. After transurethral resection of the prostate a tumour was seen in the bladder dome at cystoscopy. Biopsy revealed an adenocarcinoma. Based on intraoperative and histological examination of the partial cystectomy specimen, the tumour was judged to be an urachus carcinoma. Four years after diagnosis the patient has no evidence of tumour. While partial cystectomy is the most frequent treatment, a radical total cystectomy is advocated in poorly differentiated tumours.

A tumor-like form of urinary schistosomiasis is reported. Diagnosis was established intraoperatively. The finding of hematuria and of a stay in an area where schistosomiasis is endemic suggested the diagnosis but specific investigations were negative. Cystoscopy revealed a budding, hemorrhagic tumor lying against the posterior aspect of the bladder with no other lesions suggestive of schistosomiasis. The true prevalence of these tumor-like forms of urinary schistosomiasis is analyzed. Emphasis is put on the need for using several investigations to reach the correct diagnosis.

A case of eosinophilic cystitis in a child is presented and the pediatric literature reviewed. The etiology of this condition is still unknown. Mean age of children with eosinophilic cystitis is 5.9 years. Black children appear to be more affected than white children, with a male preponderance. Hematuria and dysuria are the most common symptoms. The diagnosis of eosinophilic cystitis is made by cystoscopy with biopsy. The disease appears self-limited with its course unaltered by antibiotics. Recurrence of eosinophilic cystitis is possible.

A case of pseudotumor of the urinary bladder secondary to diverticulitis of the sigmoid colon with colo-vesical fistula is reported. Computed tomography and cystoscopy of the granulomatous mass caused by the inflammation suggested a colon or bladder cancer. A transurethral biopsy from the tumor was therefore performed at first, but showed only inflammation. Because primary bladder cancer was ruled out, partial cystectomy and partial colectomy were subsequently performed. Through histological examination the mass proved to be an inflammatory granuloma.

A case of adenocarcinoma appearing in the bladder six years after supravesical urinary diversion is reported. The reason for the diversion was chronic bladder inflammation, as in all but one of the 11 earlier reported cases of carcinoma in defunctionalized bladder. The theory that chronic bladder inflammation favors the development of, or possibly leads to malignancy is reviewed,. The conclusion is a recommendation for periodic cystoscopy of the bladder "left behind", and possibly for cystectomy at the time of urinary diversion in patients with a long history of bladder inflammation.

A 60-year-old female presented with the main complaints of hematuria, pain upon urination, and pollakiuria. Simple abdominal X-rays revealed a dumb bell-shaped calculus shadow, and a calculus was detected in the right posterior wall by cystoscopy. At surgery, a left ovarian dermoid cyst accompanied by a calculus that was adhesive and perforating in the right posterior wall of bladder was detected. Twenty-eight cases of ovarian dermoid cyst with bladder perforation including this case are reported.
