Desmosis coli - a case report and review of the literature.

Desmosis coli is a rare pathology presenting as slow transit constipation. In this case we would like to discuss the presentation and management of desmosis coli. A 14-month-old female hailing from western India with a history of chronic constipation presented with acute massive abdominal distension and vomiting. At laparotomy, a hugely dilated transverse and sigmoid colon with a transition zone at the lower sigmoid was found. A transverse stoma was done after taking multiple sero-muscular biopsies. The patient underwent re-exploration on day 14 because of the non-functioning of the stoma and

Therapeutic Armamentarium for Stricturing Crohn's Disease: Medical Versus Endoscopic Versus Surgical Approaches.

One-third of patients with Crohn's disease (CD) present as stricturing phenotype characterized by progressive luminal narrowing and obstructive symptoms. The diagnosis and management of these patients have been intriguing and challenging. Immunomodulators and biologics have been successfully used in treating inflammatory and fistulizing CD. There are issues of efficacy and safety of biological agents in treating strictures in CD. Rapid mucosal healing from potent biological agents may predispose patients to the development of new strictures or worsening of existing strictures. On the other han

Bilateral ectasia after femtosecond laser-assisted small-incision lenticule extraction.

UNLABELLED: This case report describes clinical and topographic features of bilateral corneal ectasia after femtosecond laser-assisted small-incision lenticule extraction. The case suggests that patients with preoperative forme fruste keratoconus or early keratoconus might develop significant progression of corneal ectasia after the small-incision lenticule extraction procedure and shows that the procedure can affect the corneal biomechanics. FINANCIAL DISCLOSURE: The author has no financial or proprietary interest in any material or method mentioned.

A Giant Right Coronary Artery of Diffuse Ectasia Induced by a Right Ventricular Fistula.

Coronary artery fistula (CAF) is a rare entity. Sometimes it may associate with mild diffuse or segmental coronary ectasia. CAF with giant coronary artery is exceptionally rare. We present a unique case of a 49-year-old female patient with a giant right coronary artery of diffuse ectasia coexisting with a fistula draining into the right ventricle. To our best knowledge, CAF with diffuse coronary ectasia of such giant size has never been reported. The patient was treated successfully by resection of the dilated right coronary artery, fistula closure, and coronary artery bypass grafting.

Surgical management of cystic dilatation bile ducts in adults.

BACKGROUND: The cystic dilatation of the biliary tract is a rare disease and uncertain origin. It is recognized more frequently in children; however, its incidence comes increasing in adults, representing 20% of the cases. AIM: To evaluate morbimortality rates, evolution and handing of patients with cystic dilatation bile ducts in adults. METHODS: Were evaluated, retrospectively, five adults who had the diagnosis of choledochal cyst and that had been submitted to some surgical procedure. RESULTS: Abdominal pain was the commonest complain to all patients. Jaundice was present in 80%. Ultraso

Evidence-based review of gastrointestinal bleeding in the chronic kidney disease patient.

Patients with end-stage renal disease are more likely to suffer from gastrointestinal (GI) problems, including bleeding from upper and lower sources. Peptic ulcer disease is the most common cause of upper GI bleeding, and although there is some debate in the literature regarding whether the frequency of ulcer disease is higher in patients with kidney disease, it is well established that outcomes are worse in patients with compromised renal function. Angioectasias can be found throughout the GI tract and are another common cause of bleeding; management can be divided into localized endoscopic t

Acute dilatation of the ascending aorta and aortic valve regurgitation in Loeys-Dietz syndrome.

Loeys-Dietz syndrome (LDS) is a recently recognized connective tissue disorder caused by mutations of the transforming growth factor (TGF)-β receptors. It is an autosomal dominant syndrome characterized by the triad of arterial tortuosity and aneurysms, hypertelorism, and bifid uvula or cleft palate. We treated an 18-year-old woman with a 100-mm-diameter aortic root aneurysm and severe aortic valve regurgitation. She underwent urgent aortic root replacement and bioprosthetic valve implantation. LDS was diagnosed by postoperative genetic screening results. Histopathologic examination of the aor

Scheimpflug imaging for keratoconus and ectatic disease.

Scheimpflug cross-sectioning anterior segment imaging offers significant advantages over traditional placido based curvature analysis and ultrasound pachymetry. The accurate measurement of both the anterior and posterior corneal surfaces and the anterior and posterior lens allows for the creation of a three-dimensional reconstruction of the anterior segment. Changes on both the posterior cornea and/or corneal thickness map are earlier indicators of ectatic change than would otherwise be identifiable with only anterior curvature and ultrasonic pachymetry. Scheimpflug imaging also covers signifi

Jugular phlebectasia presenting as globus pharyngeus.

Even though much has been written on the aetiology of globus pharyngeus, it still remains elusive and multifactorial. We present a case of a 54-year-old woman who was referred by an orthopedist to the radiology department with a 6-day history of an intense feeling of "pressure" and "tightness" in the jugular notch. After performing a contrast-enhanced computed tomography scan, a phlebectasia of the right anterior jugular vein was discovered. When trying to determine the cause of the globus sensation one has to consider the possible existence ofa phlebectasia of one of the jugular veins.

High-resolution manometry classifications for idiopathic achalasia in patients with Chagas' disease esophagopathy.

BACKGROUND: Idiopathic achalasia (IA) and Chagas' disease esophagopathy (CDE) share several similarities. The comparison between IA and CDE is important to evaluate whether treatment options and their results can be accepted universally. High-resolution manometry (HRM) has proved a better diagnostic tool compared to conventional manometry. This study aims to evaluate HRM classifications for idiopathic achalasia in patients with CDE. METHODS: We studied 98 patients: 52 patients with CDE (52 % females, mean age, 57 ± 14 years) and 46 patients with IA (54 % females; mean age 48 ± 19 years). All

Reversible bronchiectasis in an adult: a case report.

Bronchiectasis is classically defined as irreversible bronchial dilatation that represents the sequelae of chronic airway inflammation and is permanent, reflecting the presence of bronchiectasis. It is increasingly being recognized with the wider availability of high-resolution computed tomography. In contrast to adults, children with noncystic fibrosis bronchiectasis may have resolution or significant improvement in bronchial dilatation. We report a case of reversible bronchial dilatation in an adult as evidenced by high-resolution computed tomography, which demonstrates that, although quite
