Adrenalectomy using natural orifice translumenal endoscopic surgery (NOTES): a transvaginal retroperitoneal approach.

BACKGROUND: Endoscopic adrenalectomy currently is performed using either a retroperitoneal or transperitoneal approach. The retroperitoneal approach is ideal for patients with small lesions who have undergone previous intraabdominal surgery. This study aimed to explore transvaginal retroperitoneal right and left adrenalectomies in porcine and cadaver models. METHODS: Right and left adrenalectomies were performed for two female pigs. With the pig supine under general anaesthesia, the retroperitoneal space was entered with a double-channel endoscope (Storz) through a posterior colpotomy. A retr

Intraabdominal nonvascular operations combined with abdominal aortic aneurysm repair.

The therapeutic approach to a patient who has an abdominal aortic aneurysm (AAA) and an intraabdominal nonvascular surgical disorder simultaneously remains controversial. To establish guidelines for the management of those patients, a retrospective review of patients who had concomitant AAA and intraabdominal nonvascular surgical disorders was undertaken. During the period January 1988 to December 1997 a series of 162 patients underwent surgical repairs of AAA in our hospital. Among them 16 patients (9.9%) had several kinds of intraabdominal nonvascular surgical disorders, and 13 underwent one

Exploratory analysis of the associations of short-chain fatty acids and their related factors with growth performance in growing pigs using a Bayesian network.

We suggested the causal relationships between daily weight gains (DG) and feed conversion ratios (FCR) of growing pigs, and short-chain fatty acids (SCFA) concentrations in digesta and blood, and SCFA-related factors such as intestinal morphology, SCFA transporters and tight junction (TJ) proteins. Twelve pigs were divided into 5% of rapeseed meal-, beet pulp- or starch-supplemented groups, kept for 21 days, and dissected. Bayesian network showed that factors affecting DG were expressions of sodium-coupled monocarboxylate transporter 1, claudin-1, and Zonula occludens-1 in cecal mucosa, monoca

Rare Pancreatic Arterial Variations: A Cadaveric Case Report.

The dorsal surfaces of the pancreatic head and body receive a significant amount of blood supply from the dorsal pancreatic artery, whose common origins include the splenic artery, superior mesenteric artery, celiac trunk, and the common hepatic artery, with the most common being the splenic artery. In the majority of cases, the dorsal pancreatic artery is present in some form. In this case study, the dorsal aspect of the head and body of the pancreas is supplied by a large artery not commonly found in the literature, referred to here as the right posterior pancreatic artery (RPPA). The RPPA w

Thoracic endovascular stent graft placement: a case report.

Endovascular stent grafting (EVSG) is a minimally invasive alternative to open repair of thoracic aortic aneurysms. It is useful in the treatment of thoracic aneurysms, dissections, and ruptures. Currently, the incidence of thoracic aortic aneurysms is 6:100,000 people. Comorbidities often include hypertension, coronary artery disease, chronic obstructive pulmonary disease, peripheral vascular disease, and cerebrovascular disease, and there often is a history of smoking. Without surgical intervention, a high risk of mortality exists, primarily due to aneurysm rupture. Due to the complexity of

Endovascular treatment of traumatic thoracic aortic rupture combined with diaphragmatic rupture--a case report.

The purposes of this report are to describe the immediate repair of a diaphragmatic rupture and subsequent endovascular treatment of a thoracic aortic rupture secondary to blunt chest and abdominal trauma and to discuss the outcome in the light of the trauma scores. A 29-year-old man was injured in a car collision. There were signs of a left diaphragmatic rupture and spleen injury. After urgent laparotomy, the diaphragm was repaired. The spleen appeared uninjured; however, there was a dissection with a transmural tear of the thoracic aorta, which was subsequently treated intraluminally by usin

[Influence of anesthesia on the localization of Schistosoma mansoni. Intestinal schistosomiasis in white mice].

General anesthetics or hypnotics (ketamine, propanidid, thiopental, althesin, halothane, enflurane, ether), were tested on white mice infested with Schistosoma mansoni to study possible changes in the migration of this trematode from the mesentery to the liver. With this in mind, populations of 25 mice of the same sex and age were chosen at random; they were divided into a control group and a group submitted to an anaesthetic agent. The mice were perfused after dissection and catheterization of the abdominal aorta. A vacuum-pump was used to collect separately the parasites from the mesenteric

Morphometric measurements of systemic atherosclerosis and visceral fat: Evidence from an autopsy study.

BACKGROUND: Morphometric measurements of systemic atherosclerosis and direct quantification of visceral fat are only possible using materials from autopsy studies. However, the few autopsy studies that have investigated the association of visceral fat with atherosclerosis had small sample sizes and focused on coronary arteries of young or middle-aged White subjects. We aimed to investigate the association of pericardial fat (PF) and abdominal visceral fat (AVF) with atherosclerosis in the aorta, coronary, carotid, and cerebral arteries in a large autopsy study. MATERIALS AND METHODS: We evalu

Acute type A aortic dissection complicated by acute pancreatitis and abdominal hemorrhage: a case report of following exploratory laparotomy.

BACKGROUND: There are few reports of postoperative acute pancreatitis (AP) in patients with acute type A aortic dissection (ATAAD), but we reported a case of ATAAD complicated by AP and later reoperation for intraperitoneal hemorrhage. We discussed the causes of AP in this patient and summarized some of the experiences of postoperative management of patients with ATAAD involving the celiac trunk and/or superior mesenteric artery. CASE PRESENTATIONS: A 44-year-old male patient was diagnosed with ATAAD and underwent partial resection of the ascending aorta with graft replacement and total aorti

Intervention rates and outcomes in medically managed uncomplicated descending thoracic aortic dissections.

OBJECTIVE: To evaluate the long-term incidence and outcome of aortic interventions for medically managed uncomplicated thoracic aortic dissections. METHODS: Between January 2012 and December 2018, 91 patients were discharged home with an uncomplicated, medically treated aortic dissection (involving the descending aorta with or without aortic arch involvement, no ascending involvement). After a median period of 4 (first quartile: 2, third quartile: 11) months, 30 patients (33%) required an aortic intervention. Patient characteristics, radiographic, treatment, and follow-up data were compared f

Aortic dissection in Taiwan.

We retrospectively reviewed all patients with a final diagnosis of spontaneous thoracic aortic dissection treated at Linkou Chang Gung Memorial Hospital between January 1989 and December 1994. There were a total of 109 patients with a mean age of 55 +/- 11 years ranging from 19 to 88 years. The male-to-female ratio was 2 to 1 (73 to 36). There was a predilection to present during the colder months, with 69% seen between September 1 and February 28 and only 31% during the warmer half of the year. In most patients, hypertension (85%) was the major predisposing factor with another 7% having Marfa

Diseases of the Aorta and Kidney Disease: conclusions from a Kidney Disease: Improving Global Outcomes (KDIGO) Controversies Conference.

Chronic kidney disease (CKD) is an independent risk factor for the development of abdominal aortic aneurysm (AAA), as well as for cardiovascular and renal events and all-cause mortality following surgery for AAA or thoracic aortic dissection. In addition, the incidence of acute kidney injury (AKI) after any aortic surgery is particularly high, and this AKI per se is independently associated with future cardiovascular events and mortality. On the other hand, both development of AKI after surgery and the long-term evolution of kidney function differ significantly depending on the type of AAA int

Aortic diseases--do we need MR techniques?

MR with its various imaging protocols including spin-echo MR-imaging, gradient MR imaging, flow-mapping and MR angiography is the ideal tool for noninvasive assessment of any kind of aortic diseases. The technique allows to image the aorta both in the thoracic cage and in the abdominal aspects better than any other tomographic image modality. With the capability of angiography after intravenous injection of gadolinium even the information previously only obtained from invasive catheter-based studies is now available within minutes after an intravenous injection. The overview manuscript compare

Transvenous Access for Emergent Thoracic and Thoracoabdominal Aortic Aneurysm Repair in Patients Without Femoral Access.

PURPOSE: To describe the technique of transvenous access for emergent endovascular repair of thoracic and thoracoabdominal aneurysms exemplified with 2 cases. TECHNIQUE: Transvenous access to the aorta is described as an alternative access method to deliver aortic endografts in emergency situations. A 68-year-old female patient with severely compromised iliac and subclavian artery access was treated for a ruptured extent V thoraco-abdominal aortic aneurysm with a t-Branch (Cook Medical, Bjaeverskov, Denmark) delivered through a transcaval access. To avoid severe aortocaval shunting a balloon-

Arterial dissections associated with pregnancy.

Two cases of spontaneous arterial dissection occurring in young, multiparous women shortly after delivery of uncomplicated pregnancies are described. Histologic analysis of arterial tissue samples obtained in both cases at points near and remote from the dissection sites shows evidence of significant arterial degeneration and loss of integrity, with changes similar to those observed in pregnant women, women using oral contraceptives, and animals given female sex hormones. The types of arterial lesions associated with pregnancy and their sites of predilection and the etiologic roles of the hemo

Coagulopathy induced by aortoiliac aneurysms.

Diffuse intravascular coagulopathy (DIC) or consumptive coagulopathy is infrequently associated with aortic aneurysms. When the two coexist, they create a difficult clinical problem that requires optimal medical and surgical care. Two cases of coagulopathy associated with aortoiliac aneurysm are presented to exemplify the broad clinical picture that can be present. The definitive treatment of DIC is removal of the underlying cause. The following recommendations are made for the management of patients with aortic aneurysms and possible coagulopathy: preoperatively examine the patient for hemato

Nanoparticle-Mediated Delivery of Pitavastatin to Monocytes/Macrophages Inhibits Angiotensin II-Induced Abdominal Aortic Aneurysm Formation in Apoe-/- Mice.

AIM: Abdominal aortic aneurysm (AAA) is a lethal and multifactorial disease. To prevent a rupture and dissection of enlarged AAA, prophylactic surgery and stenting are currently available. There are, however, no medical therapies preventing these complications of AAA. Statin is one of the candidates, but its efficacy on AAA formation/progression remains controversial. We have previously demonstrated that nanoparticles (NPs) incorporating pitavastatin (Pitava-NPs)-clinical trials using these nanoparticles have been already conducted-suppressed progression of atherosclerosis in apolipoprotein E-
