Cervical diverticulitis: a novel complication of a neonatal colonic interposition graft following oesophagectomy.

A 47-year-old woman presented to a tertiary emergency department with an 8-day history of odynophagia, a 4 cm swelling on her left neck and intermittent fevers. Shortly following her birth, a congenital oesophageal atresia had been managed surgically with colonic interposition graft. Contrast CT of the neck demonstrated several large diverticula within her interposition graft at the level of the cervical vertebrae. A colocutaneous fistula was identified between the colon and left neck, with an associated abscess. The patient received intravenous meropenem followed by abscess drainage. A high o

Clinically Diagnosed Acute Diverticulitis in Outpatients: Misdiagnosis in Patients with Irritable Bowel Syndrome.

BACKGROUND: Physicians often diagnose diverticulitis and prescribe antibiotics in outpatients with abdominal pain and tenderness without other evidence. AIM: We investigated the misattribution of irritable bowel syndrome (IBS) symptoms to diverticulitis in outpatients. METHODS: In patients diagnosed with diverticulitis and dispensed antibiotics in an integrated healthcare system, we retrospectively compared 15,846 outpatients managed without computed tomography (CT) versus 3750 emergency department/inpatients who had CT. We assessed demographics and past history, including 17 symptom-based s…

Elderly patient with acute, left lower abdominal pain: perforated jejunal diverticulitis (2010:7b).

An elderly patient with acute, left, lower abdominal pain is described, for whom the diagnosis of perforated jejunal diverticulitis was established by computed tomography (CT). The presence of a jejunal segmental inflammatory process, with or without abscess or perforation, in the setting of jejunal diverticulosis, is very suggestive of jejunal diverticulitis.

Imaging findings of right colon diverticulitis that mimics colon cancer: a case report.

We present a case report of a patient with imaging findings of right colon diverticulitis that mimics colon cancer. The computed tomography showed a segment of narrowing with shoulder formation at ascending colon suggesting cancer. The colonoscopy and the follow-up imaging clarify the diagnosis of diverticulitis. We assess the value of computed tomography findings of acute diverticulitis in excluding cancer.

Long-term experience with the laparoscopic approach to perforated diverticulitis plus generalized peritonitis.

BACKGROUND: The treatment of perforated diverticulitis is changing form the current standard of laparotomy with resection, Hartmann procedure, and colostomy to a minimally invasive technique. In patients with complicated acute diverticulitis and peritonitis without gross fecal contamination, laparoscopic peritoneal lavage, inspection of the colon, and intraoperative drain placement of the peritoneal cavity appears to alleviate morbidity and improve the outcome. In this article, we report our experience of a laparoscopic peritoneal lavage technique with delayed definitive resection when necessa…

Targeted helical CT of the acute abdomen: appendicitis, diverticulitis, and small bowel obstruction.

CT, especially helical CT, provides a fast and reliable modality for evaluation of the patient presenting with acute abdominal pain. Helical CT can provide an accurate diagnosis in the majority of patients and has found great utility in the evaluation of acute gastrointestinal emergencies, including acute appendicitis, diverticulitis, and small bowel obstruction. This article reviews proper helical CT technique, diagnostic imaging findings, and pitfalls of interpretation in evaluation of these acute abdominal disorders.

Neutrophil-to-lymphocyte ratio as a predictor of severity and adverse outcomes in complicated acute diverticulitis.

Complicated acute diverticulitis (CAD) requires accurate risk stratification to guide emergency management and surgical decision-making. Hemogram-derived inflammatory biomarkers, including the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), may provide rapid and accessible prognostic information. This study evaluated the diagnostic and prognostic utility of these biomarkers compared with the APACHE II score in patients with CAD. This retrospective study included 204 adult patients with CT-confirmed CAD treated at a tertiary referral center in Mexico between March 2…

An Umbrella Review of Systematic Reviews on the Utility of Antibiotics in Patients With Acute Uncomplicated Diverticulitis.

BACKGROUND: The role of antibiotics in acute uncomplicated diverticulitis (AUD) has been debated, as recent trials suggest antibiotics may not add benefit in mild AUD. Conclusions from systematic reviews on this subject have been inconsistent. This umbrella review aimed to synthesize and critically appraise the evidence from systematic reviews regarding the role of antibiotics in AUD. METHODS: This was a PRISMA-compliant systematic review on the outcome of nonantibiotic conservative management compared to standard antibiotic therapy for AUD. PubMed, Scopus, Cochrane Library, and Google Schola…

The diagnostic and therapeutic value of endoscopic irrigation for cecal suppurative diverticulitis: A case report.

RATIONALE: Cecal diverticulitis closely mimics acute appendicitis in clinical presentation, leading to frequent misdiagnosis. Traditional mistaken appendectomy is not only ineffective but may also delay proper treatment, risking serious complications such as perforation. Abdominal computed tomography plays a key role in differential diagnosis. This report explores the value of endoscopic irrigation as a novel diagnostic and therapeutic approach. PATIENT CONCERNS: A patient presented with symptoms strongly suggestive of acute appendicitis, prompting further diagnostic workup to rule out altern…

Development and internal validation of a nomogram to predict colorectal cancer risk in patients with diverticulitis.

AIM: Diverticulitis, considered a benign condition, has emerging evidence linking it to colorectal cancer (CRC). Current guidelines lack standardised risk stratification tools to guide post-diverticulitis surveillance. This study aims to create and validate a nomogram to predict CRC risk in patients with diverticulitis. METHOD: This retrospective cohort study included 1546 patients diagnosed with diverticulitis at a UK tertiary hospital between January 2021 and December 2023. Patients aged ≥ 18 years who underwent endoscopic assessment following a diverticulitis episode were included. A logis…

Clinical relevance of the 3-cm threshold in sigmoid diverticulitis with abscess: consensus or quandary?

PURPOSE: Diverticular abscess is a common manifestation of acute complicated diverticulitis. We aimed to analyze the clinical course of patients with diverticular abscess initially treated conservatively. METHODS: All patients with diverticular abscess undergoing elective or urgent/emergency surgery from October 2004 to October 2022 were identified from our institutional database. Depending on the abscess size, patients were divided into group A (≤ 3 cm) and group B (> 3 cm). Conservative treatment failure was defined as clinical deterioration, persistent or recurrent abscess, or urgent/emerg…

Clinical and Radiographic Characteristics in Segmental Colitis Associated With Diverticulosis, Diverticulitis, and Crohn's Disease.

BACKGROUND AND AIMS: Segmental colitis associated with diverticulosis (SCAD) is an underrecognized disease characterized by chronic segmental inflammation surrounding colonic diverticula. SCAD is recognized as an autonomous entity, although shares similar pathogenic and therapeutic aspects to inflammatory bowel disease and may be considered a complication of diverticulitis. We aimed to characterize the clinical, endoscopic, and radiographic findings of SCAD and its potential overlap with diverticulitis and inflammatory bowel disease. METHODS: All patients with suspected diagnosis of SCAD were…
