[Elbow dislocation in children and adolescents caused by sports].

In the Sportsclinic Stuttgart 11 young athletes were treated because of a dislocation of the elbow joint from 1988-1990. They were 7 girls and 4 boys with an average age of 12.5 years. Mechanism of the dislocation and its diagnosis and treatment are reported. Indication and ways for non-operative and operative treatment in the Sportsclinic Stuttgart are described. Operation should be performed on open injuries, concomitant vessel and nerve damages and dislocated bone fractures. Two typical case reports of young athletes are given.

Evaluation of loose bodies in the elbow with MR imaging.

The evaluation of loose bodies in the elbow is usually done by means of clinical examination, radiography, and postarthrographic computed tomography (CT). The authors review their experience with magnetic resonance (MR) imaging in place of post-arthrographic CT for the evaluation of loose bodies in the elbow. The prospective interpretation of MR studies of the elbow in 20 patients was compared with arthroscopic findings. All elbows were imaged in multiple planes with thin sections, surface coils, and combinations of T1, T2, and proton-density weighting. The sensitivity for showing loose bodies…

Valgus Extension Injuries of the Elbow in the Throwing Athlete.

Valgus extension injuries of the elbow are common among throwing athletes. There is a wide spectrum of these injuries, ranging from early soft-tissue inflammatory changes to attenuation and incompetence of the ulnar collateral ligament, as well as bone changes, including chondromalacia, osteochondritis, and osteophyte formation. Early treatment should be directed toward decreasing pain and inflammation, followed by global strengthening and conditioning of the elbow with special emphasis on plyometric strengthening of the flexor-pronator musculature. In patients who remain symptomatic after an …

Common nonarticular syndromes in the elbow, wrist, and hand.

Nonarticular elbow, wrist, and hand discomfort commonly results from periarticular or tendon disorders or nerve entrapment. Physicians with a practical knowledge of upper extremity anatomy and mechanics and an awareness of the typical features of disease are best equipped to treat these often-encountered conditions. Therapy is based on an accurate diagnosis and should be tailored to each disorder.

The influence of anatomic factors in elbow joint dislocation.

Twenty-seven patients with elbow joint dislocation were reviewed and treated between 1979 and 1983. For statistical reasons a sample of 27 elbows of healthy adults matched on age and sex were also roentgenographed. A computer-aided analysis of the carrying angle of the arm and the bow angle of the ulnar trochlear notch was made. The stability of the joint was tested clinically. The carrying angle showed no pathologic deviation. Values of the bow angle were at the lower end of the range given in the literature. Referring to these results, only normal anatomic constitutional factors predisposing…

Arthroscopy of the elbow. Diagnostic and therapeutic benefits and hazards.

We analyzed the results of seventy-one arthroscopies of the elbow in seventy patients who had been followed for an average of thirty-four months, in order to evaluate the risks and benefits of the procedure. Thirty-four arthroscopies were done for diagnostic purposes, fifteen were done for treatment, and twenty-two were done for both diagnosis and treatment. Overall, fifty-one (73 per cent) of the seventy patients benefited in some way. There were diagnostic benefits in thirty-six (64 per cent) of the fifty-six elbows in which diagnosis was an indication for the procedure, and there were thera…

Derangement of the articular surfaces of the elbow in young gymnasts.

We report the long-term follow-up of lesions of the articular surface of the elbow joint in a group of 12 young gymnasts. There was a high incidence of osteochondritic lesions, intraarticular loose bodies, and precocious signs of joint aging. The prognosis of elbow joint articular surface lesions in these athletes is not good, with only one athlete being able to continue high-level activity at maturity.

[Endoprosthesis of the elbow joint in sequelae of gunshot wounds].

5 patients have been implanted the endoprostheses of Sivash K. M. to restore functions of the upper limb after the gunshot injury in the area of elbow joint with the clinico-roentgenologic bone defect detection. Coarse cicatricial changes in the area of an articulation with presence of foreign bodies are characteristic for those patients. There have been achieved significant improvement of restoration of active movement of elbow joint and upper limb in whole with 4 of them in spite of high danger of possible post-operative inflammation. In one case due to hard physical labour there have been n…

Elbow dislocation with avulsion of the medial humeral epicondyle.

After dislocation of the elbow with avulsion of the medial epicondyle, the management of the latter is controversial. Of 28 children followed up after initial closed reduction of the elbow, 19 had a satisfactory closed reduction of the epicondyle and were treated in plaster. At follow-up, 11 children had a normal elbow and eight had lost an average of 15 degrees of flexion. Nine children had had open reduction and internal fixation of the fragment, one for an open injury, three for displacement of the epicondyle and six for intra-articular entrapment of the fragment. Five of these children had…

Osteoid osteoma of the elbow. A study of twelve cases.

The authors report 12 cases of osteoid osteoma of the elbow. They emphasize the peculiarity of the clinical, radiographic and histological pictures and discuss the criteria for differential diagnosis with other tumours, pseudotumours and inflammatory lesions of bone. The results obtained are discussed in the light of data emerging from a review of the literature on the subject.

The diagnosis and nonoperative treatment of elbow problems in the athlete.

An understanding of the anatomy and biomechanics of the elbow is essential for accurate diagnosis and treatment. In the athlete, the lateral compressive and medial traction forces account for the majority of problems. Lesions range from the valgus extension overloads to osteochondritis dissecans. Soft-tissue problems, be they "tennis elbow" or ulnar collateral ligament injuries, are frequently the greatest diagnostic and therapeutic challenge.

[Secondary ligament instabilities in the area of the elbow joint].

Secondary capsular or ligamentous injuries around the elbow joint, which are generally a rare finding, can give rise to medial, lateral, anterior, and posterior instability. Medial instability is especially bothersome during sporting activities, such as javelin throwing and baseball, and requires tightening or replacement of the ulnar collateral ligament. On the lateral side it is particularly the subluxation of the head of the radius that makes surgical tightening of the annular ligament essential. Anterior instability, which would cause the joint to dislocate anteriorly, is extremely rare an…

Neglected posterior dislocation of the elbow. A reconstruction operation.

A new technique for repair of neglected posterior dislocation of the elbow is described, consisting of open reduction with tendon graft stabilisation to create a medial collateral and an intra-articular "cruciate" ligament. This allows flexion-extension exercises to start six days after operation. Eleven cases are reported in which the average range of movement improved from 38 degrees before operation to 105 degrees after a minimum follow-up of two years, with a mean valgus-varus instability of only 13 degrees. One patient had a postoperative infection but all the others were satisfied with t…

Comparison of diflunisal and naproxen in the treatment of tennis elbow.

The authors favor conservative treatment of tennis elbow, starting with cessation of the offending activity and prescription of a nonsteroidal anti-inflammatory drug (NSAID) and followed by isometric and isotonic exercises when pain and inflammation have subsided. Two NSAIDs, diflunisal and naproxen, were evaluated in 38 patients with mild-to-moderate pain associated with tennis elbow. The physicians' assessments found no statistically significant differences between the two drugs; both provided effective reduction of pain and swelling. Patients' assessments of pain relief achieved with their …

Anatomical variations of the semilunar notch in elbow dislocations.

The anatomical relations of the semilunar notch of the ulna were studied in radiographs, taken in a strict lateral view, from 100 patients with elbow dislocations. The results were compared with those from a control group of 150 patients without dislocation or other radiographic pathology. We found that the coronoid process and an angle (a) formed between the axis of the ulna and a line going through the tip of the coronoid process and the tip of the olecranon were significantly smaller than those in the control group (P = 0.02). The central angle of the semilunar notch was significantly large…

[Fractures in the elbow joint area in childhood and adolescence].

73 cases of elbow joint fractures were treated in our Dep. of Paediatric surgery over a period of 10 years. Most of the fractures in the region of the distal humerus are localized at the ulnar epicondyle and the radial condyle. At the proximal forearm the radial head and the radial collum fractures are dominant in respect to olecranon fractures. Only two cases involved a comminuted fracture of the distal humerus. Due to the fact that only a small number of cases involving a fracture of the radial epicondyle after a luxation of the elbow were registered, it is possible that we paid no attention…
