Mesenchymal stem cell application in children with subacute sclerosing panencephalitis.

Subacute sclerosing panencephalitis (SSPE) is a serious, often fatal disease that responds poorly to current treatment modalities. Recently, the ability of mesenchymal stem cells (MSCs) to produce neurotrophic factors and inflammatory molecules has placed them among potential treatment agents for neurological conditions. We report the results of four patients treated with MSC for SSPE. The patients were followed up clinically, and by periodical laboratory evaluations, magnetic resonance imaging (MRI), and electroencephalography. One patient deteriorated to stage III of the disease, two patient…

A novel missense mutation in a neonate with nonketotic hyperglycinemia.

Nonketotic hyperglycinemia (OMIM #605899), also known as glycine encephalopathy, is an autosomal recessive disorder of glycine metabolism caused by a defect in the glycine cleavage system. A term neonate developed progressive lethargy, muscular hypotonia, and respiratory insufficiency on day 2 after birth, but no overt clinical seizures. Amplitude-integrated electroencephalography indicated a continuous burst-suppression pattern. The diagnosis of nonketotic hyperglycinemia was made biochemically and was confirmed by genetic studies, which revealed two missense mutations (one not previously des…

Regional cerebral hemodynamics during re-build-up phenomenon in childhood moyamoya disease. An analysis using 99mTc-HMPAO SPECT.

We investigated the role of ischemic hypoxia in the appearance of re-build-up phenomenon on electroencephalography in childhood moyamoya disease using 99mTc-hexamethyl propyleneamine oxime and single photon emission computed tomography (99mTc-HMPAO SPECT). In the case reported on, critical reduction of regional cerebral blood flow (rCBF) was observed during re-build-up phenomenon in the bilateral parieto-occipital area where acetazolamide testing revealed severe impairment of the perfusion reserve. On electroencephalography, the re-build-up phenomenon originated in these areas. In addition, re…

The electroencephalogram and acute ischemic stroke. Distinguishing cortical from lacunar infarction.

One hundred consecutive cases of clinically diagnosed, acute forebrain infarction were studied using computerized tomography (CT) and electroencephalography (EEG). Computed tomography confirmed brain infarction in 91 patients and was normal in the remaining nine. Each EEG was read independently by two experienced electroencephalographers without knowledge of CT or clinical details and their findings were compared with those of CT. Lateralized theta and/or delta activity predicted ipsilateral cortical infarction with a sensitivity of 76% and specificity of 82%. In contrast, cerebral hemisphere …

Depth of anesthesia monitoring: Current evidence, clinical impact, and future directions.

Depth of anesthesia monitoring has become an important component of modern anesthetic practice. Electroencephalography-based technologies, such as the bispectral index, entropy, and Narcotrend, allow clinicians to assess the level of consciousness more objectively and guide anesthetic titration more precisely. This narrative review summarizes current evidence on the relationship between depth of anesthesia monitoring and major perioperative complications. Available evidence suggests that maintaining an appropriate depth of anesthesia reduces the risk of intraoperative awareness, hypotension as…

Preliminary findings on a deep learning model using electroencephalogram for multi-level neuropathic pain detection in post-stroke patients.

AIM: Neuropathic pain occurs commonly after stroke and represents a major source of disability for affected patients. This study aims to develop an accurate and computationally efficient framework for multi-level neuropathic pain detection using electroencephalography signals. METHODS: A Quantum-Inspired Pyramid Depthwise Separable Residual Network is proposed, which integrates three innovations: a depthwise separable Residual Network to reduce computational complexity, a pyramid attention mechanism to capture multi-scale patterns, and a quantum-inspired transformation layer to model complex …

Interrater Reliability of Electroencephalogram Interpretations in Febrile Comatose African Children: Implications for Automated Interpretation Algorithm Creation.

Electroencephalography (EEG) is a diagnostic and prognostic tool used worldwide in the clinical care of comatose patients. Scalability of EEG use in resource-limited settings is constrained by multiple factors, including the lack of neurophysiologist interpreters. Although machine learning offers a path to creation of automated interpretation algorithms, development is constrained by the need for independent inputs from multiple neurophysiologists. Development may be accelerated if independent inputs have high concurrence. We assessed interrater reliability of key EEG variables from studies re…

Multivariate Pattern Analysis Identifies Potential Intertrial Resting-State EEG Biomarkers in Fibromyalgia.

Fibromyalgia involves widespread musculoskeletal pain and hypersensitivity, often accompanied by neurological, cognitive, and affective disturbances. Resting-state electroencephalography studies have revealed abnormal brain activity in chronic pain conditions, with anxiety and symptom duration potentially exacerbating these alterations. This study applied multivariate pattern analysis to differentiate intertrial resting-state electroencephalography signals between fibromyalgia patients and healthy controls across frequency bands associated with pain processing, incorporating state and trait an…

Exploring effects of anesthesia on complexity, differentiation, and integrated information in rat EEG.

To investigate mechanisms underlying loss of consciousness, it is important to extend methods established in humans to rodents as well. Perturbational complexity index (PCI) is a promising metric of "capacity for consciousness" and is based on a perturbational approach that allows inferring a system's capacity for causal integration and differentiation of information. These properties have been proposed as necessary for conscious systems. Measures based on spontaneous electroencephalography recordings, however, may be more practical for certain clinical purposes and may better reflect ongoing …

Electroencephalographic lateralization, clinical correlates and pharmacological response in untreated Parkinson's disease.

BACKGROUND: In Parkinson's disease (PD), different topographically defined cortical-subcortical oscillatory networks have been implicated in motor program dysfunction. Few studies have focused on clinical correlates of cortical activity asymmetry using quantitative electroencephalography. METHODS: We retrospectively selected N = 34 L-dopa naïve PD subjects who had undergone standardized electroencephalography. We selected N = 18 subjects group-matched by age, sex and hand dominance with normal electroencephalography and no parkinsonism and/or cognitive decline as controls. A Welch's periodogr…

Episodic tremors representing cortical myoclonus are characteristic in Angelman syndrome due to UBE3A mutations.

OBJECTIVE: Neurological manifestations including psychomotor developmental delay and epilepsy in patients with Angelman syndrome caused by ubiquitin protein ligase E3A (UBE3A) mutations has been considered similar but is relatively milder than that in patients with deletion-type Angelman syndrome. This makes the diagnosis of the former subgroup often difficult. We here characterized epilepsy, specifically the types of tremulous movement, in 4 patients (age, 3-38years) with Angelman syndrome caused by UBE3A mutations. METHODS: Ictal electroencephalography was used to record episodic tremors in…

Hypothalamic Hamartoma With Infantile Spasms: Case Report With Surgical Treatment.

We report a 10-month-old boy with treatment-resistant infantile spasms associated with hypothalamic hamartoma (HH). Electroencephalography before surgical treatment showed modified hypsarrhythmia. Transventricular endoscopic resection and disconnection resulted in immediate and enduring disappearance of the epileptic spasms and improvement in the postoperative electroencephalography. After 8 years of treatment, the patient has nondisabling gelastic seizures associated with a small amount of residual HH but no other seizure types. He is not taking any antiepilepsy drugs. He is academically and …

Electroencephalographic signatures of pain and analgesia in rats.

Pain modulates rhythmic neuronal activity recorded by Electroencephalography (EEG) in humans. Our laboratory previously showed that rat models of acute and neuropathic pain manifest increased power in primary somatosensory cortex (S1) recorded by electrocorticography (ECoG). In this study, we hypothesized that pain increases EEG power and corticocortical coherence in different rat models of pain, whereas treatments with clinically effective analgesics reverse these changes. Our results show increased cortical power over S1 and prefrontal cortex (PFC) in awake, freely behaving rat models of acu…
