PURPOSE OF REVIEW: This article reviews the evidence base and recommendations for medical management for secondary stroke prevention.
RECENT FINDINGS: Recent developments for secondary stroke prevention include evidence to support the use of short-term dual antiplatelet therapy after minor stroke and transient ischemic attack, direct oral anticoagulants for nonvalvular atrial fibrillation, reversal agents for direct oral anticoagulant-associated hemorrhage, and aspirin rather than presumptive anticoagulation with a direct oral anticoagulant for embolic stroke of undetermined source.

BACKGROUND: Economic and social disparities in health care contribute to differential outcomes among patients with acute stroke. A patient's insurance status reflects access to care and serves as a surrogate of social determinants of health, which can be targeted for intervention. We examined the relationship between insurance status and risk of recurrent stroke, major bleeding or death following an embolic stroke of undetermined source (ESUS).
METHODS: This is a secondary analysis of a United States multicenter (n=27) retrospective observational cohort of patients with ESUS, which examined t…

Neurological complications occurring in patients with infective endocarditis (IE) are a major determinant of morbidity and mortality. Among these, ischemic stroke secondary to septic embolism is the most common and often results from embolization of friable vegetations into the cerebral circulation. Such events typically occur early in the disease course, frequently within the first two weeks of antimicrobial therapy. The embolic material not only causes vascular occlusion but may also introduce infection into cerebral tissue, increasing the risk of secondary abscess formation, meningitis, and

About 25% of ischaemic strokes are of cryptogenic origin and a significant proportion of them has a certain embolic nature, and for these patients the term embolic stroke of undetermined source (ESUS) has been coined. In the absence of subclinical atrial fibrillation (AF) identifiable through prolonged electrocardiogram monitoring, atrial cardiomyopathy, demonstrable through non-invasive cardiac imaging, aortic plaques and heart failure with preserved sinus rhythm, have been recognized among the potential causes of ESUS. In patients with ESUS, randomized clinical trials performed so far have f…

BACKGROUND: Middle cerebral artery (MCA) stenosis is a rare occlusive disease in western populations, with a high risk of stroke recurrence. An artery-to-artery embolic mechanism has been argued. We report the detection of a new pattern of microembolic signals (MES) in the MCA poststenotic segment in a patient with multiple recurrent transient ischemic attacks.
CASE DESCRIPTION: A 75-year-old man was admitted to our hospital with a transient leg monoparesis on the left side. Right MCA stenosis was detected by transcranial Doppler ultrasonography (TCD). In spite of aspirin treatment, the patie

We performed 123I-IMP single photon emission computed tomography (SPECT) in 43 patients who had a small infarction (less than 2 cm) in subcortical area and who were less than 1 month after onset of stroke. Hypoperfused area of brain was qualitatively assessed and was compared with functional outcome at 6-month after the stroke and the cerebral angiogram. Functional outcome was poorer in patients who had wider hypoperfused area in brain (chi 2 = 29.3; p less than 0.001). The extent of brain hypoperfused area showed a positive correlation with the degree of stenosis in the extracranial and/or in

Some patients develop ischemic stroke despite taking direct oral anticoagulants because of the presence of other risk factors such as coagulopathies. A 65-year-old male patient with non-valvular atrial fibrillation (NVAF) taking rivaroxaban was diagnosed as having embolic stroke and antithrombin-III (AT-III) deficiency. Echocardiography revealed a thrombus in the left atrial appendage (LAA). He was prescribed warfarin, and after resolution of the thrombus, we successfully performed percutaneous LAA closure (LAAC), with no subsequent recurrence or device-related thrombosis. Warfarin and LAAC ma…

Neurological deterioration or new focal deficits in patients with primary brain tumors are usually related to intratumoral hemorrhage, disease progression, seizures (Todd paralysis) and, rarely, ischemic stroke. Ischemic strokes in this group of patients are usually a postoperative complication, a long-term result of radiation vasculopathy, embolic due to hypercoagulability and, less commonly, caused by vessel occlusion by an adjacent brain tumor. We report a rare case of ischemic stroke secondary to a newly diagnosed high-grade glioma and the possible mechanisms that resulted in this medical

BackgroundThromboembolic events remain a major cause of morbidity and mortality in cardiovascular diseases. Asundexian, an oral factor XIa inhibitor, has been proposed to reduce thrombosis while minimizing bleeding risk. The recent OCEANIC-STROKE trial provided substantial new evidence. We conducted an updated systematic review and meta-analysis incorporating this large trial.MethodsPubMed, Scopus, embase and CENTRAL were searched through April 2026. Five RCTs (n=31,301) comparing asundexian 50 mg daily with placebo or apixaban in patients with AF, acute myocardial infarction or stroke were in

OBJECTIVE: The aim of this study was to investigate the correlation between shaggy aorta and embolic complications during thoracic endovascular aneurysm repair (TEVAR), based on a shaggy aorta scoring system.
METHODS: The entire aorta was assessed based on 5 mm slice computed tomography (CT) from the sinotubular junction to the aortic bifurcation using a three dimensional workstation. One shaggy point (shaggy score) was given when the following conditions were met: 1) ulcer like thrombus, 2) maximum thrombus thickness ≥ 5 mm, and 3) mural thrombus occupies more than two thirds of the circumfe

BACKGROUND: S-Nitrosoglutathione (GSNO) is a nitric oxide donor that has been investigated for neuroprotective and neuro-recovery effect. We aimed to conduct a systematic review on the published literatures using GSNO in both pre-clinical and clinical stroke studies.
METHODS: We searched PubMed up to June 30, 2019, using the following keywords: S-Nitrosoglutathione, GSNO, stroke, cerebrovascular, carotid arteries, middle cerebral artery, and middle cerebral artery occlusion. Only studies published in the English language providing efficacy results of GSNO on ischemic stroke were included. Str

Atrial fibrillation (AF) can be detected in nearly 25% of all patients with stroke by sequentially combining different electrocardiographic methods. Prediction of early cardio-embolic stroke remain a permanent challenge in everyday practice. The early identification of an increased risk for atrial fibrillation episodes (which are frequently asymptomatic) is essential for the prevention of cardioembolic events. One of the noninvasive modalities of atrial fibrillation prediction is represented by the electrocardiographic P-wave analysis. This includes study and diagnosis of interatrial conductio…

Despite many advances in our understanding of ischemic stroke, cryptogenic strokes (those that do not have a determined etiology) remain a diagnostic and therapeutic challenge. Previous classification approaches to cryptogenic stroke have led to inconsistent definitions, and evidence to determine optimal treatment is scarce. These limitations have prompted international efforts to redefine cryptogenic strokes, leading to more rigorous diagnostic criteria, outcome studies, and new clinical trials. Improvement in our ability to detect paroxysmal atrial fibrillation in patients with cryptogenic s

Stroke in pediatric patients occurs with a frequency of 3 to 8 per 100,000. The postevent evaluation attempts to identify the etiology of ischemia whether anatomic, hematologic, or embolic, with the intention of preventing future events. We present the case of a previously healthy male who developed unilateral facial and extremity weakness 2 weeks after receiving an appendectomy. Once the usual etiologies of stroke in pediatric patients were excluded, an evaluation of the peripheral venous catheter (placed for postoperative antibiotic delivery) demonstrated arterial misplacement. Spontaneous dissection of the extracranial vertebral artery (VA) may cause ischemic stroke in the posterior circulation. A 22-year-old female and a 38-year-old male presented with sudden onset of vertigo and nausea without trauma. Angiography was initially interpreted as normal, but retrospective examination disclosed extracranial VA dissection in the V3 segment in both cases. Arterial dissection resulting in embolic stroke in the territory of the ipsilateral posterior inferior cerebellar artery was highly suspected. Both patients were treated conservatively without sequelae. Careful angiograp…

A 46-year-old woman presented for a routine echocardiogram, which revealed a giant atrial septal aneurysm (ASA) with an atrial septal defect (ASD). We surgically excised the ASA and closed the ASD with a pericardial patch. ASA, although rare, poses embolic risks, thus necessitating closure when associated with shunts. Optimal stroke prevention remains uncertain.

Transcatheter occlusion of patent foramina ovalia and atrial septal defects in 10 patients with presumptive paradoxic embolic stroke using the buttoned device appears to be encouraging, with safety and efficacy demonstrated during a follow-up of up to 60 months. Anticoagulation should be continued until complete disappearance of the right-to-left atrial shunt.

BACKGROUND Cardioversion is a medical procedure aimed to convert abnormal rhythm to normal rhythm in hemodynamically unstable patients in ventricular tachycardia. The complications of cardioversion are mainly thromboembolism, with stroke being the most common complication. However, emboli can travel anywhere, including to the coronary arteries. In the literature, the complications highlighted are usually tailored towards atrial fibrillation. Indeed, acute embolic myocardial infarction following direct-current cardioversion of ventricular tachycardia has not been identified in the literature. T

BACKGROUND: Atrial myxoma (AM) is responsible for multiple serious complications, such as ischemic stroke, and requires cardiac surgery. However, diagnosis and, thus, treatment may be delayed by an initial misleading clinical presentation including nonspecific extracardiac symptoms. Involvement of the peripheral nervous system as a first symptom of AM has been reported only once before and never in the form of multiple mononeuropathy.
CASE: We report the case of a 51-year-old woman who had suffered from pharmacoresistant neurological pain sequentially in several peripheral nerve territories f

BACKGROUND: Although most stroke patients have underlying vascular risk factors, it is important to consider infectious causes of stroke in young adults without traditional risk factors or patients with cryptogenic stroke. Pulmonary vein thrombosis and air embolism can potentially cause cerebral infarction. However, the association between infection and pulmonary vein thrombosis or air embolism is often overlooked. In this case, we present a rare infectious cause of stroke and air embolism involving a pulmonary abscess and pulmonary vein thrombosis.
CASE PRESENTATION: A 37-year-old male patie
