Transtympanic gentamicin in the treatment of Meniére's disease.

Aminoglycosides enable a pharmacological approach, before resorting to surgery, to the treatment of Menière's disease (MD) proving unamenable to routine medical treatment. The staging of MD is an essential preliminary step in the implementation of a specific and personalized treatment with transtympanic gentamicin. The ideal condition is the presence of endolymphatic hydrops with little or no damage to the cochlear and vestibular hair cells. The technical aspects of the method are discussed. In 21 patients with unilateral Menière's disease in various stages, this treatment achieved the regress…

A new aspect in pathogenesis of experimental hydrops: role of calcium.

An imbalanced Ca++-homeostasis in the inner ear was demonstrated using an animal model for Menière's disease (MD). An increase of Ca++ concentration in the endolymph, as well as in some cells of the inner ear tissue, causes a rise of osmotic pressure and decrease of electric potential. Based on these results, we assume a new aspect in the pathophysiology of MD. It is proposed that the common denominator of MD, experimental endolymphatic hydrops (EEH), and space motion sickness (SMS) is primarily a shift of the inner ear Ca++ homeostasis towards a higher concentration of free Ca++ in the fluid …

Idiopathic perilymphatic fistulae: a temporal bone histopathological study. Clinical, surgical and histopathological correlations.

The temporal bone histopathological findings herein reported are the first to be from a patient who had, during life, the diagnosis of idiopathic perilymphatic fistulae. The patient received surgical treatment followed by rapid improvement of his vestibular symptoms. These specimens histopathologically demonstrate the patencies of the labyrinth capsule that were diagnosed (predicted) clinically during life, the qualities of the membranous labyrinth in both the operated and unoperated ears, and the elements of the surgical repair. Notably there is no evidence of endolymphatic hydrops. The diagn…

Clinical significance of posterior expansion of the utricular endolymphatic space.

BACKGROUND: Posterior expansion of the utricular endolymphatic space (ES) is a finding occasionally observed, and often occurs in bilateral ears. The clinical significance of posterior expansion of the utricular ES is not clear. OBJECTIVES: To investigate the clinical significance of posterior expansion of the utricular ES detected on magnetic resonance imaging (MRI). MATERIALS AND METHODS: Among 957 patients who underwent contrast-enhanced MRI to investigate the presence of endolymphatic hydrops (EH), clinical findings in cases with posterior expansion of the utricular ES were evaluated and…

Meniere's disease of syphilitic etiology.

Congenital or acquired syphilis is the cause of Meniere's disease in six percent of all cases. The pathogenesis is endolymphatic hydrops and osteitis of the otic capsule which produces the characteristic fluctuating hearing loss, tinnitus and ear pressure associated with episodic spells of vertigo. The ear symptoms are, at first, typical of Meniere's disease of other etiologies. Failure to establish the specific etiologic diagnosis can result in unnecessary surgical treatment or inappropriate medical therapy. In some cases, if a steroid, the specific treatment, is not given promptly, hearing c…

Pathologic significance of Meniere's symptom complex. A histopathologic and electron microscopic study.

Histologic and ultrastructural findings in the temporal bones of a patient clinically diagnosed as having unilateral Meniere's disease are presented. The patient underwent two endolymphatic subarachnoid shunt operations that failed to relieve symptoms and a middle fossa vestibular nerve section that completely relieved episodic vertigo and stabilized hearing in the ear operated upon. Histopathologic examination of the temporal bones showed no endolymphatic hydrops in either ear. The maculae and cristae on the side operated upon showed severe degenerative changes attributed to vestibular neurec…

The effects of indomethacin on inner ear fluids and morphology.

Metabolic disturbances may affect inner ear fluid homeostasis in various ways. The membrane sodium-potassium pump must play a role in this h … on? Further, could energy system problems involving prostaglandins be related to the development of endolymphatic hydrops? These questions prompted this study. Sublethal doses of Indomethacin were given to 14 guinea pigs in order to maximally suppress prostaglandins. Samples of perilymph and endolymph were analyzed for sodium and potassium concentrations and compared t …

Morphometric Evaluation of the Facial and Vestibulocochlear Nerves Using MR Imaging in Patients with Menière Disease.

BACKGROUND AND PURPOSE: Menière disease (MD) is a condition of unknown etiology, involving genetic predisposition, autoimmune processes, vir … m duration. This finding contrasts with previous ones of correlations between clinical features and endolymphatic hydrops. A disease process starting before clinical symptom onset could be a possible explanation. Morphometric evaluation of brain nerves using hydrops MRI sequences is practical and provides similar results compared with T2-weighted imaging, improving pat …

Ménière's Disease Pathophysiology: Endolymphatic Sac Immunohistochemical Study of Aquaporin-2, V2R Vasopressin Receptor, NKCC2, and TRPV4.

Objectives Endolymphatic sac (ELS) pathophysiology in Ménière's disease (MD) remains poorly understood. We identified from the literature a group of proteins expressed on the ELS and involved in endolymph volume regulation: aquaporin-2 (AQP2), vasopressin receptor V2R, sodium potassium chloride cotransporter 2 (NKCC2), and transient receptor potential cation channel V4 (TRPV4). Our objective was to determine whether their ELS expression was altered in MD, to better understand the pathophysiology of endolymphatic hydrops. Study Design Prospective case-control study. Setting Tertiary care center…

Automated measurement of hydrops ratio from MRI in patients with Ménière's disease using CNN-based segmentation.

Ménière's Disease (MD) is difficult to diagnose and evaluate objectively over the course of treatment. Recently, several studies have reported MD diagnoses by MRI-based endolymphatic hydrops (EH) analysis. However, this method is time-consuming and complicated. Therefore, a fast, objective, and accurate evaluation tool is necessary. The purpose of this study was to develop an algorithm that can accurately analyze EH on intravenous (IV) gadolinium (Gd)-enhanced inner-ear MRI using artificial intelligence (AI) with deep learning. In this study, we developed a convolutional neural network (CNN)-b…

Diagnosis and management of cochlear hydrops.

In an early case of cochlear hydrops, the symptoms of a mild pressure sensation or fullness in the involved ear may be mistaken for a blocked eustachian tube. More severe cases demonstrate a true sensori-neutral hearing loss that fluctuates. Tinnitis is usually present during the attack and will often subside when the hearing returns to normal. The neurotologic evaluation is negative and attempts to establish an etiology are seldom fruitful. The treatment is usually medical, and empirically, these individuals are treated with diuretics, low salt diet, and vasodilators. It is felt by most inves

Post-traumatic hydrops.

Post-traumatic Meniere's disease is a true clinical entity based on clinical observations and histopathologic study. The traumatic insult may be a blow to the head, a temporal bone fracture, infectious sequelae of measles, mumps, syphilis, or even acoustic trauma. Although there are many cases of Meniere's disease for which no definite etiologic agent can be identified, a detailed clinical history is the only way to identify possible etiologic causes for Meniere's disease. In all forms of post-traumatic Meniere's disease, the symptoms manifest in a delayed fashion following the insult, usually

Occlusion of the Lateral Semicircular Canal, Endolymphatic Sac Surgery, and Cochlear Implantation: A Low Destructive Treatment for Unilateral Ménière's Disease and Deafness.

INTRODUCTION: Surgical treatment of Ménière's disease (MD) and deafness aims to treat vertigo and hearing disabilities. Current treatment options like labyrinthectomy and cochlear implantation (CI) have shown acceptable results but are destructive. Less destructive procedures, like the occlusion of the lateral semicircular canal and endolymphatic sac surgery, have been shown to be successful in vertigo control. The combination of both procedures with CI has not been investigated; therefore the objective of this study was to investigate the outcome of this combination in patients with single-si
