ARBOALVO: A Bayesian spatiotemporal learning and predictive model for dengue cases in the endemic Northeast city of Natal, Rio Grande do Norte, Brazil.

BACKGROUND: Urban arbovirus transmission is spatially and temporally heterogeneous. Estimating the risk of dengue through statistical models that consider simultaneous variability in space and time provides more realistic estimates of transmission dynamics, facilitating the identification of priority areas for intervention focused on surveillance and control. These models also enable predictions to support timely interventions for arboviruses like dengue, chikungunya, and Zika. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed dengue case reports by epidemiological week and neighborhood in Natal, R

Influenza A virus during pregnancy disrupts maternal intestinal immunity and fetal cortical development in a dose- and time-dependent manner.

Epidemiological studies link exposure to viral infection during pregnancy, including influenza A virus (IAV) infection, with increased incidence of neurodevelopmental disorders (NDDs) in offspring. Models of maternal immune activation (MIA) using viral mimetics demonstrate that activation of maternal intestinal T helper 17 (TH17) cells, which produce effector cytokine interleukin (IL)-17, leads to aberrant fetal brain development, such as neocortical malformations. Fetal microglia and border-associated macrophages (BAMs) also serve as potential cellular mediators of MIA-induced cortical abnorm

A Coupled Statistical and Deterministic Model for Forecasting Climate-Driven Dengue Incidence in Selangor, Malaysia.

The mosquito-borne dengue virus remains a major public health concern in Malaysia. Despite various control efforts and measures introduced by the Malaysian Government to combat dengue, the increasing trend of dengue cases persists and shows no sign of decreasing. Currently, early detection and vector control are the main methods employed to curb dengue outbreaks. In this study, a coupled model consisting of the statistical ARIMAX model and the deterministic SI-SIR model was developed and validated using the weekly reported dengue data from year 2014 to 2019 for Selangor, Malaysia. Previous stu

Long-term clinical impact of introducing a human papillomavirus 16/18 AS04 adjuvant cervical cancer vaccine in Spain.

BACKGROUND: Human papillomavirus (HPV) epidemiology and screening practices vary considerably between countries and specific analyses are required to estimate the impact of HPV vaccination. This study aimed to predict the clinical benefits of introducing a bivalent HPV16/18 vaccine in Spain, where the cervical cancer (CC) incidence is 10.3 per 100 000. METHODS: A Markov model based upon the natural history of HPV and CC was developed to simulate transitions between health states, in the presence of specific screening programmes. Published data were used to reflect the Spanish situation in ter

Modelling response strategies for controlling gonorrhoea outbreaks in men who have sex with men in Australia.

The ability to treat gonorrhoea with current first-line drugs is threatened by the global spread of extensively drug resistant (XDR) Neisseria gonorrhoeae (NG) strains. In Australia, urban transmission is high among men who have sex with men (MSM) and importation of an XDR NG strain in this population could result in an epidemic that would be difficult and costly to control. An individual-based, anatomical site-specific mathematical model of NG transmission among Australian MSM was developed and used to evaluate the potential for elimination of an imported NG strain under a range of case-based

Tobacco-driven digestive disease burden: A China-US comparative analysis, 1990-2035.

INTRODUCTION: Smoking is a well-established risk factor for digestive diseases, yet direct comparisons of smoking-attributable digestive disease burden between nations with divergent tobacco control trajectories remain scarce. This study compared temporal trends, driving factors, and future projections of this burden in China and the United States from 1990 to 2023. METHODS: Using Global Burden of Disease 2023 data, we analyzed smoking-attributable digestive disease deaths and disability-adjusted life years. Joinpoint regression assessed temporal trends. Decomposition analysis partitioned bur

Environmental and epidemiological drivers of dengue forecasting across heterogeneous transmission settings in Brazil and Panama.

BACKGROUND: Dengue fever is the fastest-growing mosquito-borne viral disease globally, with unprecedented epidemics reported across the Americas in 2024. Brazil and Panama experienced record transmission that strained public health systems and highlighted limitations in existing surveillance and early warning approaches. Despite growing evidence that climatic variability, land-use change, and other environmental factors influence dengue transmission, scalable approaches integrating heterogeneous environmental datasets remain limited. METHODS: We integrated climatic, environmental, demographic

A preregistered simulation study provided evidence on the appropriate use of data-driven variable selection in descriptive multivariable regression modeling.

OBJECTIVES: Multivariable regression modeling is central in epidemiological research. Common practice often uses data-driven variable selection to identify the variables most strongly associated with the outcome (the goal of descriptive modeling), but this may lead to omission of relevant covariates, inclusion of noise variables, biased coefficient estimates, and unstable models. Although methodological recommendations exist, guidance based on systematic and neutral comparison studies remains limited. Therefore, we aimed to provide evidence-based guidance on the appropriate use of data-driven

A climate-informed dengue transmission model with Bayesian decision support.

BACKGROUND: It is crucial to model the spread of dengue under different weather conditions so that we can understand how it changes with the seasons and plan targeted actions. Conventional deterministic models frequently overlook the time-varying impact of environmental factors, hence constraining their forecast accuracy and policy significance. METHODS: This research formulates a non-autonomous, climate-sensitive SEIR-SI dengue model with crucial parameters like transmission rate, vector mortality, and recruitment are articulated as explicit functions of temperature, humidity, and precipitat

INTRODUCTION: Since the first discovery and reporting of the COVID - 19 pandemic towards the end of 2019, the virus has rapidly propagated across the world. This has led to a remarkable spike in the number of infections. Even now, doubt lingers over whether it has completely disappeared. Moreover, the issue of restoring normal life while ensuring safety continues to be a crucial challenge that public health agencies and people globally are eager to tackle. METHODS: To thoroughly understand the epidemic's outbreak and transmission traits and formulate timely prevention measures to fully safegu

How do recurrent malaria infections occur in clinical cohorts: a mathematical modelling study to support study planning.

BACKGROUND: Recurrent events of infectious diseases are common and the subject of analyses in many clinical studies. A proper understanding of disease occurrence over time within a cohort provides a basis for study planning and sample size estimation. This study mathematically describes the recurrence of malaria in a malaria-naïve cohort and highlights the necessary assumptions to inform study planning. METHODS: To represent different disease transmission scenarios, five mathematical models with different levels of complexity were constructed to mimic possible real-life scenarios. Model A rep

Early Detection of Dengue Outbreaks: Transmission Model Analysis of a Dengue Outbreak in a Remote Setting in Ecuador.

BACKGROUND: Pathogen transmission of an outbreak generally begins well before it is identified by a surveillance system, particularly for infectious diseases in which a high proportion of cases are subclinical, as is the case for arboviruses. We aimed to ascertain the most likely date of the primary case (the first infection, whether detected or not) in an outbreak. METHODS: Using data from a 2019 dengue outbreak in a rural, riverine town in Northwestern Ecuador, we investigated potential undetected dengue virus transmission before the outbreak detected in mid-May. The outbreak was preceded b

Optimization of testing protocols to screen for COVID-19: a multi-objective model.

In this paper we develop a new multi-objective simulated annealing (MOSA) algorithm to generate optimal testing protocols for infectious diseases, using the COVID-19 pandemic as our context. A SEIR (susceptible-exposed-infected-recovered) epidemiological model is embedded as the computational platform for our MOSA algorithm to optimize testing protocols for screening across three joint objectives: minimum cost of test materials, minimum total infections over the testing horizon, and minimum number of false negatives over the horizon. We demonstrate the application of this optimization tool to

Quantifying the Risk and Cost of Active Monitoring for Infectious Diseases.

During outbreaks of deadly emerging pathogens (e.g., Ebola, MERS-CoV) and bioterror threats (e.g., smallpox), actively monitoring potentially infected individuals aims to limit disease transmission and morbidity. Guidance issued by CDC on active monitoring was a cornerstone of its response to the West Africa Ebola outbreak. There are limited data on how to balance the costs and performance of this important public health activity. We present a framework that estimates the risks and costs of specific durations of active monitoring for pathogens of significant public health concern. We analyze d

Predictors of 25-hydroxyvitamin D and its association with risk factors for prostate cancer: evidence from the prostate testing for cancer and treatment study.

PURPOSE: Circulating 25-hydroxyvitamin D (25(OH)D) may protect against prostate and other cancers. Few epidemiology studies have measured 25(OH)D on all participants, weakening the evidence-base through reduced statistical power and the potential for bias. We developed a score to predict individual 25(OH)D based on potential predictors, including sun exposure, nutrient intake, and vitamin D pathway genes, providing a method of substituting missing values. We assessed the usefulness of predicted 25(OH)D by comparison with multiple imputation of 25(OH)D levels. METHODS: Amongst 1,091 controls f

Potential assessment of a neural network model with PCA/RBF approach for forecasting pollutant trends in Mong Kok urban air, Hong Kong.

The forecasting of air pollutant trends has received much attention in recent years. It is an important and popular topic in environmental science, as concerns have been raised about the health impacts caused by unacceptable ambient air pollutant levels. Of greatest concern are metropolitan cities like Hong Kong. In Hong Kong, respirable suspended particulates (RSP), nitrogen oxides (NOx), and nitrogen dioxide (NO2) are major air pollutants due to the dominant usage of diesel fuel by commercial vehicles and buses. Hence, the study of the influence and the trends relating to these pollutants is

Poliovirus surveillance by examining sewage water specimens: studies on detection probability using simulation models.

Efficiency of environmental surveillance of poliovirus circulation was studied using simulation models. First, three transmission models were defined for describing different scenarios of poliovirus infections in a large unstructured population. Second, environmental factors, such as the total volume of the sewage network and losses of viruses, were modeled for computing the virus output at the sewage sampling site. Third, the effect of sampling and laboratory procedures was accounted for in the probability of detection, given the amount of polioviruses in a specimen. The simulation model can
