Malformations of cortical development and epilepsy.

Malformations of cortical development (MCDs) are macroscopic or microscopic abnormalities of the cerebral cortex that arise as a consequence … enes. This review will highlight the best-known of the human cortical malformations associated with epilepsy. The pathological, clinical, imaging, and etiologic features of each MCD will be summarized, with representative magnetic resonance imaging (MRI) images shown for each MCD. The malformations tuberous sclerosis, focal cortical dysplasia, hemimegalenc …

Issues for mature women with epilepsy.

Specific concerns regarding mature women with epilepsy (WWE), specifically epilepsy-associated issues during perimenopause, menopause, and postmenopause, have been emerging in the epilepsy community. This chapter presents evidence that for WWE, seizure frequency may increase during perimenopause and decrease at postmenopause, especially if a catamenial epilepsy pattern was present during the reproductive years. This finding implies that, as in other age groups, a subset of mature WWE are particularly susceptible to endogenous reproductive hormonal changes. An adverse effect on seizure frequenc…

Sexual dysfunction in women with epilepsy.

Sexual dysfunction in women with epilepsy (WWE) is an important comorbidity. A significant minority of WWE have markedly decreased sexual interest and it appears that orgasmic dysfunction occurs more frequently in WWE than in control women. Enzyme-inducing antiepileptic drugs can adversely affect sexual functioning by decreasing bioactive testosterone levels. Temporal lobe epilepsy of right-sided versus left-sided origin may also be a risk factor for sexual dysfunction. In addition to these factors, emerging evidence suggests that the serotonin transporter protein is related to temporal lobe e…

The representation of epilepsy in popular music.

Much can be learned about the contemporary stereotypes associated with epilepsy by studying the representation of the disorder in paintings, literature, and movies. Popular music is arguably the most accessible and ubiquitous of the creative art forms, touching most of us on a daily basis. Reviewed here are the ways in which epilepsy and seizures are used in the lyrics of musicians from a wide variety of musical genres, from hip-hop to rhythm and blues. Many of the ancient associations of epilepsy with madness, horror, and lunacy can be found in these lyrics. However, the language of epilepsy …

[Management of epilepsy in developing countries].

Epilepsy is a major public health problem in developing countries. In addition to the high prevalence of epilepsy in some regions, there is an acute shortage of trained personnel, clinical equipment, and antiepileptic drugs. As a result of these problems, most patients do not have access to adequate treatment. Despite wake-up calls from the WHO, LICE, and BIE in 2001 using the slogan "Bring epilepsy out of the shadows", few national epilepsy control programs have been implemented in developing countries. Nevertheless reports describing several recent pilot projects have demonstrated that effec…

[A descriptive study of childhood epilepsy].

AIM: To analyse the epidemiological characteristics and the relative distribution of the different types of epilepsy and epileptic syndromes in the childhood population. PATIENTS AND METHODS: We reviewed the medical histories of 365 patients (192 males and 173 females) with epilepsy. Epidemiological and clinical data were collected, together with information from complementary examinations. The diagnostic criteria applied were those of the International League Against Epilepsy (ILAE). RESULTS: The mean age at diagnosis was 5.9 years and the mean follow-up time was 4.6 years. A personal and/o…

Menstrual disorders in women with epilepsy.

Women with epilepsy are more likely to have menstrual disorders than women in the general population. Estimates vary because of different definitions of menstrual disorder. Our best estimate is that perhaps one of every three women with epilepsy may be affected compared with one of seven in the general population. Menstrual disorders are significant because they are associated with anovulatory cycles that may increase the risks for infertility, migraine, emotional disorders, and female cancers. They are neurologically important because they are associated with greater seizure frequency. Increa…

Treatment of epilepsy in Rett syndrome.

INTRODUCTION: Epilepsy is very frequent in Rett syndrome (RTT) patients and often difficult to treat. Because most cases of RTT are caused by mutations in the MECP2 gene it is reasonable to assume that convulsions are based on common pathogenetic mechanisms and thus should have a similar response to antiepileptic drugs. PURPOSE: To find the optimal treatment for epilepsy in RTT. METHODS: We performed a retrospective study on 110 female patients with confirmed MECP2 mutations. RESULTS: The median age was 10 years, 58% had a history of epilepsy and 55% received antiepileptic drugs (AEDs). Onl…

Acquired encephaloceles and epilepsy in osteopetrosis.

Osteopetrosis is a condition in which there is a defect in bone resorption by osteoclasts. With thickening of the skull and skull base, the cranial capacity becomes compromised and skull foramina gradually occlude, resulting in a wide range of neurological symptoms and signs. We present a case of autosomal dominant osteopetrosis with temporal lobe epilepsy and nasal obstruction due to acquired bifrontal encephaloceles associated with a decreased intracranial capacity. Neurosurgical reconstruction of the frontal skull base alleviated the symptoms of epilepsy and nasal obstruction.

[Epilepsy in children with cerebral palsy].

The incidence of epilepsy in 110 patients with cerebral palsy (the majority with spastic tetraplegia) was 46,4%. Almost half of the patients with spastic tetraplegia (45%) and hemiplegia (52%) had epilepsy. The incidence was lower in patients with spastic diplegia (32%). Half of epilepsy in spastic hemiplegia were partial seizures and the other half consisted of generalized seizures, while generalized tonic-clonic episodes predominated in all other forms of C.P. A high incidence of West syndrome was observed in patients with spastic tetraplegia. Etiological factors of C.P. were perinatal in 75…

[Sudden death and mortality in epilepsy].

Mortality in epilepsy is 2-3 times higher than in the age- and sex-matched general population. It is the highest in young male epilepsy patients with generalised tonic-clonic seizures living in low socio-economical situation. The main factors of early mortality unrelated to seizures are the neurological conditions underlying epilepsy. Suicide is an important factor first of all in temporal lobe epilepsy. The group of mortality directly related to epilepsy is made up of the high-mortality grand mal status epilepticus rarely seen in treated epilepsy; the accidents related to seizures and sudden …

[Epilepsy and myasthenia: a case report].

Epilepsy is 5 to 7 times more frequent among patients with myasthenia than in the general population. Myasthenia gravis is often associated with other affectsion, most of immunological origin. However, hypotheses explaining the association are still uncertain. We report a case of association in a woman who had epilepsy since the age of 9 years and developed myasthenia at the age of 23 years after having discontinued here anti-epileptic treatment for two years.

Contribution of neuropsychology to epilepsy surgery.

Surgical treatment is being used with increasing frequency for patients with intractable epilepsy. Operative success depends to a large degree on the results of a comprehensive pre-operative patient evaluation the main purpose of which is to delineate the epileptogenic lesion. The pre-operative assessment includes video EEG monitoring, structural and functional (fMRI) neuroimaging and neuropsychological evaluation. The likelihood of the success of surgery is increased when all test results point to a single epileptogenic focus. The unique contribution of neuropsychology, which includes standar…

Progressive myoclonus epilepsy in a beagle.

A nine-year-old, neutered female beagle was presented with a history of progressive myoclonic jerks. Clinical signs included mental depression and paroxysmal jerks of the head and forelimbs, apparently elicited by changes in light, noise or movements. Electroencephalographic findings were in accordance with myoclonus epilepsy. Postmortem histopathological findings included multiple periodic acid-Schiff-positive inclusion bodies throughout the central nervous system, but especially in the cerebellum, confirming the suspected diagnosis of Lafora's disease.

[The ecstatic epilepsy of Teresa of Jesus].

AIMS: Our aim is to define the ecstasies (raptures) of Teresa de Ahumada (Teresa of Jesus) and to link them with ecstasies recently identified as convulsive seizures (Dostoevsky s epilepsy, Saint Paul s disease, ecstatic epilepsy). DEVELOPMENT AND CONCLUSIONS: We review the Carmelite nun's pathography, with special attention given to the excellent detailed descriptions of her ecstasies. The stereotypic movements and comparison with similar cases leads us to consider them as being epileptic, possibly secondary to neurocysticercosis.

Epilepsy and malformations of the cerebral cortex.

Malformations of the cerebral cortex (MCC) are often associated with severe epilepsy and developmental delay. About 40% of drug-resistant epilepsies are caused by MCC. Classification of MCC is based on embryological brain development, recognising forms that result from faulty neuronal proliferation, neuronal migration and cortical organisation. Hemimegalencephaly, an enlarged dysplastic hemisphere, can present as early onset severe epileptic encephalopathy or as partial epilepsy. In focal cortical dysplasia (FCD), MRI shows focal cortical thickening and simplified gyration. Patients have drug-…

Overintepretation of EEGs and misdiagnosis of epilepsy.

The overinterpretation of EEGs is a known problem that has not been reported specifically. The authors report a series of EEGs on patients who were diagnosed eventually with psychogenic nonepileptic seizures and who had an EEG read as epileptiform. Of the 15 actual records available for review, the overread patterns were wicket spikes (n = 1), hypnagogic hypersynchrony (n = 1), and hyperventilation-induced slowing (n = 1). In the other 12 records, the overread patterns were simple fluctuations of sharply contoured background rhythms or fragmented alpha activity. Rather than well-described norm
