Temporal lobe epilepsy caused by mesial temporal sclerosis and temporal neocortical lesions. A clinical and electroencephalographic study of 46 pathologically proven cases.

This study aims to determine whether there are important clinico-electrical differences between patients with temporal lobe epilepsy (TLE) secondary to mesial temporal sclerosis (MTS) and those with TLE secondary to a discrete temporal neocortical lesion (NL). The case histories, interictal EEG, seizure semiology, ictal EEG and postoperative outcome of 46 pathologically proven patients (31 MTS and 15 NL) were compared. A history of febrile convulsions (FC) was more common in MTS patients (58% versus 26%, P < 0.05), as was a history of a significant cerebral event at < 4 years of age (22% versu

Serotonin receptor expression in hippocampus and temporal cortex of temporal lobe epilepsy patients by postictal generalized electroencephalographic suppression duration.

OBJECTIVE: Prolonged postictal generalized electroencephalographic suppression (PGES) is a potential biomarker for sudden unexpected death in epilepsy (SUDEP), which may be associated with dysfunctional autonomic responses and serotonin signaling. To better understand molecular mechanisms, PGES duration was correlated to 5HT1A and 5HT2A receptor protein expression and RNAseq from resected hippocampus and temporal cortex of temporal lobe epilepsy patients with seizures recorded in preoperative evaluation. METHODS: Analyses included 36 cases (age = 14-64 years, age at epilepsy onset = 0-51 year

Costs Associated with Laser Interstitial Thermal Therapy Are Lower Than Anterior Temporal Lobectomy for Treatment of Temporal Lobe Epilepsy.

BACKGROUND: Laser interstitial thermal therapy (LITT) is a minimally invasive alternative to anterior temporal lobectomy (ATL) for treatment of temporal lobe epilepsy. It has gained popularity as familiarity with technique increases and outcomes are better characterized. There has been no direct cost comparison between the 2 techniques in literature to date. The current study directly compares hospital costs associated with LITT with those of ATL patients and analyzes the factors potentially responsible for those costs. METHODS: Patients who underwent ATL (27) and LITT (15) were retrospective

Neuropsychological functioning in children with temporal lobe epilepsy and hippocampal atrophy without mesial temporal sclerosis: a distinct clinical entity?

Unilateral hippocampal atrophy (HA) is considered as a precursor of mesial temporal sclerosis (MTS) in some patients with temporal lobe epilepsy. However, in other cases, it has been suggested that HA without MTS may constitute a distinct epileptic entity. Hippocampal atrophy without MTS was defined as HA without T2-weighted hyperintensity, loss of internal architecture, or associated lesion seen on the MRI data. To date, no study has focused on the cognitive pattern of children with epilepsy with HA without MTS. The objectives of the present study were to characterize the cognitive profile of

Epilepsy surgery for low-grade epilepsy-associated neuroepithelial tumor of temporal lobe: a single-institution experience of 61 patients.

BACKGROUND: Low-grade epilepsy-associated neuroepithelial tumor (LEAT) is highly responsive to surgery in general. The appropriate surgical strategy remains controversial in temporal LEAT. The aim of this study is to analyze the surgical seizure outcome of temporal LEAT, focusing on the aspects of surgical strategy. METHODS: Sixty-one patients from a single epilepsy center with temporal LEAT underwent surgery. The surgical strategy was according to the multidisciplinary presurgical evaluation. Electrocorticogram (ECoG)-assisted resection was utilized. Surgical extent including lesionectomy an

Brain structural differences in temporal lobe and frontal lobe epilepsy patients: A voxel-based morphometry and vertex-based surface analysis.

PURPOSE: Exploration of the effect of chronic recurrent seizures in focal epilepsy on brain volumes has produced many conflicting reports. To determine differences in brain structure in temporal lobe epilepsy (TLE) and extratemporal epilepsy (using frontal lobe epilepsy (FLE) a surrogate) further, we performed a retrospective analysis of a large cohort of patients with seizure-onset zone proven by intracranial monitoring. METHODS: A total of 120 TLE patients, 86 FLE patients, and 54 healthy controls were enrolled in this study. An analysis of variance of voxel-based morphometry (VBM) was used

Different Functional Network Connectivity Patterns in Epilepsy: A Rest-State fMRI Study on Mesial Temporal Lobe Epilepsy and Benign Epilepsy With Centrotemporal Spike.

The stark discrepancy in the prognosis of epilepsy is closely related to brain damage features and underlying mechanisms, which have not yet been unraveled. In this study, differences in the epileptic brain functional connectivity states were explored through a network-based connectivity analysis between intractable mesial temporal lobe epilepsy (MTLE) patients and benign epilepsy with centrotemporal spikes (BECT). Resting state fMRI imaging data were collected for 14 MTLE patients, 12 BECT patients and 16 healthy controls (HCs). Independent component analysis (ICA) was performed to identify t

Depression and quality of life after anterior hippocampectomy in temporal lobe epilepsy: A prospective study.

BACKGROUND: Temporal lobe epilepsy (TLE), frequently associated with mesial temporal sclerosis (MTS), is the most common form of focal epilepsy in adults and is often accompanied by depressive symptoms that impair quality of life. While anterior hippocampectomy is an established surgical approach for drug-resistant TLE, its postoperative psychiatric and psychosocial impact remains insufficiently studied, particularly in non-Western populations. This study assessed changes in depressive symptoms and quality of life following anterior hippocampectomy in a Middle Eastern cohort. METHODS: Depress

Convergent functional networks of intrinsic activity alterations in temporal lobe epilepsy and their molecular correlates.

BACKGROUND: Temporal lobe epilepsy (TLE) is increasingly recognized as a network disorder, yet reported regional intrinsic neural activity alterations from resting-state fMRI studies remain spatially heterogeneous. This study aimed to determine whether these heterogeneous alterations converge onto a shared functional network and to characterize its normative transcriptomic and neurochemical correlates. METHODS: Using a coordinate-based network mapping approach (functional connectivity network mapping, FCNM), we delineated a common brain network functionally connected to regional intrinsic neu

Rapid hippocampal kindling of mice as a model of sudden unexpected death in temporal lobe epilepsy.

Sudden unexpected death in epilepsy (SUDEP) is the leading cause of seizure-related death in people with epilepsy. Yet, its mechanisms remain poorly understood. While most epilepsy syndromes have no known genetic component, animal SUDEP models are often of rare monogenetic mutations. Here, we present a novel non-genetic mouse model of SUDEP, relevant to temporal lobe epilepsy, the most common seizure disorder. Our accelerated CA1 hippocampal kindling paradigm, referred to as 'rapid kindling', reliably produces a spectrum of seizure phenotypes with progressive severity, from no seizure behavior

High-Order Information Analysis of Epileptogenesis in the Pilocarpine Rat Model of Temporal Lobe Epilepsy.

Temporal lobe epilepsy (TLE) is a devastating disease, often pharmacoresistant and with a high prevalence of 1% worldwide. There are a few disease-modifying therapies; thus, prevention has become a health priority. The overarching goal of this research project is to highlight the system's dynamics at different stages before TLE onset to identify an early shift in network dynamics trajectory toward disease onset. Researchers often investigate collective brain activity by tracking dynamical interactions of the signal recorded at multiple sites. However, these interactions are usually only comput

Calpastatin, a calpain specific inhibitor, reduce seizures in a mouse model of temporal lobe epilepsy.

Epilepsy is a chronic condition characterized by unpredictable and recurrent spontaneous seizures. In a previous study, we reported that pharmacological inhibition of calpain prevented epileptogenesis in the rat pilocarpine model. In this study, we demonstrate that transgenic overexpression of calpastatin, the endogenous inhibitor of calpain, reduces calpain activation and lessens seizure burden in the mouse intrahippocampal kainate model. Blockade of calpain activation was evidenced by a reduction in the generation of spectrin breakdown products, a hallmark of calpain activation. CAST overexp

Differential effects of tau expression on seizures and epileptogenesis in a mouse model of temporal lobe epilepsy.

Studies of the microtubule-associated protein, tau suggest its promise as a potential target for epilepsy disease modification, but mechanisms underlying tau's effects on seizures are not well-defined. Acquired temporal lobe epilepsy (TLE) is the most prevalent form of focal epilepsy, yet the impact of tau expression on the process of TLE development is unexplored. We investigated tau's role in the epileptogenesis of acquired TLE using the intrahippocampal kainate (IHK) model in mice lacking tau expression (i.e., tau-/- mice). We examined epileptiform activity during status epilepticus (SE) af

The analysis of glutamate metabolism modulators in platelets of patients with temporal lobe epilepsy or status epilepticus.

OBJECTIVES: The accumulation of glutamate in the brain synaptosomes is called excitotoxicity which is an underlying mechanism of epilepsy. Platelets also contain a glutamate-glutamine cycle similar to the brain. Platelets release glutamate upon activation and also clear glutamate from the blood upon uptake. It was shown before that the platelet glutamate uptake is decreased in neurodegeneration. Therefore, we aimed to analyze the mRNA expression of Glutamate Transporter 1 (GLT-1), Glutamate Dehydrogenase (GDH), Glutamine Synthetase (GS) and Glutaminase, which are the modulators of glutamate me

Disparities in Connectivity Profiles Between Structural and Functional Networks in Patients With Temporal Lobe Epilepsy.

Temporal lobe epilepsy (TLE) is one of the most prevalent forms of epilepsy. Recent advancements in research have increasingly categorized TLE as a network disorder, focusing on its network characteristics. Prior studies predominantly examined changes in connectivity properties within a single network, with limited attention to the distinctions between different connectivity networks. Both structural and functional connectivity serve as quantitative measures of connection strength among network nodes, yet they differ significantly. Functional connectivity (FC) highlights short-term cooperation

A meta-analysis to investigate the role of magnetic resonance spectroscopy in the detection of temporal lobe epilepsy.

BACKGROUND: Temporal lobe epilepsy (TLE) is a complex neuropsychiatric illness that alters patient's behavior and social well-being. Early and accurate diagnosis of TLE is a key factor in its treatment. Currently, magnetic resonance spectroscopy (MRS) is recommended due to its high sensitivity and specificity in the detection of seizures. However, some studies have reported on its limited role in lesion detection. OBJECTIVES: The present meta-analysis aims to analyze different studies thoroughly and investigate the role of MRS in the detection of TLE. MATERIAL AND METHODS: A systematic liter

Differential vulnerability of neuronal subpopulations of the subiculum in a mouse model for mesial temporal lobe epilepsy.

Selective loss of inhibitory interneurons (INs) that promotes a shift toward an excitatory predominance may have a critical impact on the generation of epileptic activity. While research on mesial temporal lobe epilepsy (MTLE) has mostly focused on hippocampal changes, including IN loss, the subiculum as the major output region of the hippocampal formation has received less attention. The subiculum has been shown to occupy a key position in the epileptic network, but data on cellular alterations are controversial. Using the intrahippocampal kainate (KA) mouse model for MTLE, which recapitulate
