[Necrotizing fasciitis of the upper limb. 12 cases].

Twelve cases of necrotizing fasciitis or streptococcal cellulitis of the upper limb are reported. Four cases presented with a low grade aggressive for and one case was chronic. Seven fulminating cases resulted in two deaths. These different presentations are in fact different stages of the same disease which is a group A beta-hemolytic streptococcal necrotizing infection of the subcutaneous tissue. It is a medical emergency in which surgery is the main treatment. In cases seen early, surgery helps by making an early diagnosis by showing the typical appearance of the subcutaneous tissue and by …

[Necrotizing fasciitis of the upper limb. Apropos of a case].

Necrotizing fasciitis is a rare infection, serious because of its risk of functional sequelae and mortality. The first clinical and histological descriptions of acute Streptococcal infection of the fascia were published in 1924 by Meleney, who referred to this condition as "Streptococcal haemolytic gangrene". However, since Meleney, numerous authors have reported cases of necrotizing fasciitis due to a wide range of bacteria. The authors report a case of necrotizing fasciitis of the upper limb in a 45 year old man following a superficial lesion of the dorsum of the hand. Despite the use of ant…

Recurrent necrotizing fasciitis of the vulva. A case report.

Necrotizing fasciitis most often occurs in the context of prior trauma or surgery. Predisposing medical conditions include diabetes mellitus, arteriosclerosis, obesity, hypertension and prior irradiation. De novo occurrence in the vulva, in the absence of prior injury, surgery or irradiation, has been reported rarely. Necrotizing fasciitis of the vulva in the diabetic patient may have an insidious onset but requires an early diagnosis and aggressive surgical episode of fasciitis occurred in an obese, diabetic woman. Aggressive, wide excision of all infected vulvar, mons and thigh tissue, follo…

[Fasciitis in granulomatous myositis--an atypical manifestation of sarcoidosis?].

We report about two female patients with a histopathological proven granulomatous myositis associated with a fasciitis. One patient showed noncaseating granulomas in the muscle fascia. She suffered from symmetrical contractures of the digital flexors and ankles. The second patient showed uveitis and polyneuropathy, too. In addition, increased activities of the angiotensin-converting-enzyme and lysozyme in the sera suggested that both cases represent a new form of sarcoidosis.

Treatment with cimetidine of atypical fasciitis panniculitis syndrome.

Three patients presented with septal fasciitis and panniculitis, associated with clinical and laboratory features which precluded straight-forward classification into eosinophilic fasciitis, localised scleroderma, or lupus erythematosus profundus. Treatment with cimetidine caused the remission of cutaneous manifestations and the extracutaneous abnormalities, such as nailfold capillary disturbances and the presence of antithyroid antibodies, improved. It is concluded that features of eosinophilic fasciitis or localised scleroderma and certain additional atypical elements should be categorised a…

[Necrotizing fasciitis of the face. Clinical and therapeutic aspects].

Necrotizing fasciitis is a synergic infection, the chief causal agents of which are beta-hemolytic group A streptococci. The authors insistingly review the different aspects of this rare affection and stress the importance of emergent medical and surgical management, as the evolution of the disease is still often lethal.

Scleroderma, fasciitis, and eosinophilia associated with the ingestion of tryptophan.

An association between the ingestion tryptophan and a syndrome characterized by scleroderma-like skin abnormalities, fasciitis, and eosinophilia has recently been recognized in the United States. We report the clinical and histopathological findings in nine patients and the results of biochemical analyses of tryptophan metabolism in seven patients with this syndrome. Edema of the extremities, frequently accompanied by pruritus, paresthesia, and myalgia, developed in the nine patients (six women and three men; age range, 30 to 66 years) 1 to 18 months after the start of therapy with tryptophan …

Necrotizing fasciitis. Review of the literature and case report.

Necrotizing fasciitis (NF) of the hand was preceded by an outbreak of porphyria cutanea tarda in a 31-year-old woman. The presentation, course, differential diagnosis, and treatment of necrotizing fasciitis, including a review of the literature, illustrates the indications for hyperbaric oxygen therapy for this life-threatening infection.

[Nodular fasciitis of the upper arm in a sports marksman].

Nodular fasciitis is a benign, fibroproliferative, tumorlike lesion. Histological differentiation from sarcomas can be difficult. Surgical excision is most effective, recurrences are rare. This case report deals with the onset and natural course of nodular fasciitis in the upper arm of a marksman. When instructing young people in shooting, he had to pace different weapons against the upper third of his arm instead against the shoulder. Here a tender swelling developed. After extirpation he reported pain relief. A recurrence has not been observed so far.

Diffuse fasciitis with hypergammaglobulinemia and eosinophilia: a new syndrome?

Two male patients (53 and 19 years old) have been seen with a diffuse scleroderma-like illness with firm taut skin bound down to underlying … abnormalities; but clinical improvement to date has been minimal. The pathogenesis of this diffuse fasciitis is obscure, although unusual physical exertion antedated the onset of illness in each case.

Necrotizing fasciitis. A serious sequela of omphalitis in the newborn.

We reviewed all cases of omphalitis seen at Childrens Hospital of Los Angeles from 1961 to 1981. One hundred and forty patients were seen and, of these, eight had necrotizing fasciitis. All patients with necrotizing fasciitis acquired omphalitis at home, making the incidence of necrotizing fasciitis over 10% in patients with community acquired omphalitis. Most of the infants appeared relatively well on admission with no fever but had a marked leukocytosis. The disease rapidly spread to involve most of the abdominal wall over a period of several hours to days. Seven of the eight patients died (…

Necrotizing fasciitis as a lethal complication of peritonsillar abscess.

Abscesses of the peritonsillar region rarely lead to serious complications. Incision and drainage, antibiotic therapy, and subsequent tonsillectomy is the accepted method of treatment. Two cases of fatal necrotizing fasciitis following peritonsillar abscess are presented. The entity of necrotizing fasciitis in the head and neck is discussed with respect to its presentation, diagnosis, and treatment.

Hypocalcemia and "toxic" syndrome associated with streptococcal fasciitis.

A 35-year-old man suffered a small puncture wound of the right hand and within four days had acute fasciitis of the right upper extremity. Cultures of wound and blood grew a group A beta-hemolytic Streptococcus. Persistent hypocalcemia was recognized daily, and he died ten days after the injury in cardiac failure. Preliminary studies have shown that the organism elaborated two streptococcal pyrogenic exotoxins (SPE-C and a new exotoxin, SPE-D). It is suggested but not proven that the cause of the patient's "toxic" illness may have resulted from the action of these exotoxins.

The value of bacteriology and serology in the diagnosis of necrotizing fasciitis.

Fourteen patients with necrotizing fasciitis are described. In thirteen the cause was Streptococcus pyogenes [Group A beta haemolytic streptococcus (BHS)]; in the fourteenth, Staphylococcus aureus was responsible. In the acute fulminating form of the disease, BHS can be cultured from the affected tissues. In the less acute form, particularly when the patient has been previously treated with antibiotics, other bacteria colonize the tissues and the BHS cannot be isolated. Serological evidence of infection with Streptococcus pyogenes can be ascertained in all such patients by finding high levels …

Necrotizing fasciitis-a disease of temperate and warm climates.

Necrotizing fasciitis is a distinct clinical entity. It is usually due to Streptococcus pyogenes but may occasionally be caused by Staphylococcus aureus. It needs to be considered in relationship to other infections due to Streptococcus pyogenes, in which the clinical disease that occurs may be associated with the depth of inoculation of the coccus. Mild cases have been identified that are self-limiting as well as serious cases which would have succumbed without surgical debridement. It is likely that some cases of necrotic tropical ulcer are due to necrotizing fasciitis.

[Etiology and diagnosis of necrotizing fasciitis (author's transl)].

This is the report of three cases of necrotizing fasciitis, an acute and progressive disease involving the connective tissue, which may appear after infection with beta-hemolytic streptococci of group A (S. pyogenes). Painless ulcerations and necrosis along the fascial tissue are pathognomonic. Treatment consists in immediate and wide incisions to terminate the toxic effects due to the cell wall constituents of the streptococci together with an adequate chemotherapy. The laboratory diagnosis and the pathogenic mechanisms are discussed.
