BACKGROUND: It is important to assess the fracture risk associated with metastasis in the proximal femur. The study aimed to clarify the effect of tumor location on the risk of pathological fracture of the proximal femur and investigate the fracture risk not only in the stance-loading configuration (SC), but also in the fall-loading configuration (FC) using a computed tomography (CT)-based finite element (FE) method based on a simulated metastatic model.
METHODS: The axial CT scans of the proximal femora of non-osteoporotic healthy men (n = 4; age range, 42-48 years) and osteoporotic post-men

For proximal varus osteotomy of the femur in children, a stable fixation, is a concern regarding the safety and the protection of the surgery. The methods of fixation are mostly represented by plating systems; however, the tension band wiring is a simple and eligible option for fixation of proximal femur osteotomy. The purpose of this retrospective case series study is to evaluate outcomes and bone remodelation after using the tension band wiring technique to fixate proximal femoral varus osteotomies in children. The study enrolled 20 patients who underwent femoral varus osteotomy to treat Legg

BACKGROUND: Long-term follow-up after proximal femur (femoral neck) fracture is required to screen for complications, especially avascular necrosis (AVN). The purpose of this study was to evaluate sociodemographic characteristics as risk factors for lost-to-follow-up (LTFU) status following proximal femur fractures.
METHODS: This retrospective case series included patients under the age of 18 years treated at a children's hospital for proximal femur fractures. Data were collected from the electronic medical record (EMR). Area deprivation index (ADI) was determined using patient addresses. Pat

A 50-year-old female patient with no significant medical history presented with left knee pain. Radiographs of the knee showed a circumferential swelling of the distal femur suggestive of neoplasia. Further evaluation revealed multiple lesions in the left iliac bone and proximal femur. Biopsy was suggestive of a reparative granuloma or an aneurysmal bone cyst. Laboratory assessment showed hypercalcaemia and elevated parathyroid hormone consistent with severe primary hyperparathyroidism. Osseous survey was significant for salt and pepper appearance of the skull. Ultrasound of the neck and 99mTc

A 34-year-old man with a subtrochanteric osteoid osteoma localized to the lateral cortex of the left femur is reported. The patient presented with mild spontaneous pain of the lateral thigh and knee. He refused a radiographic examination and was treated as a greater trochanteric pain syndrome for 9 months. He was then admitted with a transcervical fracture of the neck of the left femur after a fall from standing height. The fracture was fixed with 3 cannulated screws and healed uneventfully. His symptoms worsened after the first postoperative year. Eighteen months postoperatively the pain was

Nine cases of subtrochanteric fracture are reviewed following 300 fixations of subcapital fractures with Garden screws. In six cases technical operative errors were considered to contribute to the fracture. It is stressed that adequate reduction and suitably placed crossed Garden screws are demanding requirements and that every attempt should be made to avoid making extra holes with guidewires. In two technically satisfactory cases the low position of the lower Garden screw-hole in the femur probably predisposed to the subtrochanteric fracture. A computerized two-dimensional finite analysis co

We present the case of a 70-year-old patient, operated for a fracture of the neck of the left femur classified as Garden 4. The treatment consisted of a primary total hip prosthesis whose short-term evolution was marked by 6 complications requiring 3 surgical revisions. The availability of logistical means and the speed of the gestures made it possible to obtain an acceptable outcome.

Malignant mesotheliomas occur in the pleura, peritoneum, pericardium, and tunica vaginalis. The majority of tumors are pleural in origin. The typical pattern of spread is usually contiguous or via implantation. Hematogenous or lymphatic metastasis is not uncommon; however, metastasis to bone has rarely been well documented. This is a case report of malignant pleural mesothelioma metastatic to the femur with a pathologic fracture of femoral neck.

At a traffic accident in 1970 a then 6 year old boy got an impacted fracture of the neck of the right femur with persisting subluxation. This was not particularly noticed because of the severity of other injuries. In the meantime the most recent follow-up examination (1981) of the boy (now 17 years old) showed a dysplasia like growth disorder. Various possibilities of interpretation are discussed.

On the basis of a considerable experience in the use of Ender's nails in basal and trochanteric fractures of the neck of the femur, the authors are convinced of the excellent mechanical properties of this fixation device. They have therefore used it in five atypical and complex cases of subtrochanteric fracture. They describe and discuss the clinical and radiological results obtained. Their reasons for choosing this method in preference to rigid osteosynthesis are given in each case.

BACKGROUND: Femoral neck stress fractures (FNSFs) are a unique injury pattern not commonly treated in the civilian trauma population; however, it is particularly high with military trainees engaged in basic combat training. To date, no study has surveyed a population of military orthopedic surgeons on treatment preferences for military service members (SMs) with FNSF.
QUESTIONS: We aim to evaluate the extent of clinical equipoise that exists in the management of these injuries, hypothesizing that there would be consensus in the factors dictating surgical and non-surgical intervention for FNSF

RATIONALE: Bilateral femoral neck stress fractures are relatively rare injuries that occur frequently in military recruits, athletes and patients with osteoporosis, renal bone disease, metabolic bone disease, and chronic steroid use. Herein, a case of an elderly patient with bilateral femoral neck stress fractures is reported.
PATIENT CONCERNS: A 65-year-old man presented to the author's hospital with right hip pain for over a month. The patient was a farmer, had a long history of field labor before the onset of pain, denied any history of trauma.
DIAGNOSIS: The patient was diagnosed with a

A case report of a Marine Recruit with bilateral atypical tensile-sided femoral neck stress fracture. The patient was treated nonoperatively with protected weightbearing following an established protocol that included close observation with serial exams and advanced imaging. He recovered uneventfully and was able to return to running. He was eventually diagnosed with bilateral femoral acetabular impingement syndrome at the end of his treatment course demonstrating a growing association warranting further investigation.

OBJECTIVES: Proximal femoral plating serves as a good alternative for the fixation of femoral neck fractures, but fixation loss still occurs. This study aimed to evaluate the effect of a hybrid configuration of short- and long-threaded cannulated screws in proximal femoral plating in terms of decreasing varus collapse of femoral neck fractures.
MATERIALS AND METHODS: We retrospectively analyzed 86 patients with femoral neck fractures who were fixed by proximal femoral plating from January 2015 to June 2019. These patients were divided into two groups according to screw configuration: short- a

BACKGROUND: Research indicates that in the traditional method of cannulated screw fixation for femoral neck fractures, placing the distal screw beyond the lesser trochanter increases the risk of subtrochanteric fractures. In the Biplane double-supported screw fixation technique (BDSF), the lowest screw needs to be inserted at a high inclination angle, making its entry point further from the lesser trochanter. This could increase the risk of iatrogenic subtrochanteric fractures and compromise the stability of the fracture fixation structure. However, there is currently a lack of research on thi

PURPOSES: Proximal femoral fractures (PFFs) represent a major cause of morbidity and mortality in the elderly population. Beyond bone mineral density, proximal femoral (PF) and hip joint (HJ) morphometry have been implicated in fracture susceptibility and fracture pattern. However, the relative contribution of individual geometric parameters remains incompletely defined. The purpose of this study was to systematically evaluate the association between PF and HJ morphometry and hip fracture (HF) patterns, and to assess the predictive value of radiographic morphometric parameters derived from sta

BACKGROUND: Hollow screw internal fixation is commonly used in clinical treatment of impacted femoral neck fractures. Previous studies have demonstrated a correlation between the preoperative posterior tilt angle of the femoral head and failure of internal fixation, but there are fewer studies related to the occurrence of postoperative femoral head necrosis and the posterior tilt angle.
METHODS: To examine the relationship between three-dimensional posterior tilt angle measurements in affected femoral neck fractures and postoperative osteonecrosis of the femoral head and related risk variable

BACKGROUND: The globally acknowledged treatment for mild to moderate slipped capital femoral epiphysis (SCFE) is single screw in situ fixation, also used for prophylactic contralateral fixation. The Free-Gliding Screw (FG; Pega Medical) is a 2-part free-extending screw system designed to allow the growth of the proximal femur. We aimed to analyze the relationship between skeletal maturity and potential growth of the proximal physis and remodeling of the femoral neck using this implant.
MATERIALS AND METHODS: Females below 12 years and males below 14 years undergoing in situ fixation for stabl

INTRODUCTION: Several studies investigated the posttraumatic osteonecrosis of the femoral head (ONFH) after femoral neck fracture (FNF). However, no study has investigated the hidden ONFH after FNF, which is missed by simple radiographs, using magnetic resonance imaging (MRI).
MATERIALS AND METHODS: This retrospective study involved 58 consecutive patients who underwent implant removal surgery after internal fixation due to FNF. MRI was used to investigate the incidence of hidden ONFHs, which were not initially revealed on plain radiographs. The comparisons between hidden ONFH and other group

The most common treatment for slipped capital femoral epiphysis worldwide is in situ fixation with a threaded screw. Un-threaded screws are designed to prevent slip progression without hindering residual growth of the proximal femur. This study aimed to compare growth, remodelling and long-term outcomes after fixation with un-threaded screws and a matched cohort of patients treated with a standard screw. Six patients (nine hips) treated with un-threaded screws and 16 patients (21 hips) treated with standard screws matched for age, skeletal maturity, sex and Southwick angle were recruited. Clin
