Economic evaluation of anti-malarial drug policies across presidential regimes in Nigeria: A comparative analysis from 1999 to present.

BACKGROUND: Malaria remains a significant public health challenge in Nigeria, accounting for substantial morbidity, mortality, and economic loss. Successive administrations have implemented various anti-malarial drug policies aimed at curbing this endemic disease. This study applies a formal economic evaluation framework-integrating both cost-effectiveness analysis (CEA) and cost-benefit analysis (CBA)-to assess and compare anti-malarial drug policies across different presidential regimes. METHODS: A comparative economic evaluation was conducted using incremental cost-effectiveness ratios (IC

Bridging the gap: the Pasteur Network's approach to equitable vaccine development and manufacturing.

Equitable access to vaccines remains a cornerstone of global health security, yet persistent gaps in regional manufacturing capacity continue to undermine timely and fair distribution. The COVID-19 pandemic exposed the risks of highly concentrated production systems and underscored the need for locally anchored manufacturing models capable of responding rapidly to public health emergencies. The Pasteur Network (PN)-a global consortium of 32 public health and research institutes across Africa, Asia, Europe, and the Americas-offers an operational example of decentralized vaccine manufacturing em

Outpatient Parenteral Antimicrobial Therapy in Asia: Evolution, Models, and Challenges over the Past Decade-A Narrative Review.

Background: Outpatient Parenteral Antimicrobial Therapy (OPAT) has long been established as a safe and cost-effective alternative to inpatient care in Western healthcare systems. In contrast, its implementation across Asia has historically been fragmented and unstructured. A landmark multinational survey in 2017 identified OPAT in Asia as a "missed opportunity," citing the lack of systematic oversight, standardized protocols, and outcome monitoring systems. Methods: This narrative review examines the evolution of OPAT in Asia over the past decade, synthesizing evidence from regional studies, n

Nursing leadership in Housing First implementation: A comparative analysis of care coordination approaches across four U.S. states.

BACKGROUND: Homelessness affects over 650,000 Americans annually, with 25% to 30% experiencing co-occurring mental health and substance use disorders. Despite evidence supporting Housing First approaches, implementation remains inconsistent across states, leading to fragmented care delivery and persistent housing instability for vulnerable populations. PURPOSE: To analyze Housing First implementation across four U.S. states and identify nursing practice and policy implications using the Minnesota Public Health Intervention Wheel framework. METHODS: Comparative case study analysis of Housing

Cross-jurisdictional access to innovative drugs and devices: a five-year policy and practice review of the Hong Kong and Macao medicine and equipment connect in the Greater Bay Area of China.

Launched in 2020, the "Hong Kong and Macao Medicine and Equipment Connect" (HKMEC) policy introduced a novel delegated-authority model in China's Greater Bay Area (GBA), enabling designated hospitals to use Hong Kong- or Macao-approved drugs and devices through Guangdong provincial approval, thereby bypassing standard National Medical Products Administration (NMPA) registration. To synthesize five-year implementation evidence, this review followed PRISMA-ScR guidelines and systematically searched Chinese (CNKI, Wanfang, VIP) and English (PubMed, Embase, ScienceDirect) databases covering 2020 t

Reframing spinal cord injury rehabilitation in fragile contexts: a multi-level systems framework for Lebanon.

Background: Spinal cord injury (SCI) requires long-term, multidisciplinary rehabilitation to optimize functioning and participation. In fragile health systems, however, rehabilitation services are often fragmented, under-resourced, and inconsistently accessible. This Current Debate paper examines SCI rehabilitation in Lebanon as an example of how health system fragility constrains rehabilitation delivery and disability inclusion. Methods: Using an International Classification of Functioning, Disability and Health (ICF)-informed approach, we developed a multi-level conceptual framework through s

Welfare Regimes and Out-of-Pocket Healthcare Spending in the Last Year of Life Among Cancer Decedents in Europe and Israel.

Background/Objectives: Out-of-pocket (OOP) healthcare spending can remain substantial near the end of life even in countries with broad health coverage, yet less is known about whether these expenditures follow broader welfare-regime patterns. This study examined OOP healthcare expenditure among cancer decedents and assessed whether welfare-regime differences persisted after adjustment for individual demographic and socioeconomic characteristics. Methods: We analyzed Survey of Health, Ageing and Retirement in Europe (SHARE) End-of-Life data for 1950 cancer decedents aged 50 years and older fro

Health care systems as determinants of outcomes in multiple myeloma: final results from the Latin American MYLACRE study.

Although systemic therapy for multiple myeloma (MM) has evolved considerably over the past 2 decades, state-of-the-art treatment is not uniformly available in Latin America. In some countries, disparities between the public and private sectors in clinical presentation, access to novel agents, and transplantation are striking, with the public sector lagging. We conducted a multicenter, observational study of patients with MM in 5 Latin American countries (Argentina, Brazil, Colombia, Mexico, and Panama). We enrolled patients aged ≥18 years diagnosed with MM between January 2016 and June 2021, u

Children and Wealth Contexts in the United States: Differences by Household Type.

OBJECTIVE: To examine whether the wealth context of households with children, marked by high rates of inequality and low levels of wealth for those at the bottom, also applies to elderly households and households without children. BACKGROUND: Children experience higher income poverty than elderly or working-age adults, but wealth and wealth deprivation comparisons across these groups have not been done. Exploring these differences may reveal another economic dimension on which households with children are uniquely vulnerable and inform policies aimed at financial stability. METHODS: Data are

What makes health systems resilient? An analytical framework drawing on European learnings from the COVID-19 pandemic based on a multitiered approach.

INTRODUCTION: The COVID-19 pandemic posed an unprecedented challenge, which caught many health systems widely unprepared. The aim of this research was to develop a comprehensive analytical framework on health system resilience in the context of pandemics. In addition to serving as a tool to analyse the preparedness and resilience of health systems, the framework is intended to provide guidance to decision-makers in health policy. METHODS: The analytical framework was developed based on a multitiered approach. A comprehensive review of the existing literature was conducted to identify relevant

Financial aspects of haemophilia care in Puerto Rico and other Latin American countries.

The present cost of optimal care in haemophilia is very high. The paradigm of comprehensive care approach, including the essential elements of continuous integrated multidisciplinary health services and the provision of home care with early use of antihaemophilic products requires abundant economic resources that usually are not readily available in nonaffluent countries. A cost-effective comprehensive paediatric haemophilia programme has been operating in Puerto Rico during the last 15 years that provides quality care to over 90% of paediatric haemophiliacs, and is financed mainly by the loca

Urinary concentrations of environmental phenols in pregnant women in a pilot study of the National Children's Study.

Environmental phenols are a group of chemicals with widespread uses in consumer and personal care products, food and beverage processing, and in pesticides. We assessed exposure to benzophenone-3, bisphenol A (BPA), triclosan, methyl- and propyl parabens, and 2,4- and 2,5-dichlorophenol or their precursors in 506 pregnant women enrolled in the National Children's Study (NCS) Vanguard Study. We measured the urinary concentrations of the target phenols by using online solid-phase extraction-isotope dilution high performance liquid chromatography-tandem mass spectrometry. NCS women results were c

Controlled-release oxycodone admissions in public and private substance abuse treatment: associations with organizational characteristics.

Although non-medical use of controlled-release (CR) oxycodone is increasing, little is known about the substance abuse treatment facilities serving individuals dependent on this medication. This study examines the associations between the characteristics of treatment organizations and two dependent variables: any CR-oxycodone admissions and the number of CR-oxycodone admissions in the past six months. Face-to-face interviews were conducted with administrators of 363 publicly funded and 401 privately funded treatment centers in the US. Publicly funded non-profit organizations were less likely t

Effects of state coalitions to reduce underage drinking: a national evaluation.

INTRODUCTION: Drinking by youth remains prevalent. The Reducing Underage Drinking through coalitions (RUD) project funded ten states for 8 years to form coalitions designed to change the policy and normative environment regarding youth access to alcohol. An independent national outcome evaluation of this $21-million effort was conducted. METHODS: Using a longitudinal quasi-experimental design, the ten intervention states were compared with the other 40 states, with repeated annual measures of outcomes from 1995 to 2004. Measures included print news media coverage, legislative bills enacted, y

Assisted reproduction and reproductive rights: the case of in vitro fertilization.

In vitro fertilization (IVF) and other assisted reproduction technologies (ARTs) have become widely accepted as therapy for a wide array of fertility problems and accompanied by the rapid expansion of clinics that provide full range of ARTs. Although these technologies undoubtedly offer benefits for some individuals, they raise important questions over reproductive rights to safe and effective treatment as well as access. This article analyzes current data concerning the safety, effectiveness, and cost of IVF. It concludes that IVF and related techniques have been transformed too rapidly and e
