A scoping review of conservative treatment approaches for volar plate injuries of the proximal interphalangeal joints in an acute health care setting.

BACKGROUND: A volar plate injury (VPI) of the proximal interphalangeal joints (PIP) is a common injury that occurs when a finger is hyperextended, rupturing or separating the thick ligament from the bone on the finger's palm side. The degree of damage determines the course of treatment. For stable injuries without dislocation, conservative treatment such as hand rehabilitation, splinting, and rest is typically recommended. It is crucial to explore the effectiveness of conservative treatment of VPIs, as evidence suggests that surgical treatment is often over-utilised in these cases. METHODS: A

Predicting complete finger extension in Dupuytren's disease.

Multiple studies have reported the effectiveness of treatment on contracture reduction in Dupuytren's disease. However, very few studies have attempted to quantify to which extent patient and disease characteristics influence the chance of achieving a straight finger after surgery. Therefore, the aim of this study is to explore to which extent pre-operative patient and disease characteristics can reliably predict a straight finger after surgery for Dupuytren's disease. In total, 812 and 281 patients, who underwent a limited fasciectomy or needle fasciotomy, respectively, were included in the f

Biomechanics of the Acute Boutonniere Deformity.

PURPOSE: To demonstrate which structures of the extensor mechanism create a boutonniere deformity, when damaged, in a cadaver model. An analysis of how damage to these anatomical structures affects the biomechanical performance of the extensor mechanism was also performed. METHODS: We secured 18 fresh cadaveric hands onto an apparatus consisting of a computer-controlled motor and tensiometer attached in series to the extensor communis tendon of the ring and middle digits. The central slip, transverse, and oblique fibers of the interosseous hood and the triangular ligament were sequentially di

Posttraumatic Boutonnière and Swan Neck Deformities.

Boutonnière and swan neck deformities of the finger can be the result of trauma. The complex anatomy of the extensor mechanism of the finger makes understanding the pathomechanics of these deformities challenging. These posttraumatic deformities should not be confused with those associated with inflammatory arthritis because the treatment options are often very different. An accurate clinical assessment is essential for selecting the appropriate treatment method. Physical examination, including Elson and intrinsic-plus tests, and plain radiography are important tools for diagnosis. A variety o

Kirner's deformity: progressiveness and classification.

In a 30-year-old man, bilateral Kirner's deformity has been diagnosed. In the fully extended finger, the long axes of the middle and terminal phalanges formed an angle of 10 degrees in the volar and 5 degrees in the radial direction. Owing to the altered morphology of the distal phalanx, its palmar surface was at an angle of 25 degrees to the long axis of the middle phalanx. The deformity has existed since birth and has not changed except in size. His mother had the same bilateral deformity. Other closer relatives showed a unilateral appearance of Kirner's deformity: his maternal grandmother a

A demonstration of validity of 3-D video motion analysis method for measuring finger flexion and extension.

This study demonstrates the validity of using 3-D video motion analysis to measure hand motion. Several researchers have devised ingenious methods to study normal and abnormal hand movements. Although very helpful, these earlier studies are static representations of a dynamic phenomenon. Despite the many studies of hand motion using scientifically impeccable techniques, little is known about digital motion, and there are still few researchers investigating dynamic three-dimensional motion of the hand. Results from a three-camera video motion analysis system were compared to those from the "gol

Dynamism in Dupuytren's contractures.

Dynamism of Dupuytren's contractures may occur if a contracture crosses over both the metacarpophalangeal (MCPJ) and proximal interphalangeal (PIPJ) joints of one finger. In this situation the extension deficit at one joint can be influenced by the position of the other. This may affect clinical measurements and introduce errors and bias into research using angular deformity as an endpoint. This study reports the dynamism encountered when measuring passive joint extension deficits in pre-operative contractures. Eighty-five digits with a contracture affecting the PIPJ or MCPJ and PIPJ on 72 han

[Repair of defect on finger skin with reverse fascial pedicle island flap of dorsal branch of digital artery].

OBJECTIVE: To introduce the surgical procedure and indication of the reverse fascial pedicle island flap of the digital artery dorsal branches in repairing finger skin defect. METHODS: By use of the dorsal branches of the digital artery as the pedicel, the reverse island flap was designed. The skin defects of the proximal interphalangeal joint and beyond in 35 cases (42 fingers) were repaired and the joint or extensor tendon was reconstructed simultaneously. Donor site was primarily closed or a skin graft was used. The flap size ranged from 1.0 cm x 2.5 cm to 1.5 cm x 3.5 cm. RESULTS: Thirty

Impairment of voluntary control of finger motion following stroke: role of inappropriate muscle coactivation.

Subjects with chronic hemiplegia following stroke attempted to perform voluntary isometric, isokinetic, and free contractions of the extensor muscles of the metacarpophalangeal (MCP) joints. We recorded torque, metacarpophalangeal joint angle and velocity, and electromyographic (EMG) activity of the extrinsic extensors and flexors and the first dorsal interosseous (FDI). We found that voluntary MCP joint extension in hemiparetic subjects was greatly impaired in comparison with control subjects: only two of the 11 stroke subjects were able to generate even 0.21 N-m of isometric extension torque

Hemi-hamate arthroplasty for dorsal fracture-dislocation of proximal interphalangeal joint.

Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint, often accompanied by palmar lip fractures, are commonly seen in hand trauma. Management of these fractures requires restoring joint congruity and stability while allowing for early mobilisation. This case report discusses the successful use of a hemi-hamate autograft for reconstruction of the base of the middle phalanx in a young male patient with a severe dorsal PIP joint fracture-dislocation. The results demonstrate a promising technique for joint restoration with minimal functional impairment and excellent postoperati

Variable Adjustment Methods for Hemihamate Autograft Arthroplasty in Proximal Interphalangeal Joint Fracture/Dislocations: A Surgical Technique Revisited and Case Series.

Proximal interphalangeal joint fracture/dislocations, even after surgical correction, often lead to long-term complications including posttraumatic arthritis, prolonged joint stiffness, and chronic joint instability. A wide range of surgical techniques has been devised to resolve this issue, but none has been promising enough. Despite this circumstance, arthroplasty using a hemihamate autograft of size and contour that match the middle phalangeal base has progressed into one of more acceptable methods that provide both articular congruency and osseous stability. In this article, we introduce v

Anatomical basis for functional treatment of dorsolateral dislocation of the proximal interphalangeal joint.

Dorsolateral dislocation of the proximal interphalangeal (PIP) joint is a common injury to the hand. Closed reduction of the dislocation anatomically realigns the avulsed ligaments; thus patients may be managed non-operatively. Standard treatment involves placement of a dorsal splint to prevent hyperextension and lateral stresses. This allows early active motion of the PIP joint while preventing a flexion contracture. In this fresh cadaver study, the PIP joint in 24 fingers was dorsolaterally dislocated. Four digits had to be excluded from the investigation due to a fracture dislocation with a

Adhesions as a component of the trigger finger: a dynamic sonographic study.

We performed a detailed dynamic high-resolution ultrasound examination of the flexor tendons in trigger fingers and compared this with normal contralateral digits. There was a loss of defined linear tendon margins and/or traction of the flexor tendons on the surrounding soft tissue during passive flexion of the distal interphalangeal joint in 17 out of 20 trigger fingers, which indicated adherence to the surrounding tissues. The differential motion between the flexor digitorum profundus tendon and the flexor digitorum superficialis tendons was also lost in ten trigger fingers, which suggested

Severe Nail Fold Psoriasis Extending from Nail Psoriasis Resolved with Ustekinumab: Suggestion of a Cytokine Overflow Theory in the Nail Unit.

Because nail psoriasis is difficult to treat, therapy with many biological drugs has been attempted. Ustekinumab is approved for chronic plaque psoriasis and psoriatic arthritis (PsA), with some trials reporting nail improvement using this agent. A 51-year-old man with severe chronic plaque psoriasis had severe involvement of all fingernails and toenails, with accompanying nail fold psoriasis. He also had PsA of the small joints of the fingers. Despite multiple conventional therapies, the nail lesions did not improve, and his nail psoriasis severity index score was 97. After a fourth ustekinum
