Crohn's disease in the Chinese population. An experience from Hong Kong.

PURPOSE: Crohn's disease was extremely rare among Chinese. We reviewed all cases diagnosed as having Crohn's disease during a five-year period. METHODS: A diagnosis of Crohn's disease was made only if all of the following criteria were fulfilled: 1) clinical symptom(s) and sign(s) compatible with chronic inflammatory bowel disease; 2) exclusion of intestinal infection by repeated stool cultures; 3) macroscopic features of small and/or large intestinal inflammation with skip lesion, stricture, and fistula formation; 4) histologic features of Crohn's disease, i.e., focal lymphoid aggregate, foc

Tissue reaction of the skeletal muscles of cattle both to a spontaneous and experimental infection with Cysticercus bovis.

The reaction of the tissues to an infection with Cysticercus bovis was studied on material from cattle with a spontaneous and an experimental infection. The authors examined a total of 232 cysts from the skeletal muscles. A tissue reaction to C. bovis was inflammatory in nature and originated always at the site at which the invaginated scolex opened into the surface. Typical of these changes were the origin of a pseudoepithelial rim and a zone of granulation tissue maturing on the periphery. At a later phase marking the start of necrotic changes in the cysticercus, the inflammation accompanied

[Clinical picture of single and multiple rhinogenic brain abscesses].

The authors examined 11 patients with rhinogenous abscesses of the brain aged from 2 to 45, 5 of them with multiple abscesses. According to their data the infection penetrated into the cranial cavity by two principal ways, namely, contactual (in 2 patients) and hematogenous via venous passages (in 4 patients). Contactual cerebral abscesses of rhinogenous genesis more frequently appear during frontitis, have a lingering chronic course and localize at the pole of the frontal lobe. Clinically they manifest themselves by focal and total cerebral symptoms including craniographic changes of hyperten

Incidentally Detected Congenital Bronchial Atresia in Late Adulthood: A Case Report and Imaging Review.

Bronchial atresia is a rare congenital anomaly characterized by focal interruption of a bronchus, typically associated with distal mucoid impaction and hyperinflation of the corresponding lung parenchyma. It is usually identified incidentally in children or young adults, whereas diagnosis in late adulthood is uncommon. We report the case of a 59-year-old man who presented with chronic cough and dyspnea. Chest computed tomography (CT), performed before and after intravenous contrast administration, revealed focal unilateral hyperlucency involving the posterior segment of the right upper lobe, a

Neonatal seizures.

Epileptic seizures are more frequent in the neonate than at any other time. The incidence of neonatal seizures (NNS) is estimated to be between 1.5 and 5.5/1000 living births, its onset being during the first week in 80% of cases. Mortality rate remains very high (20-45%). Not all paroxysmal manifestations are epileptic, and differential diagnosis remains an important challenge. Neonates may present with different types of seizures: clonic, tonic, myoclonic (axial, focal, erratic), epileptic spasms, and subtle seizures, including autonomic signs or automatisms. The main etiology is hypoxic-isc

Bacteremia with Streptococcus pneumoniae: sepsis and other risk factors for 30-day mortality--a hospital-based cohort study.

We conducted a hospital-based cohort study among adult patients with first-time Streptococcus pneumoniae bacteremia (SPB) from 2000 through 2008. Patients were identified in a population-based bacteremia database and followed up for mortality through the Danish Civil Registration System (CRS). The aim of the study was to determine the focal diagnosis of SPB, the severity of sepsis at presentation, demographics and comorbidity characteristics of the patients, and to determine the 30-day mortality rate and factors related to mortality. We identified 481 patients, of which 238 were males. The mea

Cerebral venous and dural sinus thrombosis in severe falciparum malaria.

Common causes of coma in falciparum malaria are cerebral malaria, hypoglycaemia and electrolyte disturbances. Focal deficits due to arterial infarcts may sometimes occur in children, but are rare in adults. Three adults with falciparum malaria who had fever, altered consciousness and focal neurological deficits (one of whom also had seizures) are being reported here. CT scan of the brain revealed haemorrhagic infarction of the cerebral cortex and subcortical white matter with surrounding oedema suggestive of venous infarction in all three patients. The diagnosis of cerebral venous thrombosis w

CINC-1 is an acute-phase protein induced by focal brain injury causing leukocyte mobilization and liver injury.

Following injury or infection, the liver releases acute-phase proteins (APP). After a severe focal injury, this systemic response can be excessive and may lead to multiorgan dysfunction (MODS). CINC-1 is a neutrophil chemoattractant, and we have now established that it also functions as an early APP after injury to the brain or to peripheral tissues. After induction of a focal inflammatory lesion in the brain, there is rapid hepatic and serum CINC-1 induction, which is associated with increases in neutrophil numbers within the liver and within the circulation. CINC-1-mediated recruitment of ne

Intestinal lesions in a horse associated with eastern equine encephalomyelitis virus infection.

The primary lesions of eastern equine encephalomyelitis (EEE) virus infection in the horse are limited to the brain and spinal cord. Intestinal lesions in addition to the changes in the central nervous system were found in a 6-month-old male Tennessee Walking Horse. One week prior to death, this colt was vaccinated for EEE virus, western equine encephalomyelitis virus, influenza virus, equine rhinopneumonitis virus, and tetanus. The clinical signs consisted of ataxia and rear-end weakness, with a body temperature of 102.8 F. Gross lesions consisted of yellowish discoloration, swelling, edema,

[Immuno-histological findings of the liver in virus-related autoimmune hepatitis--preliminary findings].

As virus-associated autoimmune hepatitis a frequently chronic HBsAg-seronegative hepatitis was defined, in which in the liver tissue virus-markers and in the serum high titre (titre more than 1:320) autoantibodies are to be proved. The immunohistological findings of the liver of 11 patients with such a disease suggest a hepatitis B-virus, a hepatitis non-A/non-B-virus and combination forms. The solitary occurrence of virus-markers seems to be typical. In the hepatitis B-associated form furthermore a diffuse IgG-deposition at the liver cell membrane was characteristic as well as focal cytoplasm

Immune-inflammatory markers in massively disseminated cysticercosis.

Disseminated cysticercosis is a common endemic tropical infection. We report a 24-year-old man who presented with a single episode of focal seizure and transient depression. Imaging studies revealed disseminated vesicular cysticercosis involving almost all the body parts. In spite of severe dissemination and significantly raised immunological inflammatory markers, the patient had negligible symptoms. He was subsequently followed up with oral corticosteroid and antiepileptic drug without further complication. The extensive dissemination, relatively benign nature of the disease and clinicoradiol

[Intrauterine infection and the preterm brain: dimensions of aetiology research].

Perinatal brain damage has a diverse and complex aetiology. Over the past decades, much progress has been made in this research field. In this article, I offer a discussion of seven dimensions of aetiological perinatal brain damage research: (1) hypoxia-ischaemia vs. inflammation; (2) "classic" vs. "remote" intrauterine infection; (3) focal vs. diffuse white matter damage; (4) maternal vs. foetal inflammatory response; (5) clinical vs. experimental data; (6) bacterial vs. viral infection; and (7) preterm vs. term delivery. Despite these complexities, it is hoped that obstetricians, neonatologi

Parotid abscess due to salmonella enteritidis: A case report.

BACKGROUND: Salmonella infection of the parotid gland is rare. CASE: An instance in a 50-year-old man of Salmonella enteritidis parotiditis initially recognized by microbial culture of a fine needle aspiration cytology material is described. The identified predisposing factor was chronic alcoholic abuse. For the infection source, a carrier state of salmonella parotitis was postulated, which progressed to focal abscess and was subsequently complicated by bacteremia and hematogenous spread to the liver, spleen and lungs. CONCLUSION: Salmonella should be included in the differential considerati

[Lyme disease from the neurologist's viewpoint].

The gamut of neurological symptoms observed in Lyme disease is outlined on the basis of 45 case histories. The Borrelia infection of the nervous system manifests itself primarily at the spinal roots and the cranial nerves. Signs of spinal cord and brain involvement were found in well over a third of the patients studied. These may be taken as the result of the infection spreading to parenchymatous structures or as vessel reactions and infections. Long-lasting exhaustion and residual focal deficits are possible late sequelae. Cases of chronic meningoencephalitis, demyelinisation processes and d

Glenohumeral joint preservation: current options for managing articular cartilage lesions in young, active patients.

This is a review of joint-preservation techniques for the shoulder. Whereas the management of diffuse articular cartilage loss in the glenohumeral joints of elderly and less active patients by total shoulder arthroplasty is well accepted, significant controversy persists in selecting and refining successful operative techniques to repair symptomatic glenohumeral cartilage lesions in the shoulders of young, active patients. The principal causes of focal and diffuse articular cartilage damage in the glenohumeral joint, including previous surgery, trauma, acute or recurrent dislocation, osteonecr

Alopecia areata of dental origin.

The association of alopecia areata and infectious foci of dental origin is relatively common, and may be explained by the autoimmune nature of the disorder. We describe a case of alopecia areata with no apparent cause and that was effectively resolved by eliminating a focalized dental infection via endodontic treatment. The presence of common immune mediators in the pathogenesis of both alopecia areata and dental infection could account for the dental origin of the hair loss. In this sense, patients with localized alopecia should be subjected to careful exploration of the oral cavity in search

Wound, Ischemia, Foot Infection (Wifi) Classification System And Its Predictive Ability Concerning Amputation-Free Survival, Mortality And Major Limb Amputation In A Portuguese Population: A Single Center Experience.

INTRODUCTION: Commonly used chronic limb-threatening ischemia (CLTI) classifications lack granularity and detail to precisely stratify patients according to risk of limb loss, expected revascularization benefit and mortality. The aim of this study is to evaluate in a Portuguese population the prognostic value of an updated CLTI classification based on Wound, Ischemia, and foot Infection (WIfI) proposed by the Society for Vascular Surgery. MATERIALS AND METHODS: Single-center retrospective evaluation of prospectively collected data of consecutive patients with CLTI submitted to lower limb reva

Dexmedetomidine sedation for awake fiberoptic intubation of patients with difficult airways due to severe odontogenic cervicofacial infections.

PURPOSE: Odontogenic infections present challenging airway scenarios to surgeons and anesthesiologists. Among specialists, there is controversy over airway management for those patients with airways made difficult by trismus and swelling with anatomic impingement and derangement. Awake fiberoptic intubation has achieved favor in the oral and maxillofacial surgery and anesthesiology communities for management of such difficult airways, but patient comfort and anxiety management with traditional agents may prove hazardous because of potential suppression of protective mechanisms and respiratory
