Diabetic foot ulcers (DFU), one of the most serious complications of diabetes, are essentially chronic, nonhealing wounds caused by diabetic neuropathy, vascular disease, and bacterial infection. Given its pathogenesis, the DFU microenvironment is rather complicated and characterized by hyperglycemia, ischemia, hypoxia, hyperinflammation, and persistent infection. However, the current clinical therapies for DFU are dissatisfactory, which drives researchers to turn attention to advanced nanotechnology to address DFU therapeutic bottlenecks. In the last decade, a large number of multifunctional …

Diabetic foot infections, a common but diagnostically challenging clinical presentation, requires the difficult differentiation between soft tissue-only infection, diabetic neuropathic osteoarthropathy, osteomyelitis or a combination of these pathological processes. While there are clinical predictors for osteomyelitis and simple bedside tests available, imaging is often required for accurate diagnosis. A variety of anatomic and molecular imaging tests are in clinical use, each with its advantages and disadvantages. This review will provide an overview of the different available imaging modali…

This article summarizes key elements of the 2020 Harold A. Stein Lecture presented at the Eye and Contact Lens Special Symposium "Healing the Ocular Surface: What's New for Neurotrophic Keratitis." The major point that I made in this talk is that we need to think about neurotrophic keratopathy as ophthalmology's diabetic foot problem; as a chronic condition that is, often best managed by prevention. Neurotrophic keratopathy is not primarily an inflammatory condition, as suggested by the term ‟neurotrophic keratitis." The epitheliopathy in this condition is a consequence of the dysregulated epi…

Energy storing and returning prosthetic feet are commonly prescribed. Research has demonstrated advantages to use these types of prosthetic feet. However, their stiffness in the sagittal plane is fixed and cannot adapt to different walking tasks and user preference. In this paper, we propose a novel prosthetic foot design capable of modulating its stiffness in the sagittal plane. The Variable Stiffness Ankle unit (VSA) is mounted on a commercially available prosthetic foot. The stiffness of the foot is adjusted with a lightweight servo motor controlled wirelessly. The stiffness change is accom…

UNLABELLED: Diabetes mellitus continues to be an increasingly common comorbidity. Diabetic foot infections are one of the most common causes of hospitalization in this population, and account for a significant portion of increased hospitalization and healthcare expenditure. Complications, such as osteomyelitis, can necessitate the use of multiple, prolonged antibiotic courses. These courses often consist of broad-spectrum, empiric therapy determined by organisms considered to be commonly associated with these types of infections. Extended periods of broad-spectrum antibiotic regimens can contr…

Surgery of the foot constitutes a substantial portion of orthopedic procedures, performed by both orthopedic surgeons and doctors of podiatric medicine. Little research exists on the medicolegal implications of foot surgery amongst these specialties. This study seeks to investigate the different medical and legal factors associated with foot surgery-based malpractice litigation. Malpractice data between 2004 and 2017 was collected using the VerdictSearch legal database. Cases involving foot surgery were identified, and case information including physician specialty, procedure, medical outcome,…

This article describes approaches to and the management of complex cavus foot deformities. Correction of rigid multiplanar deformities can be very challenging, given the presence of skeletal deformities in multiple planes and combined with a varying degree of muscle imbalance. The complexity of these cases always requires a case-by-case approach. Some of the cases presented here occur in patients who have previously undergone surgical management for their deformity, several of which are complicated by additional deformities. With a firm understanding and application of the principles of deform…

Diabetic foot infections (DFIs) are severe complications of long-standing diabetes, and they represent a diagnostic challenge, since the differentiation between osteomyelitis (OM), soft tissue infection (STI), and Charcot's osteoarthropathy is very difficult to achieve. Nevertheless, such differential diagnosis is mandatory in order to plan the most appropriate treatment for the patient. The isolation of the pathogen from bone or soft tissues is still the gold standard for diagnosis; however, it would be desirable to have a non-invasive test that is able to detect, localize, and evaluate the e…

SURGICAL MANAGEMENT OF DIABETIC FOOT OSTEOMYELITIS. painless due to diabetic neuropathy. Osteomyelitis is most often managed surgically after both clinico-radiological examination of the wound and treatment of an arteriopathy if present. Multiple conservative surgical techniques currently exist, depending on the infection's location, and aim to treat and prevent ulcer recurrence. These techniques are briefly described presently. Combined conservative surgery, appropriate antibiotherapy, and off-loading allow full healing in the majority of cases. Amputation should only be considered after fail…

Diabetic foot ulcers (DFUs) remain a challenge for clinicians despite decades of study. Clinicians now realize that healing strategies should focus on the patient as well as the ulcer. The search continues for interventions that affected individuals and their care providers can count on to heal and manage these challenging wounds, stopping the dreaded progression to amputation and death. The path to healing a patient's DFU often requires multidisciplinary care, including consistent offloading of insensate areas, endocrinologic and metabolic control, and improved circulation. The studies review…

Description of a clinical case of a long-standing diabetic patient, who on admission to prison presents ulcerations on both feet, of ten years of evolution. Until his admission to the Penitentiary Center he has suffered repeated hospital admissions, repeatedly proposing amputation, which the patient refused. The objective of this work is to demonstrate the importance of performing an etiological diagnosis of the lesion, taking into account all the factors that are present in a diabetic foot lesion.
