Percutaneous fixation of forefoot, midfoot, hindfoot, and ankle fracture dislocations.

Open reduction with rigid internal fixation is the basic principle for surgical management in foot and ankle trauma. High-risk patients present a surgical dilemma for the foot and ankle surgeon because the possible complications are magnified in this patient population. Percutaneous fixation is a unique alternative for achieving anatomic stabilization without increased physical strain to the patient. The significant advantages of percutaneous fixation include minimizing damage to the vascular supply, maintaining and preserving a stable soft tissue envelope, and decreasing the potential risk fo

Primary semi-constrained arthroplasty for chronic fracture-dislocations of the elbow.

We present six patients with chronic dislocation of the elbow who were treated by primary semiconstrained total elbow arthroplasty. All were women with a mean age of 65 years (51 to 76), the mean interval between dislocation and surgery was 17 weeks (5 to 52) and the mean follow-up 58 months (24 to 123). The most dramatic improvement was in function. The mean American Shoulder and Elbow Surgeon score was 5.2 times better (p < 0.001) and the mean total range of movement increased from 33 degrees to 121 degrees (p < 0.001) after operation. Three patients developed wear of polyethylene. One requi

Concomitant fractures of the femoral head and neck without hip dislocation.

This case report describes two patients who sustained ipsilateral fractures of the femoral head and femoral neck without dislocation of the hip. The fractures in the two patients resulted from vehicle accidents. The femoral head was fractured in the sagittal plane, and the femoral neck was fractured at the subcapital portion. However, the hip was not dislocated in either patient. Both patients were treated using cementless total hip arthroplasty. These unusual hip injuries have not been reported previously and are not categorized according to any known classification system of hip injuries.

[Therapy of dislocated calcaneus joint fracture with the AO calcaneus plate].

From August 1992 to March 1997, 66 patients with 71 displaced intraarticular calcaneal fractures were prospectively examined after an operative treatment using an extended lateral approach and the ASIF calcaneal plate followed by early functional postoperative treatment (mean follow-up 25 months, retrieval rate 96%). To classify the type of fracture and to verify the results of reduction and of retention CT scans in the coronal and transverse plane were performed pre- and postoperatively and on the day of assessment. The Zwipp Score was used for clinical evaluation. After fractures with 5 to 8

[Injury sequelae and late damage of dislocation fracture of the head of the tibia].

Consequences of injury as well as succeeding joint-impairment after fracture dislocation of the tibial head are analysed. Of 38 patients who had undergone operative therapy for fracture dislocation of the tibial head between 1982 and 1991, 30 were followed up after a mean of 6.3 years. With an average age of 46.8 years and a distribution between sexes of 26 men vs. 12 women, types II (17) and V (14) (classification acc. to Moore) clearly outweighed types III (3) and IV (3) as well as type I (1). Causes of injury were dominated by traffic accidents (22/38), 9 patients suffered from accompanying…

Bilateral central acetabular fracture dislocations secondary to sustained myoclonus.

This case report presents a patient with bilateral central acetabular fracture dislocations secondary to sustained myoclonus treated with delayed bilateral total hip arthroplasty. This is an unusual mechanism of injury, but is similar to other uncontrolled muscular contractions, such as electroconvulsive therapy and seizures. Because of ongoing myoclonus, the patient initially was treated nonoperatively. The patient then successfully had staged bilateral total hip arthroplasty 15 months after injury. This case exemplifies that forceful, uncontrolled muscular contraction can cause bilateral sym…

Complications of smooth pin fixation of fractures and dislocations in the hand and wrist.

A retrospective review of patients treated with internal fixation of fractures or dislocations of the hand or wrist over a four-year period was undertaken to determine the complication rates of pin fixation for stabilization of these injuries. One hundred thirty-seven patients who received 422 pins were studied. All pins were unthreaded, measured 0.035-0.069 inches (0.9-1.8 mm) in diameter, were placed with a power drill, and were left protruding through the skin. The mean time that pins were left in place was 6.5 weeks, (range, two days to 24 weeks). Minimum follow-up time was 43 days after p

Bosworth fracture-dislocation of the ankle. A case report and review of the literature.

The Bosworth fracture-dislocation is a rare fracture-dislocation of the ankle where the proximal fibular fragment lodges behind the tibia, rendering it irreducible by routine closed manipulations. Clinically the patient's foot is in severe external rotation. This external rotation makes the initial roentgenograms difficult to interpret. Consequently, the diagnosis is often overlooked, resulting in inappropriate treatment and a disastrous outcome. If properly recognized, these injuries can be successfully treated by closed or open techniques with return of near-normal ankle function. The proble

[Rehabilitative treatment in old and inveterate fracture-dislocations of the bones of the feet].

The authors present the dynamics of the clinico-roentgenologic and biomechanical results of the treatment of 57 patients with late and inveterate dislocation fractures of the bones of the foot using the method of distraction. The analysis of the economic damage has demonstrated that a continuous disturbance in the working ability of the patients with this pathology causes an economic damage to the state averaging 7043.3 +/- 259.2 rubles. The favourable anatomic and functional results obtained in the absolute majority of the patients testify to the effectiveness of the method of distraction tre

Fracture-dislocation of the lumbosacral spine. Report of a case and review of the literature.

A rare lumbosacral fracture-dislocation occurred in a 22-year-old woman. In a review of the literature describing 17 additional cases, the injury was classified as rare and usually caused by severe trauma. The mechanism of injury is hyperflexion and rotation. In general, attention is drawn to the association of fractured transverse processes as an important clue to the diagnosis. Neural injury is frequent, as are fractures of the lumbosacral facet joints and sacral promontory. Attempts at closed reduction are futile, and the treatment of choice is open reduction and internal fixation with lumb

Fracture-dislocation of the ankle with posterior entrapment of the fibula behind the tibia.

Fracture-dislocation of the ankle with entrapment of the fibula behind the tibia is a rare condition. Prompt recognition and treatment are necessary to prevent permanent disability. In three of four patients, the injury was managed by closed reduction. If the injury is accurately diagnosed and if traction and medial rotation are applied to the foot while the fibular shaft is pushed laterally, a stable, satisfactory reduction may be achieved. If closed reduction fails, a lateral approach through a small vertical incision directly over the fractured fibular malleolus is sufficient to allow open

[Fractures and dislocations of the cervical spine caused by hanging (author's transl)].

In comparison to the pattern of lesions of the soft tissue of the neck and the CVC in case of suicidal hanging 17 postmortal fracture-dislocations of the neck are investigated. It could be demonstrated that on principle in case of hanging the dens-fracture can occur. With respect to the hangman's fracture (Wood-Jones) a differentiation into 3 types is proposed.
