Operative challenges of intramedullary nailing for subtrochanteric blastic pathological femur fracture: a case report.

Prostate adenocarcinoma metastasizes to bone and forms fragile blastic lesions, which can present as dense obstacles intraoperatively. There are limited reports on the challenges surgeons face when operating through these lesions. A 60-year-old male with a pathologic subtrochanteric femur fracture in the presence of blastic lesions was successfully treated with intramedullary (IM) fixation. Pathologic fractures from blastic bone lesions are expected to increase in prevalence as survivability improves for metastatic prostate cancer. Orthopedic surgeons, when performing IM fixation for these fra

Telescoping rodding with adjunctive external fixation or threaded wires in osteogenesis imperfecta: evaluation of outcomes.

The use of locking plates with intramedullary rodding in children with osteogenesis imperfecta reduces rotational and longitudinal instability. However, adjunctive locking plates do not allow early weight-bearing. We explored the outcomes of intramedullary rodding combined with external fixation or titanium threaded wires (ThW). The study included osteogenesis imperfecta children who underwent surgery with intramedullary telescopic rods and adjunctive fixation with external fixation or ThW. The timing of and reason for implant failure were primary outcome variables. Our cohort consisted of 62

[Treatment of unstable fractures of the proximal end of the humerus using elastic curved intramedullary wires].

At the Accident Hospital Lorenz Böhler and the Accident Hospital Meidling in Vienna, 48 patients with proximal humeral fractures were treated by closed reduction, intramedullary fixation with elastic pins and immobilization for two weeks in a Velpeau type stockinette (Gilchrist) between February 1985 and February 1986. The pins have a diameter of 2 mm. At there lower end there is a 20 mm long elastic curve which is ment to anker the pin in the entry hole to the intramedullary cavity, preventing sliding out of the implant. At the upper end the last 20 mm of the wire are angulated for 15 degrees

A novel classification for aseptic femoral shaft nonunion after intramedullary nailing: a retrospective study.

BACKGROUND: Although intramedullary nailing has been established as the gold standard for treating femoral shaft fractures, nonunion following intramedullary nailing remains a major concern for clinicians, severely affecting patients' walking ability and quality of life. Presently, there are certain controversies and deficiencies in nonunion classification and treatment. Herein, we propose a novel classification system for aseptic femoral shaft nonunion after intramedullary nailing based on X-ray-assessed nailing morphology and stability. Furthermore, we sought to explore the new classificatio

Ten years' experience with femoral shaft fractures.

One hundred one patients with femoral shaft fractures treated by open reduction and intramedullary fixation have been presented. The results demonstrated no nonunions in nonpathologic bone; less than 2% infection; clinical union in 4 to 6 months; radiologic union at 6 to 8 months; excellent range of knee motion in more than 90% of patients; unsupported full weight-bearing usually present by the eighth postoperative week, and an average hospital stay of 6 to 7 weeks. The advantages and disadvantages of the most common methods of treatment of femoral shaft fractures have been reviewed. The non-o

Flexibility in fixation: an update on femur fractures in children.

Although non-operative strategies remain a viable option for the management of some children's femur fractures, surgical management has become more popular and predictable, with potential complications better recognized and thus more easily avoided. This paper addresses the operative management of children's femur fractures highlighting the multiple techniques available to treat the spectrum of pediatric femur fractures. We present the applications of Elastic Stable Intramedullary Nailing and Locked intramedullary nailing, sub-muscular or open plating, and external fixation, and discuss the re

Subtrochanteric Femur Fractures: Pearls and Pitfalls.

Management of subtrochanteric femur fractures is challenging because of the multiple planes of fracture deformity. Specific techniques starting with patient positioning and appropriate operating room table selection can improve the efficiency of the surgery. Sequential reduction techniques starting with closed methods, percutaneous techniques, and finally open clamping can be performed to obtain anatomic reduction of the fracture. The gold standard implant for definitive fixation is a locked intramedullary nail and overall outcomes are excellent if anatomic alignment and stable fixation is ach

The effects of fixation methods on the biomechanics of comminuted distal metaphyseal tibia fractures.

Bone fractures require the proper integration of fixation devices to ensure long-term biomechanical performance and accelerated recovery, while allowing for early load-bearing and equitable load-sharing. The primary objective of this research is to elucidate the effect of four different generations of fixation devices on synthetic tibiae with induced comminuted fractures. The fixation devices considered herein comprised an intramedullary nail, as well as various distal interlocking screws and plates. The overall biomechanical response was studied under quasi-static compressive loading, followi

Mathematical modelling for the comparison of plate and intramedullary osteosynthesis stability in intraarticular distal radius fractures.

BACKGROUND: A comparison between plate and intramedullary osteosynthesis techniques in terms of the biomechanical stability of distal radius fracture fixation. METHODS: Mathematical modelling was used to simulate distal radius fractures, type C1 and type C2, and to compare the stability of osteosynthesis in both techniques under several wrist joint load conditions: axial compressive loading of the radius, bending loads in volar and dorsal flexion and radial and ulnar deviation, and axial torsional loading of the forearm. RESULTS: For both type C1 and type C2 fractures, the stability of intra

Avoiding and Managing Complications After Hip Fracture Fixation.

There is an enormous burden of disease associated with the management of a failed hip fracture fixation. The goal of surgical management is to facilitate an early return to mobilization with the retention of as much independence as possible. Despite numerous studies that are focused on the care of patients with proximal femur fractures, complication rates remain high. Surgeons should review current strategies to avoid and manage complications after hip fracture fixation. This will have important implications given the detrimental consequences of failed management of hip fractures, including pe

Achilles tendon rupture with open distal tibia and fibula fracture.

The authors present a previously unreported case of an open fracture of the distal tibia and fibula with associated Achilles tendon rupture in a 27-year-old motorcyclist. Management followed established trauma principles of wound exploration, debridement, and stabilization of the fracture with reamed locked intramedullary nailing of the tibia. The injury to the Achilles tendon was not identified until surgical exploration for fixation of the fracture. This required a 2-stage operative approach, involving initial debridement and fracture fixation followed by Achilles tendon repair during the se

Cuboid Edema Syndrome Following Fixation of Proximal Fifth Metatarsal Fractures in Professional Athletes.

Stress fractures of the proximal fifth metatarsal are common injuries in elite athletes. Fixation using an intramedullary screw represents the most popular surgery performed for treating these injuries, with excellent results in most cases. However, multiple reports in the literature highlight the possibility of painful hardware, usually related to the presence of the screw head, following intramedullary fixation In this case report, we outline 4 cases of professional athletes who developed lateral-based foot symptoms following complete healing of their surgically treated proximal fifth metata

Periprosthetic fractures about total knee arthroplasty.

Periprosthetic fracture after total knee arthroplasty presents a difficult complication for many orthopaedic surgeons. These fractures occur most frequently around the distal femur followed by the patella and then tibia. These fractures are frequently complicated by poor bone quality or compromised bone due to the presence of the implants. Surgical treatment is typically necessary and requires varied techniques of open fixation, intramedullary fixation, or revision arthroplasty. Outcomes of these injuries vary widely. This review aims to describe the epidemiology, classification, treatment opt

[Update on humeral shaft fractures].

Humeral shaft fractures are relatively common, representing up to 5% of all fractures. Conservative management is the treatment of choice for most humeral shaft fractures and offers good functional and union outcomes. Age and oblique fractures of the proximal third are two risk factors for non-union. Surgical indication threshold should be lower for patients with these factors. Functional outcomes and union rates after plating and intramedullary nailing are comparable, but the likelihood of shoulder complications is higher with intramedullary nailing. Finally, there is no advantage to early ex

Biomechanical Principles of Intramedullary Nails in Veterinary and Human Medicine.

Intramedullary nails are specialized metal rods inserted into the medullary cavity of a fractured bone and secured to reduce load on the fracture site, provide stability, and permit healing. The purpose of this review is to highlight the biomechanics of orthopaedic intramedullary nailing, as well as discuss the biomechanical considerations that have shaped implant design and fixation technique in veterinary and human medicine. Relevant studies were included from the PubMed database and Google Scholar for discussion on the basic science and nail design of intramedullary nails. Implant design an
