Successful treatment of thoracolumbar fractures with short-segment pedicle instrumentation.

STUDY DESIGN: Retrospective radiographic review. OBJECTIVE: To determine whether clinical factors or common classification systems can predict the radiologic outcome of short-segment thoracolumbar fracture fixation. SUMMARY OF BACKGROUND DATA: Previous reports have indicated that short-segment thoracolumbar fracture fixation might not be appropriate for highly comminuted fractures or for patients with multiple traumatic injuries. METHODS: We conducted a retrospective radiographic review of 46 thoracolumbar fractures treated with short-segment posterior instrumentation to determine the rate

Pull Out Suture Technique for Fixing Coronoid Fractures using a Single Lateral Extensor Digitorum Communis Split Approach and Fiberwire Endobutton Fixation by Retrograde Drilling - A Case Series.

INTRODUCTION: Coronoid fractures, while less common than other elbow injuries, present with significant challenges in maintaining elbow stability and function. Restoring the normal anatomy of the coronoid process (CP) is crucial for stabilizing the elbow joint, with surgical intervention often necessary for displaced or comminuted fractures. Suture button fixation offers advantages, such as ease of use, bone-sparing fixation, and reduced reliance on intraoperative X-rays. It is especially effective for small or fragmented fractures, with tools, such as aiming devices and repositioning pliers a

[Possibility of early operations in severe traumatic shock under the protection of assisted circulation].

The results of treatment in 28 patients with extremely severe multiple injuries are analysed: 21 of them had open, comminuted, crushed fractures or tearing of 44 extremities. After a failure of antishock measures an assist extracorporeal circulation with oxygenation was instituted, and surgery was performed. In 128 experiments on dogs a explosive bullet wound was inflicted on the knee joint: a comminuted intraarticular fracture of 2-3 bones was produced, followed by 25-30% bloodletting and additional trauma of the damage site. In 23 control experiments without using the heart-lung machine but

Minimally invasive treatment of forearm double fracture in adult using Acumed forearm intramedullary nail: A case report.

BACKGROUND: Currently, open reduction internal fixation is the conventional surgical method for treatment of double ulna and radius fracture. However, open reduction is associated with a high risk of complications. This case of forearm double fracture involved a patient treated using an Acumed intramedullary nail. The patient experienced good follow-up outcomes. The Acumed forearm intramedullary nail enables early functional exercise and hastens healing of the fracture. Few studies have reported on the use of this approach for the treatment of fractures. CASE SUMMARY: A 23-year-old male patie

Posterior Stabilization of Unstable Sacral Fractures: A Single-Center Experience of Percutaneous Sacroiliac Screw and Lumbopelvic Fixation in 67 Cases.

STUDY DESIGN: This is a retrospective study. PURPOSE: Recent advances in intraoperative imaging and closed reduction techniques have led to a shifting trend toward surgical management in every unstable sacral fracture. This study aimed to evaluate the clinicoradiological outcome of the sacroiliac (SI) screw and lumbopelvic fixation (LPF) techniques and thereby delineate the indications for each. OVERVIEW OF LITERATURE: Optimal management guidelines for unstable sacral fractures are still lacking probably due to the rarity of these injuries and varying fixation trends. METHODS: Out of the 67

Double-Row Suture Anchor Fixation for Inferior Pole Patella Fracture: A Novel Technique.

Inferior pole patella fractures are challenging to treat due to small, comminuted fragments and the high tensile forces generated by the extensor mechanism. Conventional surgical fixation techniques are associated with implant irritation, migration, hardware failure, and the need for secondary procedures. This case report describes a novel double-row suture anchor fixation technique with suture tape augmentation for treating an inferior pole patella fracture. An 80-year-old patient with a displaced comminuted inferior pole fracture underwent surgical fixation using obliquely inserted double-ro

Modified hemihamate arthroplasty technique for treatment of acute proximal interphalangeal joint fracture-dislocations.

We retrospectively reviewed 11 patients who underwent modified hemihamate arthroplasty for the treatment of comminuted dorsal fracture-dislocation of the proximal interphalangeal (PIP) joint. This technique was used to minimize the potential risk of complications at the recipient site by volarly oblique osteotomy in the coronal plane at graft harvesting and anatomical repair of the detached flexor sheath. The average joint involvement of the fracture was 58.4%, and the mean duration of follow-up was 38 months. The active range of motion of the distal interphalangeal, PIP, and metacarpophalange

Treatment of subtrochanteric femur fractures using the dynamic condylar screw.

Thirty-two consecutive subtrochanteric femur fractures were treated using the 95 degree dynamic condylar screw between 1982 and 1985. All fractures were classified according to the radiographic criteria of Seinsheimer. Twenty-two patients were available for follow-up study, with an average time of 23.8 months. Functional results were rated using a new traumatic hip score rating system. The overall union rate was 77.3% (17/22), with functional results rated as excellent or good in 68% (15/22). All five technical failures were associated with extensive bony comminution. Four of these failures we

Case Report: Management of tibial fracture nonunion: a case study of treatment failure and lessons learned.

UNLABELLED: Concept of "mechanical-biological balance" is fundamental to successful fracture healing; disruption of either component can lead to healing failure. Fractures of the middle and distal tibia are commonly encountered in clinical practice, particularly when associated with comminution. Managing fracture nonunion requires a comprehensive assessment of infection status, skeletal alignment, bone defect, implant stability, and host biological characteristics. This case report aims to explore the etiological factors of tibial nonunion and to reflect upon the biomechanical mechanisms under

Plate fixation in challenging traumatic carpal scaphoid lesions.

BACKGROUND: Complex fractures of the carpal scaphoid with poles fragmentation, edges comminution, bone loss and non-union of fractures previously treated by screw fixation remain challenging for hand surgeons. The authors present the indications, advantages and results of scaphoid plating, underlining the importance of correct plate positioning well shaped onto the bone. MATERIALS AND METHODS: The study includes 11 patients presenting acute fracture with distal pole fragmentation, acute fracture with comminution and non-union after prior failure of screw fixation. All patients were treated wi

Functional Classification of Isolated Zygomatic Arch Fracture: New Proposal.

Isolated zygomatic arch fractures are common fractures in maxillofacial trauma. These fractures can trigger a functional alteration in the mouth opening and closing. Until now, there is no published classification of an isolated zygomatic arch fracture that contains the presence or not of coronoid impingement. So we propose a new classification that is easy to handle and has the largest number of scenarios that can occur in this type of fracture. The isolated zygomatic arch fracture is classified as Type I: Nondisplaced fractures, Type II: Greenstick fracture with or without coronoid impingeme

Involvement of the medial and lateral epicondyles in distal humeral coronal shear fractures: Case series and literature review.

PURPOSE: Epicondyle involvement in capitellar and trochlear fractures is often considered a simple associated lesion that does not substantially change treatment or prognosis. Although theoretically predicted in reviews, case series almost never report elbow prosthesis use in comminuted coronal shear injuries associated with epicondylar fracture in the elderly. The purpose of this study is to focus on this underestimated injury pattern that can be a negative risk factor for treatment and prognosis. METHODS: We retrospectively reviewed all cases with coronal shear fracture of the distal humeru

Fractures of the basal joint of the thumb.

The two important fractures of the trapeziometacarpal joint are Bennett's fracture and Rolando's fracture. Bennett's fracture is a fracture-dislocation of the joint. The preferred treatment is closed reduction and percutaneous pinning of the fracture with additional fixation through the trapeziometacarpal joint verified by anteroposterior (AP) and lateral roentgenograms. If the fracture is not reducible, and open reduction through the thenar approach with Herbert screw fixation is recommended. Rolando's fracture is a comminuted T- or Y-shaped fracture through the base of the first metacarpal.

Treating Unstable Distal Radius Fractures With a Nonspanning External Fixation Device: Comparison With Volar Locking Plates in Historical Control Group.

We conducted a study to compare functional and radiographic outcomes of unstable comminuted intra-articular distal radius fractures (DRFs) treated with a nonspanning external fixation device and outcomes achieved with volar locking plates in a historical control group. Clinical and radiographic data from 25 patients with these fractures, treated with the external fixation device, were compared with outcomes data from historical control matched patients with fracture patterns treated with volar locking plates. There was no statistically significant difference in the measured outcomes for wrist

Precision of posttraumatic primary orbital reconstruction using individually bent titanium mesh with and without navigation: a retrospective study.

BACKGROUND: The aim of orbital wall reconstruction is to reestablish anatomically exact orbital volumes to avoid long-term complications. Navigation could facilitate complex reconstructions. METHODS: Quality of the orbital reconstruction (n=94) was measured based on (A) volume changes and (B) on 3D shape deviations compared to the unaffected side. Volume analysis included segmentation of the orbital cavity in the pre- and post-operative 3D data set (VoXim®, IVS Solutions, Germany), and shape analysis was performed by vector-based 3D tools (Comparison®, 3Dshape, Germany). RESULTS: Orbital vol

Linked total elbow arthroplasty as treatment of distal humerus fractures.

Treatment of distal humerus fractures may be challenging, especially in the elderly patient. Total elbow replacement has been proposed as an option in selected patients. We present the results of a linked elbow replacement in 16 patients with a comminuted fracture of the distal humerus which was not considered amenable to reliable open reduction and internal fixation. At a mean follow-up of 57 months, average range of motion was from 28 degrees to 117 degrees of flexion-extension. Five patients with moderate to severe pain (31%) were not satisfied with the results of the operation. Three patie

Cartographic mapping of the skull from computed tomography scans.

Contemporary imaging technologies (i.e., computed tomography [CT] and magnetic resonance imaging) generate an overwhelming amount of data. Salient features from these data must be abstracted and presented to facilitate anatomical comprehension and imaging utility. Three-dimensional surface and volumetric reformations of CT data have extended the integration of high resolution CT data to the study and management of craniofacial deformities. Nonetheless, conventional displays of such imaging require multiple images to assess symmetry, sutural status, and traumatic disruption of the skull. We the

3D atlas-based registration can calculate malalignment of femoral shaft fractures in six degrees of freedom.

OBJECTIVE: This study presents and evaluates a semi-automated algorithm for quantifying malalignment in complex femoral shaft fractures from a single intraoperative cone-beam CT (CBCT) image of the fractured limb. METHODS: CBCT images were acquired of complex comminuted diaphyseal fractures created in 9 cadaveric femora (27 cases). Scans were segmented using intensity-based thresholding, yielding image stacks of the proximal, distal and comminuted bone. Semi-deformable and rigid affine registrations to an intact femur atlas (synthetic or cadaveric-based) were performed to transform the distal
