PURE healthy diet score, genetic susceptibility, and incident chronic kidney disease.

Background: By using the Prospective Urban Rural Epidemiology (PURE) score-a novel dietary pattern that includes exclusively protective foods-we investigated the relationship between PURE score and the risk of chronic kidney disease (CKD), and compared its performance with some conventional dietary patterns, including the Dietary Approaches to Stop Hypertension (DASH), Alternate Mediterranean diet (aMed), Alternate Healthy Eating Index-2010 (AHEI-2010), and healthful Plant-Based Diet Index (hPDI). Methods: 179 569 participants without CKD at the baseline from the UK Biobank were included. Diet

Integrating genetic and imaging information to enhance cardiovascular risk stratification: rationale and design of the CVRISK-IT randomized controlled trial.

AIMS: Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality worldwide. Current guidelines recommend algorithms to estimate 10-year CVD risk in apparently healthy individuals, but their precision remains limited. Whether the integration of imaging and genetic information can improve individual risk stratification remains uncertain. METHODS AND RESULTS: The CVRISK-IT trial is a multicentre, open-label, randomized controlled study designed to enhance CVD prevention through precision-based risk assessment and targeted communication strategies. A total of 30 000 adults

Advanced Bayesian BMD-Derived Genome-Wide Polygenic Scores Enhance Clinical FRAX-Based Fracture Risk Prediction in Postmenopausal Women.

Current fracture prediction tools, including the Fracture Risk Assessment Tool (FRAX), rely primarily on clinical risk factors and bone mineral density measurements, yet omit genetic information. Given osteoporosis's substantial heritability (estimated at 50-80%), excluding genetic data may lead to clinical misclassification and uncertainty, particularly among postmenopausal women near clinical intervention thresholds. This study aimed to enhance clinical fracture risk prediction by integrating advanced Bayesian-derived genome-wide polygenic scores (GPS) into FRAX (Bayesian GPS-FRAX). Using ge

Polygenic Risk Score as a Predictor of Bone Fracture or Osteoporosis in Prostate Cancer Patients Receiving Androgen Deprivation Therapy.

BACKGROUND: The association between single nucleotide polymorphisms (SNPs) and fracture risk has been established in various studies. Androgen-deprivation therapy (ADT) for prostate cancer, while extending survival, increases the risk of osteoporosis and bone fracture. This study aimed to assess the predictive value of polygenic risk scores (PRSs) on bone events among prostate cancer patients undergoing ADT. METHODS: We enrolled 24,778 male participants, including 903 prostate cancer patients treated at Taichung Veterans General Hospital. These patients were divided into ADT and non-ADT group

Developing a Phenotype Risk Score for Tic Disorders in a Large, Clinical Biobank.

IMPORTANCE: Tics are a common feature of early-onset neurodevelopmental disorders, characterized by involuntary and repetitive movements or sounds. Despite affecting up to 2% of young children and having a genetic contribution, the underlying causes remain poorly understood, likely due to the complex phenotypic and genetic heterogeneity among affected individuals. OBJECTIVE: In this study, we leverage dense phenotype information from electronic health records to identify the disease features associated with tic disorders within the context of a clinical biobank. These disease features are the

Bladder pain syndrome/interstitial cystitis in twin sisters.

PURPOSE: We determined the genetic contribution of and associated factors for bladder pain syndrome using an identical twin model. MATERIALS AND METHODS: Multiple questionnaires were administered to adult identical twin sister pairs. The O'Leary-Sant Interstitial Cystitis Symptom and Problem Index was administered to identify individuals at risk for bladder pain syndrome. Potential associated factors were modeled against the bladder pain syndrome score with the twin pair as a random effect of the factor on the bladder pain syndrome score. Variables that showed a significant relationship with

Assessing Shared Decision-Making Clinical Behaviors Among Genetic Counsellors.

Shared decision-making (SDM) is a collaborative approach in which clinicians educate, support, and guide patients as they make informed, value-congruent decisions. SDM improves patients' health-related outcomes through increasing knowledge, reducing decisional conflict, and enhancing experience of care. We measured SDM in genetic counselling appointments with 27 pregnant women who were at increased risk to have a baby with a genetic abnormality. The eight experienced genetic counsellors who participated had no specific SDM training and were unaware that SDM was being assessed. Audio transcript

Heterogeneity in the association of genetic risk for RA and resultant RA phenotypes.

OBJECTIVES: The impact of the genetic risk for RA on resultant disease phenotype in RA is incompletely understood. Using individual genetic variants associated with RA and a polygenic score (PGS), we hypothesized that those with higher genetic risk for RA would demonstrate a more severe disease course. METHODS: We genotyped participants from the Veterans Affairs RA registry (VARA), a prospective cohort of RA. We evaluated associations between PTPN22 R620W genotype, HLA-DRB1 shared epitope (SE; 0, 1, or 2 copies of a high-risk HLA allele), and a non-HLA PGS with RA disease activity scores and

Using machine learning and single nucleotide polymorphisms for improving rheumatoid arthritis risk Prediction in postmenopausal women.

Genetic factors contribute to 60-70% of the variability in rheumatoid arthritis (RA). However, few studies have used genetic variants to predict RA risk. This study aimed to enhance RA risk prediction by leveraging single nucleotide polymorphisms (SNPs) through machine-learning algorithms, utilizing Women's Health Initiative data. We developed four predictive models: 1) based on common RA risk factors, 2) model 1 incorporating polygenic risk scores (PRS) with principal components, 3) model 1 and SNPs after feature reduction, and 4) model 1 and SNPs with kernel principal component analysis. Polygenic depression risk, childhood parental substance abuse, and G×E interaction in divergent depression trajectories from middle to late adulthood.

OBJECTIVE: National life course studies examining the interplay between genetic risk, parental substance abuse, and lifelong depression are lacking, despite the need for individualized interventions. This study investigates how the interaction between polygenic risk for depression and childhood exposure to parental substance abuse is associated with depression trajectories from mid-to late adulthood. METHOD: Data from 14 waves (1994-2020) of the Health and Retirement Study included 7512 participants of European ancestry aged 51-90 years. Primary predictors were childhood parental substance ab

Role of psychological background in cancer susceptibility genetic testing distress: It is not only about a positive result.

Clinical and familial factors predict psychological distress after genetic testing for cancer susceptibility. However, the contribution of an individual's psychological background to such distress is unclear. This study aims to analyze the psychological impact of genetic testing and to identify the profile of individuals at higher risk. This is a longitudinal multicenter study of individuals undergoing genetic testing for cancer susceptibility. Demographic, clinical, genetic, familial, and psychological (personality types, cancer worry) characteristics were assessed by validated questionnaires

Elevated polygenic burden for autism is associated with differential DNA methylation at birth.

BACKGROUND: Autism spectrum disorder (ASD) is a severe neurodevelopmental disorder characterized by deficits in social communication and restricted, repetitive behaviors, interests, or activities. The etiology of ASD involves both inherited and environmental risk factors, with epigenetic processes hypothesized as one mechanism by which both genetic and non-genetic variation influence gene regulation and pathogenesis. The aim of this study was to identify DNA methylation biomarkers of ASD detectable at birth. METHODS: We quantified neonatal methylomic variation in 1263 infants-of whom ~ 50% we

Polygenic Risk of Epilepsy and Poststroke Epilepsy.

BACKGROUND: Epilepsy is highly heritable, with numerous known genetic risk loci. However, the genetic predisposition's role in poststroke epilepsy (PSE) remains understudied. This study assesses whether a higher genetic predisposition to epilepsy raises poststroke survivor's risk of PSE. METHODS: We conducted a case-control genetic association study nested within the UK Biobank, a large UK-based prospective cohort. Our exposures of interest were 2 distinct polygenic risk scores-generalized and focal epilepsy-modeled as deciles and constructed using genetic variants identified in the latest In

A polygenic risk score for the QT interval is an independent predictor of drug-induced QT prolongation.

Drug-induced QT prolongation (diLQTS), and subsequent risk of torsade de pointes, is a major concern with use of many medications, including for non-cardiac conditions. The possibility that genetic risk, in the form of polygenic risk scores (PGS), could be integrated into prediction of risk of diLQTS has great potential, although it is unknown how genetic risk is related to clinical risk factors as might be applied in clinical decision-making. In this study, we examined the PGS for QT interval in 2500 subjects exposed to a known QT-prolonging drug on prolongation of the QT interval over 500ms

Particulate matter pollution, polygenic risk score and mosaic loss of chromosome Y in middle-aged and older men from the Dongfeng-Tongji cohort study.

Mosaic loss of chromosome Y (mLOY) is the most common somatic alteration as men aging and may reflect genome instability. PM exposure is a major health concern worldwide, but its effects with genetic factors on mLOY has never been investigated. Here we explored the associations of PM2.5 and PM10 exposure with mLOY of 10,158 males measured via signal intensity of 2186 probes in male-specific chromosome-Y region from Illumina array data. The interactive and joint effects of PM2.5 and PM10 with genetic factors and smoking on mLOY were further evaluated. Compared with the lowest tertiles of PM2.5

Large-Scale Plasma Proteomics Profiles for Predicting Atrial Fibrillation Associated Stroke Risk : Type of manuscript: Original Research.

BACKGROUND: Stroke is a major complication of atrial fibrillation (AF), and risk prediction using the congestive heart failure, hypertension, age, diabetes, stroke, vascular disease, and sex category score (CHA₂DS₂-VASc) remains limited by residual heterogeneity. We aimed to identify plasma proteins associated with post-AF stroke and evaluate whether a protein score provides incremental predictive information beyond CHA₂DS₂-VASc. METHODS: We analyzed 709 AF participants from the UK Biobank Pharma Proteomics Project, with 76 incident strokes. Stroke-related proteins were identified using multi
