Effectiveness of an information-driven targeted isolation strategy in reducing healthcare-associated infections caused by multidrug-resistant gram-negative bacteria in the intensive care unit: A prospective uncontrolled before-after study.

BACKGROUND: Cross-transmission risk of multidrug-resistant Gram-negative bacteria (MDR-GNB) is substantially higher in intensive care units (ICUs). Broad patient isolation is resource-intensive, prompting this study to assess whether a validated MDR-GNB healthcare-associated infection (HAI) risk calculator-guided targeted preemptive isolation could reduce cross-transmission. METHODS: A prospective uncontrolled before-after study was conducted across four adult ICUs in two tertiary hospitals in Urumqi, with two 3-month phases separated by a 2-week washout period. Standard precautions were appl…

From Bench-Top to Bedside: A Prospective In Vitro Antibiotic Combination Testing (iACT) Service to Guide the Selection of Rationally Optimized Antimicrobial Combinations against Extensively Drug Resistant (XDR) Gram Negative Bacteria (GNB).

INTRODUCTION: Combination therapy is increasingly utilized against extensively-drug resistant (XDR) Gram negative bacteria (GNB). However, choosing a combination can be problematic as effective combinations are often strain-specific. An in vitro antibiotic combination testing (iACT) service, aimed to guide the selection of individualized and rationally optimized combination regimens within 48 hours, was developed. We described the role and feasibility of the iACT service in guiding individualized antibiotic combination selection in patients with XDR-GNB infections. METHODS: A retrospective ca

Risk Factors for Carbapenem Resistant Gram Negative Bacteria (CR-GNB) Carriage Upon Admission to the Gastroenterology Department in a Tertiary First Class Hospital of China: Development and Assessment of a New Predictive Nomogram.

BACKGROUND: With the increasing number of critically ill patients in the gastroenterology department (GED), infections associated with Carbapenem resistant gram-negative bacteria (CR-GNB) are of great concern in GED. As the turn-around time (TAT) for a positive screening culture result is slow, contact precaution and pre-emptive isolation, cohorting methods should be undertaken immediately on admission for high-risk patients. Accurate prediction tools for CR-GNB colonization in GED can help determine target populations upon admission. And thus, clinicians and nurses can implement preventive me…
