The tools of an evidence-based culture: implementing clinical-practice guidelines in an Israeli HMO.

PURPOSE: Although clinical-practice guidelines (CPGs) are implemented on the assumption that they will improve the quality, efficiency, and consistency of health care, they generally have limited effect in changing physicians' behavior. The purpose of this study was to design and implement an effective program for formulating, promulgating, and implementing CPGs to foster the development of an evidence-based culture in an Israeli HMO. METHOD: The authors implemented a four-stage program of stepwise collaborative efforts with academic institutions composed of developing quantitative tools to e

Children's health outcomes in six different ambulatory care delivery systems.

This study assesses health outcomes across six widely varying delivery settings within one population sample, describing differences between actual and expected illness prevalences among children using each type of setting as a regular source. Illness measures are from independent clinical examinations of the children for anemia, acute and chronic ear problems, hearing loss and vision disorders. Comparisons of actual with expected prevalences based on each setting's patient clientele reveal three systematic patterns: children using solo practitioners had generally higher-than-expected illness

Self-Controlled Feedback on Motor Learning and Neuroplastic Changes in People With Parkinson Disease.

BACKGROUND AND PURPOSE: People with Parkinson disease (PD) experience impaired motor learning. Although numerous studies have shown that self-controlled feedback can facilitate motor learning in healthy young adults, whether similar benefits can be observed in people with PD has been less investigated. This study thus aimed to examine whether self-controlled feedback could benefit motor learning for individuals with PD, and to determine the associated behavioral and neurophysiological mechanisms. METHODS: Thirty-two participants with PD, matched in pairs, were recruited into a self-control gr

Efficacy, Safety, and Practicality of Tacrolimus Monitoring after Bone Marrow Transplant: Assessment of a Change in Practice.

BackgroundCurrently, there is no standardized approach to the frequency of monitoring tacrolimus levels in patients who have undergone hematopoietic stem cell transplant (HSCT). Previously, the practice at the study hospital was to monitor tacrolimus levels daily throughout a patient's admission. A recent institutional study suggested that measurement of tacrolimus level is more frequent than needed to achieve consistent time in the therapeutic range (TTR), particularly after the first 7 days. As a result, tacrolimus monitoring was changed to daily measurement for the initial week of therapy,

Effects of mental practice on mobility of individuals in the early subacute post-stroke phase: A randomized controlled clinical trial.

INTRODUCTION: Only a small number of clinical trials were designed to investigate Mental Practice (MP)'s use for gait rehabilitation in individuals in the early subacute post-stroke phase. This trial aims to investigate the effect of mental practice on mobility rehabilitation in the early subacute phase after a stroke in comparison to a control group. METHODS: Randomized controlled clinical trial with 16 individuals diagnosed with a stroke between 50 and 80 years of age. Mobility was evaluated using Timed Up and Go and the Five-Minute Walk Test. In addition, lower extremity muscular strength,

Are we overusing blood transfusing after elective joint replacement?--a simple method to reduce the use of a scarce resource.

OBJECTIVES: To determine the proportion of patients who received a blood transfusion after joint replacement, and to devise a simple method to ensure patients were transfused based on strict clinical and haematological need. DESIGN: Prospective audit over 2 years. PATIENTS AND METHODS: The study group was 151 patients who underwent total hip and knee arthroplasty in a typical district general hospital (Kettering) over a 2-year period. They were divided into three consecutive groups. Current practice was audited (producing the first group of 62 patients) and transfusion rates were compared to

General practice organisational models and heart failure medication adherence: multi-level evidence from a regional cohort in Emilia-Romagna, Italy.

BACKGROUND: Medication adherence is essential for improving heart failure outcomes yet remains suboptimal. Organisational models in general practice-such as group practices and Community Health Centres-have long been promoted as a means to strengthen primary care and are currently undergoing national reform. However, their impact on adherence remains unclear. This study investigated whether general practice organisational arrangements were associated with adherence to therapies recommended by clinical guidelines for heart failure. METHODS: We conducted a retrospective cohort study using linke…

[What is the role of otolaryngologists in deglutition practice?--A survey for medical practitioners in different fields].

The advent of a super aging society is causing a rapid increase in the number of patients with dysphagia, and, in response, the use of percutaneous endoscopic gastrostomy (PEG) has become markedly widespread over the past decade. As the result of its rapid spread, PEG is controversial both ethically and economically, and, in the revision of medical treatment fees for 2014, the preoperative deglutition usability test of all cases is fixed for the full amount request requirements of gastrostomy. Thus, the demands on dysphagia practice are rising. Therefore, we conducted a survey of the dysphagia

Retrieval-induced forgetting under psychosocial stress: no reduction by delayed stress and beta-adrenergic blockade.

Retrieval-induced forgetting (RIF) is the phenomenon that 'retrieval-practice', the repeated retrieval of a subset of initially learned material, can impair the recall of episodically related memories. Previous studies showed that RIF is eliminated when retrieval-practice is carried out under psycho-social stress, anxiety, or in negative mood. However, pharmacological manipulation by hydrocortisone did not eliminate the effect. This study investigated the effect of beta-adrenergic blockade on stress-induced modulations of RIF, addressing possible interactive effects of the glucocorticoid and s

The application effect of the segmented teaching method in training medical students on clinical practice skills.

BACKGROUND: The surgical clinical practice skill training is an important part of medical undergraduate education. Surgical skills are complex and difficult to master. However, the traditional teaching method has some disadvantages, for example, low student participation, weak learning atmosphere, inadequate mastery of the subject matter. We innovatively put forward the segmented teaching method in the field of clinical practice skills training. The segmented teaching method refers to dividing a specific teaching content into several relatively independent parts and providing segmented practic

A randomized controlled trial of changes in resting-state functional connectivity associated with short-term motor learning of chopstick use with the non-dominant hand.

INTRODUCTION: This study identified the offline brain networks associated with motor learning of non-dominant hand chopstick use within-session. METHODS: 40 healthy right-handed adults were randomly assigned to the practice and control groups (20 each). The performance, resting-state functional connectivity (RSFC), and their correlation were compared within and between groups. Both groups repeated 9 cycles of 30 s task and rest. During the task, the practice group performed the chopstick-use practice with their left hand, while the control group held chopsticks without acquiring any skills. D

Substituting some unassisted practice with robotic guidance: Assessing the feasibility of auditory-cued mixed practice for music-based interventions.

BACKGROUND: There is equivocal evidence regarding the effectiveness of robotic guidance on the (re)learning of voluntary motor skills. Robotic guidance can improve the performance of continuous/ tracking skills, although being seldom more effective than unassisted practice alone. However, most of the previous studies employed robotic guidance on all intervention trials. Recently, we showed that mixing robotic guidance with unassisted practice (i.e., mixed practice) can significantly improve the learning of a golf putting task. Yet, these mixed practice studies involved self-paced movements in
