OBJECTIVES: To evaluate efficacy of laser lipolysis in the treatment of gynecomastia to correct breast volume, flaccidity and excess skin without its excision.
METHODS: Prospectively, 32 patients with gynecomastia under tumescent anaesthesia and sedation underwent laser lipolysis with 980 nm diode laser, 15W continuous emission and 8 to 12 kJ energy per breast. A 68-year-old man with a 3-year history of rheumatoid arthritis (RA) developed gynecomastia 3 months after beginning oral low-dose methotrexate (MTX) therapy. Four months after MTX therapy was discontinued, the gynecomastia symptoms improved. Only eight cases of gynecomastia resulting from low-dose MTX administration have been reported worldwide, and no cases have previously been reported in Japan. Although it occurs infrequently, gynecomastia resulting from low-dose MTX therapy should be considered in male patients with RA.

Nonscarring sparing methods are preferred, and the minimally invasive technique is to use liposuction for the gland and the fatty tissue exclusively. Over the last decade, suction-assisted lipoplasty has been shown to be a safe and reliable procedure in the hands of trained individuals. A large number of plastic surgeons employ liposuction as an adjunct to surgery for gynecomastia with excellent results and low morbidity. Loss of libido, decreased hair growth and gynecomastia developed in a 61-year-old man after six months of applying an estrogen-containing hair tincture for diffuse alopecia areata of the head hair. All these signs disappeared within four weeks of stopping the application, and hormone levels were again normal. Locally applied estrogen preparations are in general contraindicated in males because of their potential systemic effects. If they have to be given, hormone levels (testosterone; estradiol) must be checked.

The purpose of this study was to provide a medical alternative to mastectomy in boys with excessive breast development. Eleven boys with bilateral gynecomastia, ranging in size between 9 X 7 cm and 3 X 3 cm in diameter, were treated with 200 mg of Danazol daily for 6 months.
