Cardiac injuries in aircraft occupants resulting from aircraft accidents.

The author reports that relatively frequent injuries to the thorax and to the heart are established in autopsies of aircraft fatalities. A review of 128 autopsy protocols of aircraft accident fatalities revealed 111 cases of severe contact injuries to the thorax and 98 cases of traumatic injuries to the pericardium and the heart. Accepting the fact that persons injured if traffic accidents display cardiac injuries in 25% of the cases, the number of traumatic heart lesions inflicted in persons in aircarft accidents is three times as high, a fact which may be attributed to the far higher speeds

[Aortic valve insufficiency due to rupture of the cusp in a patient with multiple trauma].

Lesions of the heart valve caused by blunt chest trauma is rare, but when it does occur it can significantly injure the patient. On the basis of autopsy studies, research shows that heart valves are injured in less than 5% of patients who have died due to impact thoracic trauma. Among the heart valves, the aortic valve is the most often lacerated, which has been proved by relevant autopsy and clinical studies. Aortic valve lesions can be the only injury, but it is possible that additional heart or large vessel injuries are also present (myocardial contusion, rupture of the atrial septum, aorti

A Longitudinal Study of Resting Heart Rate and Violent Criminality in More Than 700 000 Men.

IMPORTANCE: Low resting heart rate is a well-replicated physiological correlate of aggressive and antisocial behavior in children and adolescents, but whether low resting heart rate increases the risk of violence and other antisocial and risk-taking behaviors in adulthood has not been studied in representative samples. OBJECTIVE: To study the predictive association of resting heart rate with violent and nonviolent criminality and with fatal and nonfatal injuries owing to assaults and unintentional injuries in the population. DESIGN, SETTING, AND PARTICIPANTS: We conducted a study of data fro

Falls in Patients With Heart Failure: A Systematic Review.

BACKGROUND: Many heart failure patients show fall-related signs/symptoms including postural hypotension, cerebellar injury, and cognitive impairments. Falls contribute to injuries, increased healthcare use, and death, but falls have been understudied in this population. OBJECTIVE: The purpose of this review is to identify fall rates, fall injuries, and risk factors for falls in heart failure patients. METHODS: A systematic literature review was conducted using MEDLINE, CINAHL, PubMed, PsycINFO, and Cochrane Library to identify publications from August 1973 to June 2013. Keywords were acciden

The Journey of Harmless Bullet: The Perioperative Care of Penetrating Cardiac Injury.

Traumatic injuries to the heart contribute significantly to trauma are associated with high mortality. Cardiac gunshot wounds (GSW) are considered more lethal compared to other injuries and present several unique challenges to the anesthesia management and perioperative care. We are reporting a rare case of a trauma victim who survived a GSW to the heart. We will discuss the perioperative care of penetrating cardiac injuries, the role of the anesthesia team in resuscitation, safe anesthesia induction, cardiopulmonary bypass management, and the essential role of intraoperative transesophageal e

Association of dietary macronutrients with MRI-detected vascular brain injury and cognition in 9886 middle-aged participants from four countries: for the Canadian Alliance of Healthy Hearts and Minds (CAHHM) and the Prospective Urban Rural Epidemiological (PURE) Study Investigators.

BACKGROUND: Epidemiologic evidence relating macronutrient intake and changes in the brain and cognition are limited. We assessed the associations of macronutrient consumption, including carbohydrate, protein, total fat, saturated fatty acids, monounsaturated fatty acids, and polyunsaturated fatty acids, with vascular brain injury and cognitive scores. METHODS: We analyzed cross-sectional data from 9886 middle-aged adults in four countries free of clinically apparent cardiovascular disease from the Canadian Alliance for Healthy Hearts and Minds (CAHHM) and the Prospective Urban Rural Epidemiol

Critical Surgical Management Pearls in Penetrating Cardiac Nail Gun Injury.

BACKGROUND: Penetrating cardiac injuries are typically fatal. Nail gun injuries to the heart are rare, and surgical repair is warranted. CASE SUMMARY: This case details a rare, self-inflicted injury where multiple nails were discharged into a patient's heart and head. Remaining hemodynamically stable at presentation, he underwent a computed tomography scan to delineate the trajectory and location of the intracardiac nails. Successful removal and repair of the myocardium was carried out under cardiopulmonary bypass. Subsequent removal of the intracranial nails was performed, and the patient ma

Brain-Heart Axis and the Inflammatory Response: Connecting Stroke and Cardiac Dysfunction.

BACKGROUND: In recent years, the mechanistic interaction between the brain and heart has been explored in detail, which explains the effects of brain injuries on the heart and those of cardiac dysfunction on the brain. Brain injuries are the predominant cause of post-stroke deaths, and cardiac dysfunction is the second leading cause of mortality after stroke onset. SUMMARY: Several studies have reported the association between brain injuries and cardiac dysfunction. Therefore, it is necessary to study the influence on the heart post-stroke to understand the underlying mechanisms of stroke and

[The sympathetic innervation of the myocardium and its damage in acute massive pulmonary embolism].

The state of the adrenergic heart innervation and injuries of right (RV) and left (LV) ventricles in experimental massive embolism of pulmonary arteries (MEPA) were studied in 62 dogs under conditions of the intact thorax and natural respiration. The degree of focal myocardial injuries depended upon the density of the sympathetic innervation: when the density of adrenergic nerves (AN) was high the number of injuries decreased. Hypercatecholaminemia developing at the early period of MEPA has a cardiotoxic effect, more pronounced in dogs with decompensation. A decrease of the AN density and incr

Stress-induced arrhythmic disease of the heart--Part I.

This review deals with the following principal concepts: (1) Heart injuries in single severe stress episodes manifested primarily in disturbances of membrane lipid bilayer, sarcolemmal Na, K-pump, and sarcoplasmic Ca-pump with concurrent limited disturbances of the heart energy supply, namely, of the creatine kinase and glycolysis systems. These disturbances cause small focal myocardial lesions and reduce cardiac electrical stability: the fibrillation threshold falls and ectopic activity increases. In repeated stress, this damage, localized mainly in the richly innervated conduction system, ac…

Aging is only significant factor causing CPR-induced injuries and serious injuries.

BACKGROUND: Cardiopulmonary resuscitation (CPR) sometime cause severe injuries and can affect quality of life, lead to long-term disabilities or death of the patient. The aim of this study is to identify the risk factors causing CPR-induced injuries and those of serious injuries. METHODS: This was a retrospective forensic autopsy study in a single institution. Among 885 forensic autopsies undertaken between 2011 and 2018, those in which the victim had undergone CPR immediately after cardiac arrest were recorded. 'Serious injuries' were defined as an Abbreviated Injury Scale (AIS) score ≥ 3. C

Mild decrease in heart rate during early phase of targeted temperature management following tachycardia on admission is associated with unfavorable neurological outcomes after severe traumatic brain injury: a post hoc analysis of a multicenter randomized controlled trial.

BACKGROUND: The association between isolated admission heart rate (HR) and prognosis has been discussed, but not that between gross HR change and neurological outcome in patients with severe traumatic brain injury (TBI). In the acute phase of severe TBI, HR is influenced by several factors (e.g., pain, sympathetic activation, hypovolemia, fever, body temperature). Therefore, admission HR and gross HR change should be examined in patients with TBI treated with a well-designed protocol, such as was done in the Brain Hypothermia (B-HYPO) Study. METHODS: This was a post hoc analysis of the B-HYPO

[Therapeutic principles and priorities of polytrauma in general surgery (author's transl)].

The absolutely prior task in patients with multiple injuries is to maintain the cardiac and pulmonary functions. Furthermore a quick decision is needed regarding additional life-saving operations. Apart from a careful clinical investigation simple diagnostic procedures are necessary (chest x-ray, peritoneal lavage). The life of the injured person is endangered through hemorrhages. The most effective therapy is to recognize and to stop these bleedings. The surgical repair of intestinal ruptures, injuries to the pancreas, ruptures of the diaphragm and other problems can be postponed. The same ap

Traumatic tricuspid valve regurgitation: A two case series.

Traumatic tricuspid valve injury is rare, accounting for 0.02% of traumatic injuries. The majority of cases result from blunt force trauma to the chest, however penetrating injuries have been documented in literature. Patients' can be in the full spectrum of disease, from asymptomatic to cardiogenic shock. Indications for surgery include right heart failure or evidence of right heart volume overload in the setting of significant tricuspid regurgitation. Early surgical repair is warranted to preserve right ventricular function. Surgery also needs to be planned in conjunction with the patients'

Transient receptor potential vanilloid 1, calcitonin gene-related peptide, and substance P mediate nociception in acute pancreatitis.

Autonomic dysreflexia (AD) can occur during penile vibratory stimulation in men with spinal cord injury, but this is variable, and the association with lesion level is unclear. The purpose of this study was to characterize the cardiovascular responses to penile vibratory stimulation in men with spinal cord injury. We hypothesized that those with cervical injuries would demonstrate a greater degree of AD compared with men with thoracic injuries. We also questioned whether the rise in blood pressure could be attenuated by sildenafil citrate. Participants were classified as having cervical (n = 8

Shockwave cardiac injury in thoracoabdominal gunshot wound.

INTRODUCTION AND IMPORTANCE: Cardiac injuries are associated with high mortality rates and most affected individuals succumb to their injuries before arrival to the hospital. Even though they have a higher fatality rate, penetrating cardiac injuries are relatively easy to diagnose and they have straightforward management protocols. On the other hand unexpected non-penetrating cardiac injuries, especially in haemodynamically stable patients, are not looked out for and can be difficult to diagnose. They may have a delayed presentation leading to poor patient outcomes. Clinicians should have a hi

Complete cardiac rupture associated with closed chest cardiac massage: case report and review of the literature.

Chest skeletal injuries are the most frequent complications of external chest massage (ECM) during cardiopulmonary resuscitation, but heart and great vessels lacerations that are indeed very rare. We report the case of a 35-year-old workman who collapsed and underwent ECM by his co-workers for almost 30 min. At autopsy, no external injuries, fractures or bruises of the ribs or sternum, were observed. A hemopericardium with a rupture of the heart was found, with no signs of pre-existent cardiac disease. Vehicular accidents and cardiac concussion. A traumatic connection.

Concussion of the heart (commotio cordis) occurs in traffic accidents in which blunt force impact over the precordial area occurs, even in the absence of multiple blunt force injuries. The functional dynamics of the heart, with ventricular arrythmias and conduction disturbances, serve as the pathophysiological basis of this condition. As a cause of death in vehicular accidents, it is less common than other causes. However, it should be considered in the differential diagnosis, especially in cases with minor or no apparent injuries.
