Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired disorder characterized by complement-mediated hemolysis and a high thrombotic risk. The introduction of complement inhibitors has markedly reduced thromboembolic events. Eculizumab, the first approved C5 inhibitor, requires biweekly infusions, while ravulizumab, with a prolonged half-life, allows administration every eight weeks. To compare their effects on coagulation dynamics, we retrospectively analyzed paired plasma samples from nine PNH patients sequentially treated with both agents using the Thrombin Generation Assay TGA. TGA p…

Autoimmune hemolytic anemia (AIHA) is caused by auto-antibodies directed against self red blood cell (RBC) surface antigens. A consequence may be an intravascular hemolysis mediated by activated complement or extravascular hemolysis caused by destruction of complex of RBC with autoantibody in spleen and liver. The basic classification subdivides AIHA in primary/idiopathic and secondary with known underlying disease. A classification according to the thermal range of antibody recognizes warm AIHA, cold aglutinin disease (CAD), mixed AIHA and paroxysmal cold hemoglobinuria. Pathogenesis of AIHA …

Mutations of isocitrate dehydrogenase isoform 1 and 2 (IDH1 and IDH2) genes have been identified in glioblastoma and acute myeloid leukemia (AML). However, little is known about the molecular alterations of IDH genes in preleukemic disorders with a propensity to transform to AML. We performed polymerase chain reaction-denaturing high performance liquid chromatography (PCR-DHPLC) followed by direct sequencing to detect IDH mutations in 237 patients with myeloproliferative neoplasms (MPNs; n=108), myelodysplastic syndrome (MDS; n=22), paroxysmal nocturnal hemoglobinuria (PNH; n=41), and aplastic…

Renal thrombotic microangiopathy (TMA) is a severe complication of systemic lupus erythematosus (SLE), which is associated with the presence of antiphospholipid (aPL) antibodies. In its most fulminant form, TMA leads to a rapid and irreversible end-stage renal failure. Eculizumab, an anti-C5 monoclonal antibody, is a novel therapy of choice for patients with paroxysmal nocturnal hemoglobinuria (PNH) and atypical hemolytic uremic syndrome. Here, we report the case of a 27-year-old woman, known for SLE and end-stage renal disease due to fulminant TMA. Both aPL antibodies and antinucleosome antib…

This case report represents a documentation of severe envenomation by Proatheris superciliaris, a species for which no specific antivenom exists. A 27-year-old male bitten on the finger by an adult P. superciliaris developed immediate severe pain radiating up the limb and local ecchymotic features with tissue necrosis ensuing over the following 36h, requiring debridement of necrotic tissue. Approximately 72h post-envenomation jaundice and hemoglobinuria were observed, but no other clinical signs of coagulopathy or hemorrhage. Hemolysis was evidenced by fragmented red blood cells and hyperbilir…

Novel techniques were used to detect which cell lineages were affected by monosomy 7 in a patient who had myelodysplastic syndrome and later developed acute leukemia. The patient had had paroxysmal nocturnal hemoglobinuria for 20 years before developing refractory anemia with excess of blasts. Cytogenetic analysis at the myelodysplastic stage disclosed monosomy 7 in bone marrow mitoses. Restriction fragment length polymorphism analysis of fractionated white blood cells with the chromosome 7-specific probes MetH and MetD revealed that blood monocytes and most bone marrow erythroblasts but not b…

Paroxysmal cold hemoglobinuria (PCH) is a rare form of complement-mediated hemolytic anemia in children. It is typically triggered by viral infections and mediated by Donath-Landsteiner antibodies, which induce intravascular hemolysis upon cold exposure. We reviewed the clinical course, laboratory findings, and management of a pediatric adenovirus-associated PCH to highlight diagnostic and therapeutic considerations. The eight-year-old previously healthy, unimmunized female patient presented with acute hemolytic anemia following a febrile viral illness. She developed jaundice and severe anemia…

The complement system, which plays a key role in innate immunity, facilitates the elimination of foreign and endogenous substances in the body. Consequently, while a balance between complement activation and regulation is typically maintained, a disruption of this balance can lead to various diseases. Paroxysmal nocturnal hemoglobinuria and atypical hemolytic uremic syndrome (aHUS) are diseases in which abnormal complement regulatory factors cause excessive complement activation. To accurately diagnose diseases such as aHUS, international standard complement tests including activation markers …

BACKGROUND: Clinically insignificant hemolytic anemia is occasionally a complication of prosthetic valve replacement. However, hemolysis rel … nd lactate dehydrogenase levels increased to 3071 IU/L with gross change in urine color, indicating hemoglobinuria. We diagnosed mechanical hemolytic anemia caused by a kinked graft and planned a repeat operation. The kinked graft was resected and graft-graft anastomosis was performed. Postoperatively, the clinical course was uneventful, and the hemolytic anemia …

Gastric varices are less common than esophageal varices, and their treatment is quite challenging. Gastric varix bleedings (GVB) occur less … TO and leads to worsening of esophageal varix pressure. Other significant complications may include hemoglobinuria, abdominal pain, fever, and pleural effusion. Shock and atrial fibrillation are major complications. New and efficient treatment modalities will be possible in the future.

Although best known as an animal disease, human babesiosis is attracting increasing attention as a worldwide emerging zoonosis. Humans are commonly infected by the bite of ixodid ticks. Rare ways of transmission are transplacental, perinatal and transfusion-associated. Infection of the human host can cause a very severe host-mediated pathology including fever, and hemolysis leading to anemia, hyperbilirubinuria, hemoglobinuria and possible organ failure. In recent years, apparently owing to increased medical awareness and better diagnostic methods, the number of reported cases in humans is ris…

The majority of patients undergoing balloon retrograde transvenous obliteration (BRTO) are decompensated cirrhotic for either bleeding gastric varices (GV) or hepatic encephalopathy. These patients will require close follow-up and assessments pre- and post-BRTO including clinical, laboratory, endoscopic, and imaging evaluations. It is essential that clinicians are aware of the potential benefits and complications that may result from BRTO. These complications may include fever, chest or epigastric pain, hemoglobinuria, transient hypertension, nausea or vomiting, and many more. These complicati…

Mefenamic acid ingestion, usually in excess and over prolonged period is known to produce interstitial nephritis, or less commonly papillary necrosis, with acute renal failure. However, it is not dose-dependent for the induction of tubulointerstitial damage. Excess iodine ingestion is known to produce toxicity and possible death, but acute renal failure is rare. There is evidence from clinical and experimental data that iodine has toxic effect on tubular epithelial cells. Iodine has not been documented to produce red cell hemolysis and hemoglobinuria. We present a unique case of acute renal fa…
