Icteric hepatitis was observed in a 21-year-old female patient. Titres of the agglutination reaction for listeria monocytogenes supported the likelihood of listeria infection. Further serological studies of potentially relevant viruses and bacteria remained negative.

Granulomatous hepatitis is a generic histopathologic diagnosis seen in approximately 5 to 10% of liver biopsy specimens. In the past, tuberculosis and sarcoidosis have been most frequently incriminated, although numerous other infectious and noninfectious etiologies have been reported. We have studied 95 cases of granulomatous hepatitis representing 6% of 1500 liver biopsies performed over a period of 10 years. Of the two major types of chronic active hepatitis, that associated with autoimmunity, CAH-Type A, has been the more extensively studied and is better recognised. The subdivision of chronic hepatitis into chronic persistent hepatitis (CPH) and chronic active hepatitis (CAH) remains the basis for diagnosis, but requires modification and amplification. Mild CAH closely resembles CPH, and corticosteroids may suppress CAH to a picture of CPH. Sampling error may explain some examples of apparent transition from CPH to CAH. 5 patients treated with perhexiline maleate, 200-400 mg/day for at least 6 months, exhibited evidence of hepatitis. The picture was very similar to acute alcoholic hepatitis, clinically, biologically and histologically with presence of necrosis, Mallory's hyaline, polynuclear infiltration and to a lesser degree, steatosis. Prophylactic isoniazid therapy is currently recommended for adolescents with positive tuberculin reactions because of the risk of activation of tuberculosis in this age group. Since this drug has been associated with hepatic dysfunction in adults, the present study was undertaken to evaluate its effect on liver function in teen-agers.
