BACKGROUND: Mechanical ventilation is a double-edged sword to acute respiratory distress syndrome (ARDS) including lung injury, and systemic inflammatory response high tidal volumes are thought to increase mortality. The objective of this study is to evaluate the effects of dynamic ventilatory factors on ventilator induced lung injury in a dog model of ARDS induced by hydrochloric acid instillation under volume controlled ventilation and to investigate the relationship between the dynamic factors and ventilator-induced lung injuries (VILI) and to explore its potential mechanisms.

BACKGROUND: During synchronised mechanical ventilation, positive airway pressure and spontaneous inspiration coincide. If synchronous ventilation is provoked, adequate gas exchange should be achieved at lower peak airway pressures, potentially reducing baro/volutrauma, air leak and bronchopulmonary dysplasia. Synchronous ventilation can potentially be achieved by manipulation of rate and inspiratory time during conventional ventilation and employment of patient-triggered ventilation.
OBJECTIVES: To compare the efficacy of:(i) synchronised mechanical ventilation, delivered as high-frequency po

PURPOSE: Early lung recruitment (ELR) during high-frequency oscillatory ventilation (HFOV) in combination with prophylactic surfactant use has been reported to reduce mortality, improve respiratory outcomes, and reduce the need for repeated surfactant dosing, suggesting that surfactant might be used more selectively in very low birth weight (VLBW) infants on HFOV than generally recommended. We report our first experience from such a selective early rescue use of surfactant in VLBW infants on HFOV.
METHODS: After a deliberate ELR maneuver and "optimal" continuous distending pressure (CDP) find

The present study was designed to investigate breathing patterns across the sleep-wake state following a high cervical spinal injury in rats. The breathing patterns (e.g., respiratory frequency, tidal volume, and minute ventilation), neck electromyogram, and electroencephalography of unanesthetized adult male rats were measured at the acute (i.e., 1 day), subchronic (i.e., 2 wk), and/or chronic (i.e., 6 wk) injured stages after unilateral contusion of the second cervical spinal cord. Cervical spinal cord injury caused a long-term reduction in the tidal volume but did not influence the sleep-wa

BACKGROUND: Electromagnetic navigational bronchoscopy (ENB) is used to obtain peripheral lung tissue samples for evaluation and staging of central and peripheral lung lesions. Jet ventilation delivers and maintains a sustained airway pressure at high frequency, chest wall and diaphragmatic movement is drastically reduced compared with traditional ventilation. The current study looks to examine the effectiveness of tissue sampling (diagnostic yield) while using jet ventilation on target-lesion movement when compared with traditional ventilation.
METHODS: A total of 36 patients received total i

BACKGROUND: It has been suggested that the implementation of protective mechanical ventilation may increase respiratory acidosis when other ventilator settings, including breathing frequency, are not optimized. After cardiac surgery, this complication may be associated with hemodynamic impact, such as pulmonary hypertension. VentilO is a clinical-decision support system developed to implement protective ventilation. The primary objective of this study was to evaluate the incidence of acid/base disorders after cardiac surgery and to compare the ventilatory parameters set by the clinicians with

Polysomnograms were obtained from two groups of normal subjects (20 medical students and 20 elderly persons). The recordings included the usual sleep parameters (electroencephalograph, electrooculograph, and electromyograph of chin muscles) and respiratory data obtained by means of a pneumotachygraph (tidal volumes and respiratory frequency) and thoracic and abdominal strain gauges. In the older group, sleep was more disturbed and respiratory events (hypopneas and apneas) were more frequent both in non-rapid eye movement (NREM) and in REM sleep. The apneas were predominantly of the obstructive

Meconium aspiration syndrome (MAS) is a common cause of respiratory distress in neonates. In many affected children, the complex nature of meconium aspiration syndrome contributes to the apparent lack of response to standard therapies. Over the past decade, several new therapies have been suggested to be more effective than "conventional" management in treating meconium aspiration syndrome. These include: anti-inflammatory drugs, medications to counter the effect of prostaglandin-related compounds, high-frequency ventilation, exogenous surfactant, inhaled nitric oxide and liquid ventilation. T…

Animal experiments and clinical trials have shown that the neonatal respiratory distress syndrome (RDS) can be treated effectively by surfactant replacement via the airways. This treatment facilitates the resorption of fetal pulmonary fluid, promotes uniform air expansion of the lungs, enhances gas exchange, reduces the protein leak across the alveolar epithelium, and prevents the development of bronchiolar epithelial lesions during artificial ventilation. Data from recent animal experiments indicate that surfactant replacement prevents epithelial lung lesions also during high frequency ventil

The Adult Respiratory Distress Syndrome remains one of the most lethal complications in both surgical and medical intensive care units. Mortalities of 50 to 80 per cent are still reported in recent reviews. Many risk factors have been associated with an increased incidence of ARDS, but sepsis and direct pulmonary injury from aspiration, pulmonary contusion and near drowning are most commonly identified. Studies of physiopathologic factors of ARDS implicate granulocyte aggregation with the formation of oxygen free radicals and other cellular and chemical mediators. Pharmacologic agents and high

Extracorporeal membrane oxygenation (ECMO) has gained notoriety recently as a treatment of last resort for neonates at term or near term who suffer from pulmonary failure. Because of serious inherent risks, the procedure is at present reserved for infants with reversible pulmonary disease who have failed conventional or high-frequency ventilation and who meet the prognostic criterion of 80 percent mortality.

A case of right diaphragmatic hernia is presented associated with a complex congenital heart disease (double-outlet right ventricle, transposition of the great arteries and left isomerism) diagnosed prenatally. Despite high-frequency oscillatory ventilation plus nitric oxide and uneventful repair of the hernia, the infant died after 6 days of uncontrolled pulmonary hypertension and severe aortic coarctation that developed postnatally.

Overdistension of the lungs from high inspiratory pressure is increasingly recognized as a major contributor to lung injury and worsening respiratory failure in the child who requires prolonged mechanical ventilation. Many modes of ventilation (such as high-frequency ventilation) have been introduced in an attempt to decrease this lung injury. Recently, a new mode of tracheal ventilation, intratracheal pulmonary ventilation (ITPV), has been described. By using a catheter positioned at the carina with continuous gas flow, it is possible to achieve effective ventilation at very low pressures. Th…

INTRODUCTION: Non-invasive mechanical ventilation provides early improvement in most of the patients with acute hypercapnic respiratory failure. The aim of our study was to determine the risk factors for late failure of non-invasive mechanical ventilation in patients with acute hypercapnic respiratory failure.
MATERIALS AND METHODS: Ninety three patients were prospectively evaluated. Non-invasive mechanical ventilation was accepted to be successful if the patient was discharged from the hospital without the need for intubation (group 1) and to be late failure if a deterioration occurred after

Mechanical ventilation is considered a supportive, nontherapeutic technology used to perform the work of breathing for patients who are unable to do so on their own. In neonatology, mechanical ventilation is often used for premature neonates who are unable to sustain ventilation because of reduced functional residual capacity due to surfactant deficiency. Mechanical ventilation is thus an attempt to mimic the respiratory system's physiological function of gas exchange until the respiratory system reaches maturation. In pediatrics, mechanical ventilation is rarely used for acute respiratory dis

BACKGROUND: Cardiogenic oscillations in airflow can cause ventilator autotriggering during pressure support ventilation, potentially leading to inappropriate hyperventilation. A method to attenuate these oscillations in real time may help reduce autotriggering.
MATERIALS AND METHODS: High-resolution airflow and ECG signals were collected from intubated surgical patients receiving pressure support ventilation. Singular spectrum analysis (SSA) was applied in a sliding-window format to generate a smoothed respiratory waveform. We quantified attenuation of cardiogenic oscillations using ECG-align

Neonatal mechanical ventilation is a cornerstone in the management of respiratory failure in newborns (NBs), particularly in preterm infants, in whom pulmonary and neurologic increases their vulnerability to ventilation-induced lung injury. At present, the primary objective of respiratory support is to ensure adequate ventilation while minimizing lung and brain injury through lung-protective strategies and careful oxygen titration. Noninvasive ventilation (NIV) is currently the first-line approach for both initial respiratory support and post-extubation care, as it reduces the need for invasiv
