Assessing the costs and benefits of perioperative iron deficiency anemia management with ferric carboxymaltose in Germany.

BACKGROUND: Perioperative administration of ferric carboxymaltose (FCM) was previously shown to reduce both the need for transfusions and the hospital length of stay in patients with preoperative iron deficiency anemia (IDA). In this study, we estimated the economic consequences of perioperative administration using FCM vs usual care in patients with IDA from the perspective of a German hospital using decision-analytic modeling. MATERIALS AND METHODS: The model was populated with clinical inputs (transfusion rates, blood units transfused, hospital length of stay) from a previously reported ra

PREVALENCE AND PREDICTORS OF PREDIABETES AMONG ADMINISTRATIVE STAFF OF A TERTIARY HEALTH CENTRE, SOUTHWESTERN NIGERIA.

INTRODUCTION: Prediabetes is a condition in which individuals have blood glucose levels higher than normal but not high enough to be classified as diabetes mellitus. Inadequate moderate-intensity physical activity, amidst other risk factors, is a well-documented risk factor for prediabetes. Hospital administrative staff can be particularly vulnerable to prediabetes because of the greater number of hours spent in a sitting or sedentary position during administrative duties. The aim of this study was to find the prevalence of prediabetes and to determine its predictors among administrative staff

Probiotic Administration in Infants With Gastroschisis: A Pilot Randomized Placebo-Controlled Trial.

OBJECTIVES: Infants with gastroschisis often require long periods of gastric suctioning and hospitalization. The impact of these interventions on the intestinal microbiota and attempts to alter the microbial community have not been studied. We sought to determine how the intestinal microbiota is influenced by the current treatment of gastroschisis and whether alteration of the intestinal microbiota with a probiotic microbe will influence length of hospitalization. METHODS: We performed a randomized, placebo-controlled pilot study of administration of probiotic Bifidobacterium longum subsp. in

Historical roots of hospital-centred operational governance in China: a path dependence analysis and its implications for global health system reform.

BACKGROUND: Hospital-centred operational governance has been a persistent feature of China's health system. While this model has supported administrative control, rapid service expansion and organisational standardisation, it has also contributed to the continued concentration of resources in hospitals, constrained local flexibility and the relative marginalisation of primary healthcare. This study examines, from the perspective of path dependence, how this governance model gradually emerged and persisted from 1835 to the present. METHODS: Drawing on historical studies, policy documents, inst

Use of antipsychotic and sedative medications in older patients in the emergency department.

BACKGROUND: Antipsychotics and sedatives are used to treat agitation in the emergency department (ED) but carry significant risk in older adults. Our objective was to determine factors associated with their administration to older ED patients. METHODS: This was an observational study using data from the 2014-2017 National Hospital Ambulatory Medical Care Survey. We identified ED visits for patients aged ≥65 years and determined whether an antipsychotic or sedative was administered. Visits related to substance use/withdrawal, other psychiatric complaints, and intubation were excluded. We perfo

Exploring the knowledge level and practices of hospital pharmacists in management of oral anticoagulants in Gulf Cooperation Council countries: a scoping review of literature.

INTRODUCTION: Oral anticoagulants (OACs) are essential for managing thromboembolic events and cardiovascular conditions. However, they carry a significant bleeding risk. Pharmacists play a critical role in ensuring the safe and effective use of these medications. Within Gulf Cooperation Council (GCC) countries, hospital pharmacists are involved in managing OACs. However, region specific data on their knowledge and practices remain limited. AIM: This review analysed existing literature regarding hospital pharmacists' knowledge and practices in OACs dispensing and counselling within the GCC cou

Treatment-Related Complications in Children Hospitalized With Disseminated Lyme Disease.

We describe here treatment approaches and treatment-related complications in 138 hospitalized children with disseminated Lyme disease. The patients who received parenteral antibiotics had a higher rate of complications than those who received oral therapy (15.4 vs 4.2 per 1000 days of therapy, respectively; P < .05). Oral therapy should be used preferentially if either route is supported by current guidelines.

Role of topical tranexamic acid in the management of idiopathic anterior epistaxis in adult patients in the emergency department.

PURPOSE: The role of topical tranexamic acid in the management of anterior epistaxis in adult patients in the emergency department (ED) is examined. SUMMARY: The use of alternative agents for the treatment of epistaxis before the use of nasal packing may be reasonable due to patient discomfort, potential complications, and the need for follow-up with a healthcare provider for packing removal. One such agent is tranexamic acid. Two published studies evaluated the off-label use of topical tranexamic acid for the treatment of epistaxis. The first trial compared the efficacy of a topical gel cont

Separating the unequal. New ACHE study shows racial, gender salary disparities still plague the C-suite, a problem the group is working to address.

According to an American College of Healthcare Executives report, the salary gap between white male health executives' salaries and those of minorities is widening. "You're not very likely to run into someone who will be overtly discriminatory," said Daniel Jackson, left. "It will be more veiled. ... But it also dissipates quickly if you're effective.
