Host plasma protein biomarkers for tuberculosis disease screening in febrile adults in Tanzania.

BACKGROUND: Host circulating biomarkers may enhance access to tuberculosis (TB) diagnostics in low-resource settings. We sought to identify host plasma proteins that differentiate TB disease from other infectious causes of fever. METHODS: This secondary analysis of a prospective cohort included outpatients ≥18 years in urban Tanzania presenting with ≤7 days of fever. Fourteen plasma proteins reflecting endothelial and immunoregulatory pathways were evaluated against a composite reference standard including sputum Xpert MTB/RIF, urine lipoarabinomannan, and/or chest x-ray. Multivariable models

Performance of 2 Finger-Stick Blood Tests to Triage Adults With Symptoms of Pulmonary Tuberculosis: A Prospective Multisite Diagnostic Accuracy Study.

BACKGROUND: Non-sputum-based, point-of-care triage tests for pulmonary tuberculosis could enhance tuberculosis diagnostic programs. We assessed the diagnostic accuracy of 2 finger-stick blood tests: the Cepheid 3 gene host-response cartridge (Xpert-HR), which measures 3 host messenger RNA transcripts, and the 3-host protein multibiomarker test (MBT). METHODS: We performed a prospective diagnostic accuracy study of consecutive participants with symptoms compatible with pulmonary tuberculosis in The Gambia, South Africa, Uganda, and Vietnam. A composite reference standard for active pulmonary t

Performance of DNA methylation analysis of ASCL1, LHX8, ST6GALNAC5, GHSR, ZIC1 and SST for the triage of HPV-positive women: Results from a Dutch primary HPV-based screening cohort.

Methylation of host-cell deoxyribonucleic acid (DNA) has been proposed as a promising biomarker for triage of high-risk (hr) human papillomavirus (HPV) positive women at screening. Our study aims to validate recently identified host-cell DNA methylation markers for triage in an hrHPV-positive cohort derived from primary HPV-based cervical screening in The Netherlands. Methylation markers ASCL1, LHX8, ST6GALNAC5, GHSR, ZIC1 and SST were evaluated relative to the ACTB reference gene by multiplex quantitative methylation-specific PCR (qMSP) in clinician-collected cervical samples (n = 715) from h

Pathogen and host biomarkers to aid early diagnosis and prognosis of tuberculous meningitis.

BACKGROUND: Tuberculous meningitis (TBM) is the most sinister form of extrapulmonary tuberculosis (EPTB), associated with high mortality due to delayed diagnosis and limited sensitivity of conventional and molecular tests. Current study evaluated the diagnostic utility of Lipoarabinomannan antigen (LAM) detection in CSF and urine and explored host inflammatory biomarkers for diagnosis and prognosis of TBM. METHODS: This prospective observational cohort study enrolled 80 HIV-negative adults with presumptive TBM at a tertiary care center. Patients were classified according to the uniform TBM ca

Second sialic acid-binding site of influenza A virus neuraminidase: binding receptors for efficient release.

Influenza A viruses (IAVs) are a major cause of human respiratory tract infections and cause significant disease and mortality. Human IAVs originate from animal viruses that breached the host species barrier. IAV particles contain sialoglycan receptor-binding hemagglutinin (HA) and receptor-destroying neuraminidase (NA) in their envelope. When IAV crosses the species barrier, the functional balance between HA and NA needs to be adjusted to the sialoglycan repertoire of the novel host species. Relatively little is known about the role of NA in host adaptation in contrast to the extensively stud

IgM to Bartonella henselae in cat-scratch disease and during acute Epstein-Barr virus infection.

The diagnostic value of IgM to Bartonella henselae was evaluated in 20 children with cat-scratch disease (CSD) and controls consisting of 20 blood donors and 20 children with enlarged lymph nodes without CSD by two indirect immunofluorescence assays (IFA). One was based on B. henselae cocultivated with Vero cells (host cell-associated IFA), and the other on B. henselae grown on agar (host cell-free IFA). With the host cell-associated IFA, 18 of 20 children with CSD revealed IgM, whereas only 14 did so with the host cell-free IFA. Sera of two blood donors as well as sera from three children wit

Attraction of Culex pipiens to uropygial gland secretions does not explain feeding preference for American robins.

Culex pipiens, the endemic mosquito vector of West Nile virus in eastern North America, is responsible for maintenance of the virus in avian reservoir hosts, the most important of which appears to be the American robin. One reason for the greater involvement of robins is believed to be the feeding preference of Cx. pipiens, however, the basis of this preference is not understood. We tested the hypothesis that the species-specific chemical profile of avian uropygial gland secretions are used by Cx. pipiens as cues to locate birds and, therefore, may contribute to the observed feeding preference

Dysfunctional cytokine production by host-reactive T-cell clones isolated from a chimeric severe combined immunodeficiency patient transplanted with haploidentical bone marrow.

We have investigated the mechanism of tolerance in a patient with severe combined immunodeficiency (SCID) transplanted with HLA-haploidentical, T cell-depleted bone marrow cells obtained from the mother. At 4 years after transplantation, T cells, natural killer (NK) cells, and a small percentage (2%) of B cells were found to be of donor origin, whereas monocytes and the majority of B cells remained of host origin. In primary mixed lymphocyte cultures (MLC), the engrafted T cells of the donor did not proliferate in response to the host cells, whereas untransplanted donor T cells showed good pro

Development and optimization of cocktail-ELISA for a unified surveillance of zoonotic schistosomiasis in multiple host species.

The zoonotic characteristic of the human parasite Schistosoma japonicum infecting a significant number of wild and domestic animals highlights the need to develop a unified surveillance in multiple host species for a strengthened schistosomiasis control. It has been shown in several studies that water buffaloes and dogs are considered important reservoirs in the transmission of the schistosome parasite to humans. Recombinant antigens like thioredoxin peroxidase-1 (SjTPx-1) and tandem repeat proteins (Sj1TR, Sj7TR) have been shown to be good diagnostic antigens individually in humans, water buf

Failure to infect laboratory rodent hosts with human isolates of Rodentolepis (= Hymenolepis) nana.

Confusion exists over the species status and host-specificity of the tapeworm Rodentolepis (= Hymenolepis) nana. It has been described as one species, R. nana, found in both humans and rodents. Others have identified a subspecies; R. nana var. fraterna, describing it as morphologically identical to the human form but only found in rodents. The species present in Australian communities has never been identified with certainty. Fifty one human isolates of Rodentolepis (= Hymenolepis) nana were orally inoculated into Swiss Q, BALB/c, A/J, CBA/ CAH and nude (hypothymic) BALB/c mice, Fischer 344 an

Performance of Extracellular Vesicles From Leishmania (Leishmania) infantum for Serological Diagnosis of Human and Canine Visceral Leishmaniasis.

Visceral leishmaniasis (VL) is a zoonotic disease in which dogs are the main reservoirs. Until now, the serological tests do not present satisfactory sensitivity for diagnosis of these hosts. One of the functions of extracellular vesicles (EVs) is related to immunological host response. Here, we evaluated the ability of EVs released by Leishmania (Leishmania) infantum promastigotes (Leish-EVs) to be source of antigens for use in serological diagnosis for human visceral leishmaniasis (HumVL) and canine visceral leishmaniasis (CanVL). A total of 300 sera were tested. The 155 human sera were divi

Schistosoma species detection by environmental DNA assays in African freshwaters.

BACKGROUND: Schistosomiasis is a neglected tropical parasitic disease associated with severe pathology, mortality and economic loss worldwide. Programs for disease control may benefit from specific and sensitive diagnostic methods to detect Schistosoma trematodes in aquatic environments. Here we report the development of novel environmental DNA (eDNA) qPCR assays for the presence of the human-infecting species Schistosoma mansoni, S. haematobium and S. japonicum. METHODOLOGY/PRINCIPAL FINDINGS: We first tested the specificity of the assays across the three species using genomic DNA preparatio

Principles of anti-infective therapy.

Antimicrobial drugs, along with competent surgical procedures, provide an effective means for restoration of oral health. In dentistry, antibiotics are indicated for either treatment of acute infections or for prophylactic coverage of patients at risk for developing bacterial endocarditis or other infections as the result of bacteremia caused by dental procedures. Whenever possible, a bactericidal agent is preferred because of greater effectiveness and less reliance on host defense mechanisms. For maximal effectiveness of antibiotic therapy of orodental infections, several factors must be eval

Phase-variable Type I methyltransferase M.NgoAV from Neisseria gonorrhoeae FA1090 regulates phasevarion expression and gonococcal phenotype.

The restriction-modification (RM) systems are compared to a primitive, innate, prokaryotic immune system, controlling the invasion by foreign DNA, composed of methyltransferase (MTase) and restriction endonuclease. The biological significance of RM systems extends beyond their defensive function, but the data on the regulatory role of Type I MTases are limited. We have previously characterized molecularly a non-canonical Type I RM system, NgoAV, with phase-variable specificity, encoded by Neisseria gonorrhoeae FA1090. In the current work, we have investigated the impact of methyltransferase Ng

A capsule-dependent lytic phage for targeting multidrug-resistant and hypervirulent Klebsiella pneumoniae.

As the threat of multidrug-resistant Klebsiella pneumoniae strains rises, the potential of phages as promising alternatives to antibiotics is increasingly being demonstrated. In this study, we isolated and characterized phiK2044, a highly specific and efficient lytic phage targeting the model strain K. pneumoniae NTUH-K2044. Demonstrating wide host compatibility, potent lytic activity, and robust environmental adaptability, phiK2044 exhibited exceptional efficacy against hypervirulent subtypes, including hypervirulent K. pneumoniae (45.2%), sequence type 23 (87.5%), and capsular K1 (92.3%). Mate choice, frequency dependence, and the maintenance of resistance to parasitism in a simultaneous hermaphrodite.

Biomphalaria glabrata are simultaneous hermaphroditic freshwater snails that act as intermediate hosts for the macroparasitic trematode Schistosoma mansoni, a causative agent of schistosomiasis. Heritability and strain-specificity of both snail resistance and susceptibility to schistosome infection have been demonstrated, genetic variability for which is maintained, in part, through trade-offs between high fitness costs associated with infection and those associated with resistance. However, despite such a high cost of resistance and a low prevalence of infection in natural snail populations,

Host-parasite interactions in Acanthocephala: a morphological approach.

In this review recent morphological and histochemical descriptions have been compiled of (mainly outer) features of all developmental stages … estinal wall without showing pronounced symptoms of disease. The review also treats the subjects of host specificity, nutrient uptake and metabolism of the worms as well as their absorbance of metals and other elements. The high absorbance capacity for heavy metals suggests the usefulness of acanthocephalans as bioindicators.
