Evaluation of the osteoporotic proximal humeral fracture and strategies for structural augmentation during surgical treatment.

Fractures of the proximal humerus are relatively common injuries in the elderly population. Given the association between proximal humeral fractures and osteoporosis, elderly patients who sustain these injuries should always undergo a fragility fracture workup. Furthermore, a preoperative assessment of local bone quality can be critical in facilitating decision making regarding surgical and nonsurgical treatment. Modalities for quantifying osteoporosis in the proximal humerus include plain radiography and spiral computed tomography imaging. Optimal management of osteoporotic proximal humeral f…

Trends and variation in incidence, surgical treatment, and repeat surgery of proximal humeral fractures in the elderly.

BACKGROUND: the treatment of proximal humeral fractures in the elderly remains controversial. Options include nonoperative treatment, open reduction with internal fixation (ORIF), and hemiarthroplasty. Locking plate technology has expanded the indications for ORIF for certain fracture types in osteoporotic bone. This study was performed to characterize the incidence, treatment, and revision surgery of proximal humeral fractures according to geographic region both before (1999 to 2000) and after (2004 to 2005) the introduction of locking plates. METHODS: we used a 20% sample of Medicare Part-B…

Injury of the axillary artery after a proximal humeral fracture: a case report and overview of the literature.

A traumatic lesion of the axillary artery after a proximal humeral fracture is very uncommon. The clinical presentation can be very misleading and therefore it is mandatory to have a high index of suspicion and, as presented in this case report, to check the pulsations in different positions. Open repair remains the golden standard for most surgeons, but, in selected cases, an endovascular approach is feasible. The functional outcome is mostly determined by the associated trauma, especially the injury to the nervous structures.

A comparison of the Constant and Oxford shoulder scores in patients with conservatively treated proximal humeral fractures.

The Oxford shoulder scores (OSS) and Constant scores were collected prospectively for 103 patients treated conservatively for proximal humeral fractures. The paired scores were analyzed by assessing the degree of correlation, the agreement between the scores, and their sensitivity to change. Linear regression analysis was also performed. The scores correlated well with a coefficient of .84 (P < .001), demonstrating a clear relationship between the scoring systems. There was good agreement between the scores and both were sensitive to change over time (P < .001). We have also produced a regress…

Fracture-separation of the distal humeral epiphysis. Long-term follow-up of five cases.

Fracture-separation of the distal humeral epiphysis is a rare injury described in young children. The diagnosis is often difficult because most of the ossification centre are not visible. We report a long-term follow-up study of five cases, evaluated after an average of 38 years. At the time of fracture, all the patients were more than 3 years old. Three patients had been treated with overhead traction, closed reduction and immobilisation in a long-arm plaster cast, one patient with closed reduction and immobilisation and another one, initially not diagnosed, had been treated after 2 weeks wit

Treatment of nonunion of humeral shaft fracture with dynamic compression plate and cancellous bone graft.

BACKGROUND: This study was conducted to evaluate the treatment of aseptic nonunion of the humeral shaft with a dynamic compression plate (DCP) and cancellous bone graft. METHODS: One hundred and five cases of nonunion of a humeral shaft fracture between 1982 and 2001 were analyzed retrospectively. The study population comprised 66 males and 39 females with an average age of 46.2 years (range, 17-81 years). Sixty-seven fractures were defined as atrophic nonunion, and 20 as hypertrophic nonunion, whereas 18 could not be defined clearly. All the fractures were managed by open reduction and inter

The use of the 3-mm K-Wire to supplement reduction of humeral supracondylar fractures in children.

BACKGROUND: Most children with humeral supracondylar fractures can be treated with simple closed reduction and cross-fixation with Kirschner (K)-wires. However, in a small proportion of cases, an acceptable closed reduction cannot be obtained, and open reduction becomes necessary. An alternative to open reduction is the use of a temporary 3-mm K-wire to manipulate and reduce the distal fragment. This report introduces the method of manipulation, as well as indications, and draws comparisons with complete close reduction cases. METHODS: Of 118 patients, 76 (64.4%) were managed with the standar

Axillary artery injury from humeral neck fracture: a rare but disabling traumatic event.

Axillary artery injury from blunt trauma to the shoulder is uncommon. Fracture of the neck of the humerus is a rare cause of injury to the axillary artery. Four cases of axillary artery thrombosis from humeral neck fracture are reported. Each of the first 2 patients presented with a pulseless and acutely ischemic limb after a trivial fall. A repair of the axillary artery with saphenous vein interposition graft was performed in the first patient. The extremity was salvaged, but a residual radial and ulnar neurologic deficit persisted. The second patient presented with a pulseless insensate uppe

Fracture separation of the distal humeral epiphysis in children younger than three years old.

This study evaluated the change of carrying angle and the causes of cubitus varus after the fracture separation of the distal humeral epiphysis in young children. Twelve cases of fracture separation of distal humeral epiphysis were treated from January 1995 to July 1997. The age of all patients was younger than 3 years old. A metaphyseal fragment was seen in all cases (Salter-Harris type II), but the size of the metaphyseal fragment was either a very small flake or a large Thurston-Holland fragment. Posteromedial displacement was seen in all cases. There were three treatment groups: closed red

Temporal patterns of redisplacement risk following nonoperative treatment of pediatric supracondylar humeral fractures: A retrospective cohort study.

BACKGROUND: Although risk factors for redisplacement after nonoperative treatment of pediatric supracondylar humeral fractures (SCHFs) are well described, the temporal distribution of this risk remains poorly understood. This study investigated temporal patterns of redisplacement and identified independent predictors using time-to-event analysis. METHODS: This retrospective cohort study included 218 children (aged 3-14 years) with Gartland type II (n = 142) and selectively treated type III (n = 76) SCHFs managed with closed reduction and custom-molded triplanar splinting between September 202…

Safety pin tie-in external skeletal fixation in pediatric feline and canine humeral/femoral fractures: 3 cases.

Three animals (1 dog and 2 cats), all weighing < 3 kg and aged 4 to 7 mo, were treated for femoral or humeral fractures using the "safety pin tie-in" (SPTI), a proximal transverse-pin-free Type Ia intramedullary pin tie-in external skeletal fixator. Clinical assessment, radiographic follow-up, and owner-reported outcomes were evaluated. Only minor complications after surgery were observed, and radiographic interpretation of healing was achieved in all cases. For all animals, weight-bearing function was regained soon after surgery, limb function was classified as full (Cases 1 and 3) or accepta…

Does Immobilization Method Make a Difference in Conservative Management of Proximal Humeral Fractures? A Prospective Randomized Trial.

BACKGROUND/OBJECTIVE: The role of conservative treatment in proximal humerus fractures (PHFs) has been increasing with emerging evidence. However, the optimal conservative treatment protocol remains unclear. In particular, the immobilization method-an essential component of conservative management-has not been adequately investigated in the literature. This study aimed to compare the clinical and radiological outcomes of three different immobilization devices that position the shoulder in varying degrees of immobilization. METHODS: This prospective randomized clinical trial included patients

Does motion at 8 weeks predict nonunion in nonoperatively managed humeral shaft fractures: A prospective multicenter evaluation.

UNLABELLED: Management with a functional brace results in successful union in the majority of patients with a humeral shaft fracture. An important factor in a patient's decision for operative vs. nonoperative management is how long it will take to know if the fracture will unite if treated nonoperatively. In this prospective multicenter evaluation subset analysis, we sought to address the following: (1) What is the positive and negative predictive value of gross motion at the fracture site on physical exam at 8 weeks post-injury for union in patients with humeral shaft fractures treated nonope
