INTRODUCTION: Primary hyperparathyroidism is a common endocrine disorder with different epidemiological patterns among countries. In Denmark, the incidence of primary hyperparathyroidism was last described in 1999 when it was reported to follow an increasing trend. In the present study, we evaluated trends in the incidence of primary hyperparathyroidism in Denmark up to 2010.
MATERIAL AND METHODS: All patients diagnosed with primary hyperparathyroidism from 1977 to 2010 were included. Annual incidence rates were calculated.
RESULTS: The overall incidence was clearly increased at the end of 2…

The experience of treatment of 41 patients (aged 32-67 years) was presented in the article. The duration of disease was 2-5 years. Primary hyperparathyroidism was diagnosed in 16 patients and secondary--in 25. Diagnostics of the disease included clinical methods of treatment; studying levels of general and ionized calcium, phosphorus, parathormone; an ultrasound of thyroid and parathyroid glands, the substratum scintigraphy. All patients were undergone the operation. Adenomas of parathyroid glands were removed in the case of primary hyperparathyroidism including mini-access. Hyperplastic parat…

This descriptive study was done to evaluate the clinical, biochemical and radiological findings of primary hyperthyroidism (pHPT) patients with parathyroid adenoma. This study was done in the department of Otolaryngology and Head-Neck surgery, BIRDEM, Dhaka from July 2000 to June 2007. We retrospectively reviewed the clinical presentation, biochemical and radiological features from the case records from the last 7 years of 32 patients at a tertiary care centre in Dhaka who had documented pHPT due to adenoma. Of them 18 were male and 14 were female. Mean age±SD of the patients was 42.16±11.64 y

OBJECTIVE: Primary hyperparathyroidism (PHPT) is associated with cardiovascular morbidity and premature death, but the underlying mechanisms are incompletely understood. The aim of this study was to investigate whether adrenergic dysfunction may be a contributing factor.
PATIENTS AND METHODS: Forty-nine patients with mild PHPT (serum calcium 2·7 ± 0·1 mM) and 48 control subjects, matched for age and sex, were examined; patients within 1 month before parathyroidectomy (PTX) and 6 months postoperatively; control subjects at inclusion. Heart rate variability (HRV) was analysed in 24-h electrocar…

BACKGROUND: The most common location for supernumerary or ectopic parathyroid glands is the thymus.
METHODS: A review of patients who underwent parathyroidectomy for hyperparathyroidism from 1990 to 2010 was completed to determine indications for thymectomy, the yield of parathyroid tissue, and outcome of therapy.
RESULTS: Seventy of 379 patients with hyperparathyroidism underwent parathyroidectomy and transcervical thymectomy. Intrathymic parathyroid tissue was present in 23 (33%) patients, including supernumerary glands in 8 patients (11%). Indications for thymectomy were renal hyperparath…

PURPOSE: Primary hyperparathyroidism has been studied more extensively in adults than in adolescents. The objective of this study is to define the similarities and differences that exist between these groups.
METHODS: A retrospective review of 1,000 primary hyperparathyroidism patients undergoing parathyroidectomy at a single tertiary-care university teaching hospital between 1990 and 2004. All patients 20 years of age or younger comprised our study cohort, and were compared to two historical adult groups.
RESULTS: Of 1,000 parathyroidectomies, 21 (2.1 %) were 20 years of age or younger (ado…

Hypercalcaemia secondary to primary hyperparathyroidism may present with an insidious clinical picture and involve multiple organs, which may lead to a delay in its diagnosis and erroneous therapeutic actions, due to its similarity to other different conditions. We describe the case of a patient who was admitted to our hospital with the diagnosis of acute abdomen. A laparotomy was performed which was negative. The slow post-operative progress and the clinical neurology of the patient was resolved after checking for the presence of a significant hypercalcaemia secondary to a parathyroid lymph n…

Primary hyperparathyroidism (PHPT) is a disease to cause calcium, phosphorus, bone metabolism abnormality by the over secretion of parathyroid hormone (PTH) . A rise of PTH brings hypercalcemia, hypophosphatemia, and clinical symptoms include a recurrent urolithiasis, a letter of psychoneurosis, a gastrointestinal ulcer and bone resorption. It is the object disease of the clinical study that is important as one of the causes of urolithiasis in the urology department domain now because most of symptom type PHPT is renal stone types although it is a border domain with the internal secretion surg…

Primary hyperparathyroidism (PHPT) is a biochemical syndrome caused by the inappropriate or unregulated overproduction of parathyroid hormone, Leading to hypercalcae-mia. It was previously considered a relatively rare disorder, with clinical manifestations dominated by renal and/or bone disease. However, in modern times the diagnosis is most frequently recognized coincidentally on biochemical testing in patients evaluated for unrelated complaints. Parathyroidectomy is the only curative treatment for PHPT, with improved outcomes in symptomatic patients following this procedure. However, surgica…

For patients with chronic renal failure who develop secondary hyperparathyroidism (SHPT), imaging techniques can be useful, especially to evaluate the location, size and functional status of parathyroid glands. This review analyzes all available imaging procedures in the context of SHPT. We evaluate: 1) Cervical ultrasound (B-mode, Doppler, colour-Doppler and power-Doppler), 2) Scintigraphic studies (Tallium, 99mTc-MIBI and 99mTc-tetrofosmin), including non-standard image acquisition techniques (Pinhole, SPECT), 3) Positron emission tomography (PET), 4) Computed tomography (CT) and magnetic re…

PURPOSE: This paper is a review of the evidence base to produce recommendations for the use of intraoperative parathyroid hormone (PTH), radioguided parathyroidectomy (RGP), methylene blue (MB), frozen section, and intraoperative neuromonitoring during surgery for primary hyperparathyroidism (PHPT).
MATERIALS AND METHODS: A Medline keyword search of English-language articles led to the production of a draft document, subsequently revised by committee, containing levels of evidence and the grading of recommendations as proposed by the Agency for Healthcare Research and Quality.
RESULTS: Liter…

Secondary hyperparathyroidism is an important complication in chronic kidney disease (CKD). It is associated with mineral and bone disorders and with increased cardiovascular morbidity and mortality. Despite standard treatment, adequate control of all biochemical targets for CKD-mineral and bone disorder (CKD-MBD) remains suboptimal and is accomplished only in a minority of patients receiving hemodialysis. New insights into the pathogenesis of secondary hyperparathyroidism and the development of new therapeutic agents are offering new possibilities to improve the treatment of this condition in…

Surgery is the recommended treatment for primary hyperparathyroidism. For exceptional medical treatment, the main drugs available are cinacalcet to normalize plasma calcium, and bisphosphonates to prevent bone loss. The patients eligible for medical therapy are those who cannot be operated or whose condition persists despite optimal surgical treatment. However, PHPT can cause other complications that need special attention regarding prevention and treatment during follow-up.
