Impact of Dietary Protein on the Management of Pediatric Short Bowel Syndrome.

Essential amino acids (AAs) play a key role in stimulating intestinal adaptation after massive small gut resection. The nutritional effect of dietary amino acids during intestinal regrowth has received considerable attention in recent years. This review explores the significance of dietary amino acids in the nutritional management of infants and children with intestinal failure and short bowel syndrome (SBS) as reported in the medical literature over the last three decades. A literature search was conducted using electronic databases. Breast milk emerged as the first-line enteral regimen recom

Microbial Interrelationships across Sites of Breastfeeding Mothers and Infants at 6 Weeks Postpartum.

Infancy is a critical life stage for the establishment of the gut microbiome. Human milk contains a unique microbial ecosystem that serves as a continuous source of commensal bacteria for the infant. However, the origin of the human milk microbiota, how it is influenced by breastfeeding exclusivity, and its role in infant gut microbiota assembly are not clear. To interrogate these questions, we examined the relationships among fecal, oral, breast skin, and human milk microbiota of 33 exclusively breastfeeding (EBF) and mixed-feeding (MF; human milk + infant formula) mother-infant pairs at 6 we…

Preweaning food intake influences the adiposity of young adult baboons.

The hypothesis that preweaning nutrition influences adult fat cell number and adiposity was tested in baboons. Newborn baboons were fed Similac formulas with caloric densities of 40.5 kcal (underfed), 67.5 kcal (fed normally), and 94.5 kcal (overfed) per 100 g formula. From weaning (16 wk) until necropsy at 5 yr of age all baboons were fed the same diet. At necropsy, fat cell number and fat cell size in 10 fat depots were measured. Female baboons overfed as infants had markedly greater fat depot mass, primarily because of fat cell hypertrophy, than normally fed or underfed females. Overfed mal

Predicting birth weight in fetuses with gastroschisis.

OBJECTIVE: To determine the accuracy of commonly utilized ultrasound formulas for estimating birth weight (BW) in fetuses with gastroschisis. STUDY DESIGN: A retrospective review was conducted of all inborn pregnancies with gastroschisis within the five institutions of the University of California Fetal Consortium (UCfC) between 2007 and 2012. Infants delivered at ⩾28 weeks who had an ultrasound within 21 days before delivery were included. Prediction of BW was evaluated for each of the five ultrasound formulas: Hadlock 1 (abdominal circumference (AC), biparietal diameter (BPD), femur length

Effects of oral iodine supplementation in very low birth weight preterm infants for the prevention of thyroid function alterations during the neonatal period: results of a randomised assessor-blinded pilot trial and neurodevelopmental outcomes at 24 months.

The trace element iodine (I) is essential for the synthesis of thyroid hormones. Parenteral nutrition solutions, formula milk, and human breast milk contain insufficient iodine to meet recommended intake for preterm infants. Iodine deficiency may affect thyroid function and may be associated with morbidity or neurological outcomes. The primary objective is to assess the evidence that dietary supplementation with iodine affects thyroid function during the neonatal period. The design was a randomised controlled pilot trial. Infants who met the inclusion criteria were enrolled through consecutive

[Rickets associated to the use of elemental formula: A case report].

The rickets is a disease that affects the differentiation and mineralization of the growth cartilage, as an ultimate consequence of a balance loss in calcium and phosphate levels. Vitamin D deficiency is the most common cause of the rickets (nutritional rickets). Its clinical manifestation during the first years of life involves long bones epiphysis in a more severe way. We report an 8-month-old infant who was diagnosed with cow´s milk protein allergy and suffered from multiple fractures while receiving elemental formula as part of his treatment. The final etiology was hypophosphatemic rickets

Effect of 2'-fucosyllactose and hydrolysis degree on oxidation, Maillard reaction, and physical stability of whey protein hydrolysate emulsions.

Processing of liquid infant formulas inevitably triggers undesirable side reactions affecting product quality and nutritional value. This study investigated the effects of whey protein hydrolysis (WPH) at different degrees of hydrolysis (DH; 0%, 6%, 8%, and 10%) and 2'-fucosyllactose (2'-FL) on the physical stability, protein oxidation, lipid oxidation, and Maillard reaction in infant formula model emulsions. Increasing DH progressively enhanced DPPH and ABTS radical scavenging activities, resulting in lower peroxide values and thiobarbituric acid reactive substances, thereby improving lipid o…

Influence of fatty acid distribution on lipid metabolism and cognitive development in first-weaned mice.

There are significant structural differences between breast milk fat and the fat found in existing infant formulas, and these differences may partly explain the observed variations in growth and development between breastfed and formula-fed infants. This study used mice compared three groups: a control group (mixed vegetable oil), an OPO group (vegetable oil added with OPO), and a human milk fat substitute (HMFS) group formulated to match the fatty acid composition of breast milk. Compared to the control group and OPO group, HMFS-fed mice exhibited reduced body fat content and improved cogniti…

From breast to bite: Nutritional management in infantile food protein-induced enterocolitis syndrome (FPIES).

Food protein-induced enterocolitis syndrome (FPIES) is a non-IgE-mediated food allergy that presents primarily in infancy. Managing nutrition in infants with FPIES presents unique challenges due to its lack of diagnostic biomarkers and the high potential for feeding difficulties, restricted diets, and growth faltering. Breastfed infants may occasionally react to trace food proteins in human milk, requiring maternal elimination diets and cautious reintroduction strategies. Formula-fed infants with cow's milk- or soy-triggered FPIES may need extensively hydrolyzed or amino acid-based formulas. I

Diagnosis and Treatment of Gastroesophageal Reflux in Infants and Children.

Gastroesophageal reflux is defined as the passage of stomach contents into the esophagus with or without accompanied regurgitation (spitting up) and vomiting. It is a normal physiologic process that occurs throughout the day in infants and less often in children and adolescents. Gastroesophageal reflux disease (GERD) is reflux that causes troublesome symptoms or leads to medical complications. The diagnoses of gastroesophageal reflux and GERD are based on the history and physical examination. Diagnostic tests, such as endoscopy, barium study, multiple intraluminal impedance, and pH monitoring,

Beyond bronchopulmonary dysplasia: impact of human milk feeding on long-term respiratory morbidity in very preterm infants.

UNLABELLED: Very preterm infants are at increased risk of respiratory morbidity, with bronchopulmonary dysplasia (BPD) being one of the most common neonatal respiratory complications. Although human milk feeding has been linked to improved respiratory outcomes in preterm infants, evidence on long-term morbidity and differences between pasteurized donor human milk (PDM) and mother's own milk (MOM) is limited. The objectives of this study are to evaluate the association between human milk exposure and long-term respiratory morbidity in very preterm infants up to 18 months and to assess whether r

Strategies for the Preservation, Restoration and Modulation of the Human Milk Microbiota. Implications for Human Milk Banks and Neonatal Intensive Care Units.

Studies carried in the last years have revealed that human milk contains a site-specific microbiota and constitutes a source of potentially beneficial bacteria to the infant gut. Once in the infant gut, these bacteria contribute to the assembly of a physiological gut microbiota and may play several functions, contributing to infant metabolism, protection against infections, immunomodulation or neuromodulation. Many preterm neonates are fed with pasteurized donor's human milk (DHM) or formula and, therefore, are devoid of contact with human milk microbes. As a consequence, new strategies are re
