Health care utilization before and after COVID-19 diagnosis: a multidisease matched-cohort study by sociodemographic factors.

BACKGROUND: The COVID-19 pandemic substantially disrupted healthcare utilization across clinical conditions, populations, and care settings. Consistent and equitable access to care is essential to mitigate the long-term consequences of COVID-19, particularly among individuals with pre-existing conditions. While prior studies have documented broad patterns in healthcare use during the pandemic, limited evidence exists on how utilization shifts before and after a COVID-19 diagnosis, particularly across specific diseases, encounter types, and sociodemographic subgroups. METHODS: We conducted a m

Hungary health system review.

Hungary has achieved a successful transition from an overly centralized, integrated Semashko-style health care system to a purchaser provider split model with output-based payment methods. Although there have been substantial increases in life expectancy in recent years among both men and women, many health outcomes remain poor, placing Hungary among the countries with the worst health status and highest rate of avoidable mortality in the EU (life expectancy at birth trailed the EU27 average by 5.1 years in 2009). Lifestyle factors especially the traditionally unhealthy Hungarian diet, alcohol

From early risk to preventive action: Empowerment based digital triage and intervention for burnout, anxiety, and depression in Scandinavian healthcare workers.

BACKGROUND: Burnout, anxiety, and depression among healthcare workers are associated with long-term sickness absence, reduced quality of care, and impaired patient safety. In Sweden and other Scandinavian countries, common mental disorders account for a substantial share of sickness absence, with costs borne by employers and national insurance systems (the "payee side"). Early identification of workers at elevated risk, followed by proportionate preventive support, is therefore central to sustainable hospital management and occupational health. The Empowerment for Participation (EFP) assessmen

The Chronic Kidney Disease Care Model: Quantifying Management Strategies for Improving Patient Outcomes in the USA.

PURPOSE: While chronic kidney disease (CKD) afflicts an estimated 14% of US adults, the majority remain unaware of the ailment until later stages. CKD progression can lead to reduced quality of life, cardiovascular disease (CVD), and end-stage renal disease (ESRD). Consensus guidelines highlight novel interventions for the early recognition and management of CKD. Here, we describe the rationale and structure of a policy model for CKD that provides contemporary estimates of long-term effectiveness and cost-effectiveness of medical interventions. The utility of the model is illustrated by assess

Alzheimer's disease severity and its association with patient and caregiver quality of life in Japan: results of a community-based survey.

BACKGROUND: Alzheimer's disease (AD) dementia, a progressive neurodegenerative disease, exerts significant burden upon patients, caregivers, and healthcare systems globally. The current study investigated the associations between AD dementia patient disease severity and health-related quality of life (HRQoL) of both patients (proxy report) and their caregivers living in Japan, as well as caregiving-related comorbidities such as depression. METHODS: This cross-sectional study used self-reported data from caregivers of people diagnosed with AD dementia by a healthcare provider in Japan. Caregiv

The Use of Clozapine in the Serious Mental Illness Patients Enrolled in an Assertive Community Treatment Program.

Introduction Health care cost is projected to reach 20% of the nation's gross national product (GNP) by 2016. 6.2% of this is from mental health. The National Institute of Mental Health (NIMH) estimates the prevalence of serious mental illness (SMI) at 13.1 million or 5.2% of American adults age 18 or over. Hence, mental health care cost for this patient population is significant. Patients with SMI involved in an Assertive Community Treatment (ACT) program are individuals who experience the most intractable symptoms and the greatest level of dysfunction from their mental illness. These individ

A Mobile Virtual Reality-Based Intervention for Stress Reduction Among Young Adults: Pilot Randomized Controlled Trial.

BACKGROUND: Young adults experience disproportionately high levels of stress, creating significant health challenges. This demographic often underuses traditional stress management methods due to perceived ineffectiveness, inconvenience, or stigma. Given these challenges, mobile virtual reality (VR) offers a novel, engaging, and accessible alternative stress management tool that may be more appealing to young adults. OBJECTIVE: This pilot randomized controlled trial (RCT) evaluated the preliminary effects of MindCo Relief (MR), a mobile VR-based stress management app, accompanied by online co

Adenomyosis incidence, prevalence and treatment: United States population-based study 2006-2015.

BACKGROUND: Adenomyosis symptoms are disabling. Population-based data on incidence and prevalence of adenomyosis are lacking that could guide future evidence-based treatments and clinical management. OBJECTIVE: To evaluate the incidence, 10-year secular trends, and prevalence of adenomyosis diagnoses and to describe symptoms and treatment patterns in a large U.S. cohort. STUDY DESIGN: We performed a retrospective population-based cohort study of women aged 16-60 years in 2006-2015, enrolled in Kaiser Permanente Washington, a mixed-model health insurance and care delivery system. Periods of altered risk for non-fatal drug overdose: a self-controlled case series.

BACKGROUND: Being recently released from prison or discharged from hospital, or being dispensed opioids, benzodiazepines, or antipsychotics have been associated with an increased risk of fatal drug overdose. This study aimed to examine the association between these periods and non-fatal drug overdose using a within-person design. METHODS: In this self-controlled case series, we used data from the provincial health insurance client roster to identify a 20% random sample of residents (aged ≥10 years) in British Columbia, Canada between Jan 1, 2015, and Dec 31, 2017 (n=921 346). Association of potentially inappropriate medications and need for long-term care among older adults: a matched cohort study.

BACKGROUND: With an increase in the aging population, the number of older adults who require long-term care (LTC) is growing, enhancing drug-related issues. The reduced capacity of LTC users to precisely utilize medical services poses additional challenges owing to restrictions in daily activities. We compared older adults who required LTC with those who did not require LTC to confirm differences in the use of potentially inappropriate medications (PIMs), frequently used PIMs, and associating factors in Korea. METHODS: Using the Korean National Health Insurance Service cohort data, adults age

Comparison of long-term survival with continued medical therapy, vagus nerve stimulation, and cranial epilepsy surgery in paediatric patients with drug-resistant epilepsy in the USA: an observational cohort study.

BACKGROUND: Long-term survival in paediatric epilepsy is incompletely characterised. A better understanding of treatment effects on mortality in paediatric patients with drug-resistant epilepsy is needed for health-care decision making. We aimed to compare the long-term survival rates associated with antiseizure medications only, antiseizure medications plus vagus nerve stimulation (VNS), and antiseizure medications plus cranial epilepsy surgery in paediatric patients with drug-resistant epilepsy using a large national administrative database in the USA. METHODS: In this observational cohort

Long-term Persistence of First-line Biologics for Patients With Psoriasis and Psoriatic Arthritis in the French Health Insurance Database.

IMPORTANCE: Treatment options for psoriasis (PsO) and psoriatic arthritis (PsA) have evolved significantly throughout the era of biologics. Clinical trials are inadequate to assess the relative long-term efficacy of biologics and are often insufficient regarding safety. OBJECTIVES: To assess the long-term persistence of different biologic classes to treat PsO and PsA. DESIGN, SETTING, AND PARTICIPANTS: This nationwide cohort study involved the administrative health care database of the French health insurance scheme linked to the hospital discharge database. All adults with PsO and PsA who w

[Contribution of leflunomide to the cost effectiveness of sequential DMARD therapy of rheumatoid arthritis in Germany].

Since November 1999, leflunomide (LEF), an innovative disease-modifying antirheumatic drug (DMARD), is available in Germany for treatment of rheumatoid arthritis (RA). LEF slows radiographic disease progression and improves functional capacity as well as healthrelated quality of life of RA patients. Resources for health care of the patients are limited in Germany as in all other countries. The purpose of the analysis therefore was to compare the cost effectiveness of the following alternatives: LEF in sequential monotherapy with other DMARDs versus sequential monotherapy of other DMARDs. The t

[Prolonged Weaning - S2k-Guideline Published by the German Respiratory Society].

Mechanical ventilation (MV) is an essential part of modern intensive care medicine. MV is performed in patients with severe respiratory failure caused by insufficiency of respiratory muscles and/or lung parenchymal disease when/after other treatments, (i. e. medication, oxygen, secretion management, continuous positive airway pressure or nasal highflow) have failed.MV is required to maintain gas exchange and to buy time for curative therapy of the underlying cause of respiratory failure. In the majority of patients weaning from MV is routine and causes no special problems. However, about 20 %

A Tool to Assess Risk of De Novo Opioid Abuse or Dependence.

BACKGROUND: Determining risk factors for opioid abuse or dependence will help clinicians practice informed prescribing and may help mitigate opioid abuse or dependence. The purpose of this study is to identify variables predicting opioid abuse or dependence. METHODS: A retrospective cohort study using de-identified integrated pharmacy and medical claims was performed between October 2009 and September 2013. Patients with at least 1 opioid prescription claim during the index period (index claim) were identified. We ascertained risk factors using data from 12 months before the index claim (pre-
