Simultaneous ileocolic and abdominoperineal resection using the da Vinci SP system for synchronous triple colorectal cancer: a case report.

Robot-assisted surgery for colorectal cancer is gaining traction in Japan, particularly with the approval of the da Vinci SP system for insurance coverage in 2022. This system enables precise and flexible procedures through a single port accommodating three instrument arms and a camera. We present a case involving a 73-year-old man with synchronous tumors in the cecum, sigmoid colon, and rectum (Rb). Simultaneous resection was successfully performed using the da Vinci SP system by a robotic surgical team and a perineal team without complications. To our knowledge, this is the first reported ca

Anatomy and Physiology for the Abdominal Aortic Aneurysm Repair.

In 2006, commercially produced endovascular aneurysm repair (EVAR) devices were approved by the Japanese Ministry of Health, Labour and Welfare, and their cost began to be covered by Japanese medical insurance. Meanwhile, the number of juxtarenal abdominal aortic aneurysms (AAA) to need the suprarenal clamp are increasing and the number of infra-renal AAAs are decreasing for open repair. In this era when EVAR has been growing rapidly for 11 years, it is a good opportunity to learn the surgical repair of AAA. I review the basic and advanced anatomy and physiology concepts which are needed for a

Nerve and Tendon Transfer Surgery in Cervical Spinal Cord Injury: Individualized Choices to Optimize Function.

Background: Recent adaption of nerve transfer surgery to improve upper extremity function in cervical spinal cord injury (SCI) is an exciting development. Tendon transfer procedures are well established, reliable, and can significantly improve function. Despite this, few eligible surgical candidates in the United States undergo these restorative surgeries. Evidence Acquisition: The literature on these procedures was reviewed. Results: Options to improve function include surgery to restore elbow extension, wrist extension, and hand opening and closing function. Tendon transfers are reliable and

Linear IgA bullous dermatosis: an unusual cause of upper eyelid cicatricial entropion.

Chronic cicatrizing conjunctivitis is a relatively uncommon condition resulting in significant ophthalmic morbidity, including keratoconjunctivitis sicca, cicatricial entropion, trichiasis, corneal scarring, significant discomfort, and visual loss. The potential causes of cicatrizing conjunctivitis are varied and include commonly encountered entities such as ocular cicatricial pemphigoid, Stevens-Johnson syndrome, and trachoma and many more rare causes which are particularly difficult to diagnose and treat and may not be familiar to the ophthalmologist. The authors herein present a case of chr

[Dr. Ivo Sercer--a forgotten Croatian ophthalmologist] ].

Dr Ivo Sercer (Rakovac near Karlovac, 1892--Zagreb, 1959) graduated medical studies in Graz (1915) and specialized in ophthalmology. In April 1919 he was appointed head of the ophthalmology department of the City hospital in Subotica, where he worked until his death. Backa, a region spreading between the Danube and the Tisa, was one of Europe's major trachoma focal points and Subotica had an eye hospital ever since 1868. In that city a big City hospital with the medical, surgical and ophthalmological departments opened in 1897, and there Dr Sercer was very actively engaged. In addition to his

Psychologic factors involved in the decision to undergo reconstructive surgery after burn injury.

Burn injuries often result in permanent changes in physical appearance and function. Although reconstructive surgery is often considered to improve function or physical appearance, or a combination, variables that may predict use of surgery are relatively unknown. Burn survivors (N = 46; 48% male) were assessed at an evaluation for potential reconstructive surgery. Measures of adjustment, distress, and personality were administered. Several psychologic and demographic variables differed among those who did versus those who did not subsequently undergo surgery. Social, sexual, and family relati

Hemodialysis in the treatment of acute renal failure in tropical Africa: a 20-year review at the Korle Bu Teaching Hospital, Accra.

From 1972 to 1992, 170 patients with acute renal failure (87 M, 83 F; mean age 32.51 +/- SE 0.945) underwent hemodialysis at the renal unit of the Korle Bu Teaching Hospital, Accra. Vascular access was established initially by arteriovenous shunt (133 cases), femoral venous cannulation (10 cases), and subclavian vein cannulation (27 cases). The overall mortality for acute renal failure (ARF) was 31.8% (54/170). The mortality for obstetric cases was 43.7% (14/32); for surgical cases, 33.3% (6/18); medical cases, 28.3% (13/32); and gynecologic (posthysterectomy) cases, 28.3% (2/7). The most impo

[Lung Volume Reduction for Chronic Obstructive Pulmonary Disease].

Chronic obstructive pulmonary disease (COPD) is increasing with population aging and currently ranks as the third leading cause of death worldwide, accounting for approximately three million deaths annually. COPD is characterized by irreversible airflow obstruction, mainly caused by reduced lung elastic recoil due to peripheral airway lesions and emphysema. This condition results in lung hyperinflation, causing dyspnea, reduced exercise capacity, decreased physical activity, and an increased risk of heart failure. For patients with severe COPD unresponsive to medical therapies, surgical interv

Greater need but reduced access: a population study of planned and elective surgery rates in adult mental health service users.

AIMS: Timely access to surgery is an essential part of healthcare. People living with mental health (MH) conditions may have higher rates of chronic illness requiring surgical care but also face barriers to care. There is limited evidence about whether unequal surgical access contributes to health inequalities in this group. METHODS: We examined 1.22 million surgical procedures in public and private hospitals in New South Wales (NSW), Australia, in 2019. In a cross-sectional study of 76,320 MH service users aged 18 and over, surgical procedure rates per 1,000 population were compared to rates
