Case Report: Takotsubo Syndrome Induced by Severe Anaphylactic Reaction During Anesthesia Induction and Subsequent High-Dose Epinephrine Resuscitation.

Takotsubo syndrome (TTS) is a type of non-ischemic cardiomyopathy characterized by an acute reversible left ventricular dysfunction with typical apical ballooning, usually with subsequent complete recovery. Early diagnosis and prompt treatment are of great essence. Herein, we described a case of TTS of a patient who was scheduled initially for laparoscopic endometrial cancer staging. The 69-year-old woman presented with cardiogenic shock induced by the severe anaphylactic reaction to the antibiotics during anesthesia induction. Cardiopulmonary resuscitation (CPR) was implemented while several

Factors predicts skin ulcer following coronary artery bypass.

The number of off-pump coronary artery surgery procedures in high-risk patients such as renal failure, hepatic failure and in anticoagulant drug using patients is increasing. The associated co morbidity and repeated use of electrocautery in postoperative bleeding, caused a susceptibility of patients to pressure or electrocautery ulcers. During a period of three years, 1400 off-pump coronary artery bypass surgery were performed in our center. Of these patients, 20 (A group) suffered from electrocautery sore (ES) and 40 (B group) had pressure sore (PS). These patients were compared with respect

[Extracorporeal membrane oxygenation. A therapeutic alternative in acute heart and/or pulmonary failure?].

Extracorporeal membrane oxygenation was introduced as a supplement ot mechanical ventilation in the treatment of two patients with severe acute respiratory failure and as heart assist in one patient with acute refractory cardiac failure after open heart surgery. The system includes a membrane oxygenator and a roller pump. The whole circuit is coated with partially degraded heparin covalently bound to the surface (Carmeda Bioactive Surface), reducing the need of systemic heparinization to a minimum. In the first case of acute respiratory failure a veno-venous bypass was employed, with cannulas

Wandering coronary stenoses: adrenaline-induced coronary artery spasm in a patient resuscitated from cardiac arrest.

A 68-year-old ex-smoker man with history of allergy, presented to the emergency department with progressive dyspnea one hour following self-medication with aspirin for troublesome headache. Examination revealed diffuse sibilant rhonchi over both lungs. Electrocardiogram showed signs of ischemia. In the intensive care unit, he received bronchodilators, nitroglycerin, and aspirin. Bronchospasm increased, and then the patient was shocked, and developed cardiac arrest. After resuscitation, he was kept on mechanical ventilation and adrenaline infusion. He was scheduled for coronary angiography. The

Recurrent Takotsubo Cardiomyopathy Due to Benzodiazepine Withdrawal.

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a rare cause of acutely decompensated heart failure among patients without pre-existing chronic heart failure. We present a case of recurrent TCM attributed to benzodiazepine withdrawal. CASE SUMMARY: A 40-year-old woman with prior TCM and reverse TCM was admitted for cardiogenic shock after having recently changed her benzodiazepine regimen. She was emergently cannulated for venoarterial extracorporeal membrane oxygenation; transthoracic echocardiography showed an ejection fraction of 10% to 15% with apical hypokinesis. Intra-aortic balloon pump

Transcatheter Closure of Post-Myocardial Infarction Ventricular Septal Rupture Using an Atrial Septal Defect Occluder.

OBJECTIVE: To describe the procedural strategy and technical considerations for transcatheter closure of post-myocardial infarction ventricular septal rupture (VSR) using an atrial septal defect (ASD) occluder in a patient unsuitable for surgical repair. KEY STEPS: An elderly, frail patient with postinfarction VSR and cardiogenic shock was stabilized with an intra-aortic balloon pump. Given the high surgical risk, transcatheter closure was performed after a 14-day delay to allow infarcted tissue stabilization. Multimodality imaging was used to assess defect morphology and rim adequacy. An ASD

Recurrent Spontaneous Coronary Artery Dissection (SCAD) With a Family History of Cardiovascular Disease: A Case Report.

Spontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndrome and is often associated with fibromuscular dysplasia (FMD). We describe the case of a 55-year-old woman with a prior history of SCAD who presented with acute chest pain. Angiography revealed a new dissection in the diagonal branch of the left anterior descending artery with preserved flow, but her condition worsened with recurrent pain and ischemic electrocardiographic changes, ultimately requiring intra-aortic balloon pump support. She stabilized and was discharged on medical therapy, with plans for furth

Echocardiography for short-term mechanical circulatory support: a trans-Atlantic practical guide.

Short-term mechanical circulatory support (stMCS) devices are increasingly utilized for haemodynamic stabilization in patients with cardiogenic shock. Echocardiography plays a pivotal role across the continuum of stMCS use-from patient selection and device implantation to monitoring, troubleshooting, and weaning. This review provides a comprehensive, practical guide for clinicians on the echocardiographic assessment of commonly used stMCS devices, including intra-aortic balloon pump, Impella, ProtekDuo, and veno-arterial extracorporeal membrane oxygenation. We outline device-specific contraind

Acute Triple Coronary Artery Occlusion Leading to Cardiogenic Shock and Cardiac Arrest Emphasizing the Role of Mechanical Circulatory Support (Escalate before It's Too Late).

We report a case of cardiac arrest in a 38-year-old male with no past medical history who presented as a case of ST-segment elevation myocardial infarction, and coronary angiography showed triple coronary artery thrombosis complicated with cardiogenic shock (CS) that warrants starting on inotropic support and insertion of intra-aortic balloon pump. CS diagnosis with a high likelihood of deterioration was established based on hemodynamics assessment; hence, an early prompt decision for escalation of mechanical circulatory support to Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) wa

Acute inferior myocardial infarction combined with papillary muscle rupture: A case report.

The incidence of acute myocardial infarction (AMI) is increasing. Acute papillary muscle rupture is one of the serious and rare mechanical complications of AMI, which occurs mostly in inferior and posterior myocardial infarction. A patient with acute inferior myocardial infarction developed pulmonary edema and refractory shock, followed by cardiac arrest. After cardiopulmonary resuscitation (CPR), revascularization of criminal vessels was carried out by emergency percutaneous transluminal coronary angioplasty (PTCA) under the support of intra-aortic balloon pump (IABP) and extra corporeal memb

Acute Myocardial Infarction of the Left Main Coronary Artery Presenting with Cardiogenic Shock and Pulmonary Edema during Noncardiac Surgery.

Acute myocardial infarction (AMI) caused by severe stenosis of left main coronary artery (LMCA) presenting with cardiogenic shock and pulmonary edema during noncardiac surgery is uncommon, but a catastrophic event. A 77-year-old male with cholangiocarcinoma underwent hepatectomy. During the surgery, he presented with cardiogenic shock, which did not respond to infusion administration or vasopressor. A transesophageal echocardiogram revealed anterior, septal, and lateral severe hypokinesia and impaired left ventricular function. Emergent coronary angiogram showed severe stenosis of LMCA. The pa

Unstable angina in the era of high-sensitivity troponin testing: a genuine entity and a cautionary tale.

High-sensitivity cardiac troponin assays have emerged as a powerful tool in the management of patients presenting acutely to hospital with suspected cardiac chest pain. Recent guidelines emphasize the reassurance offered by low troponin concentrations early after admission. We describe a patient with known coronary artery disease, who presented with a classical history of recurrent myocardial ischaemia. High-sensitivity cardiac Troponin-I concentrations remained in the low-normal sex-specific reference range, despite serial testing on the day of admission, and despite recurrent pain with dynam

[Coronary artery disease : Interventional and operative therapeutic options after cardiac arrest].

Coronary artery disease (CAD) represents a common structural cause for developing cardiac arrest in older patients, whereas in young adults cardiac arrest is more often caused by cardiomyopathies and cardiac channelopathies. A structural heart disease is known in almost 50% of patients prior to cardiac arrest. The present review outlines current interventional and operative therapeutic options for patients surviving cardiac arrest. The focus is on associations between epidemiological data on the incidence of malignant arrhythmias causing cardiac arrest depending on the presence or absence of C

Evidence-based Management of Cardiogenic Shock After Acute Myocardial Infarction.

Guidelines for evidence-based management of patients with cardiogenic shock after acute myocardial infarction focuses on early revascularisation of the occluded coronary artery as well as on support of cardiac failure and improvement of impaired organ perfusion. Also of great importance is effective treatment of shock complications, especially acute respiratory failure and other forms of multiple organ dysfunction syndrome (MODS). Cardiovascular therapy has to be accompanied by best general intensive care of these critically ill patients with high mortality. Most lives can be saved by early re

[Chronic heart failure with impaired left ventricular function (systolic heart failure)].

Chronic heart failure is a common public health problem. The disease has a poor prognosis with high mortality rate and the incidence increases continuously. Prognosis of chronic systolic heart failure can be improved by several different medications as well as by special cardiac interventions based on the newly-published European and American guidelines. In case of severe systolic dysfunction, hospitalization and mortality can be reduced using angiotensin converting enzyme inhibitors, angiotensin receptor blocking drugs, beta-receptor blocking agents and aldosterone antagonists, as evidenced i
