Do the stump knotting technique and specimen retrieval method effect morbidity in laparoscopic appendectomy?

BACKGROUND: Stump closure is the most important part of a laparoscopic appendectomy (LA). Closing the appendix base with knot tying is the most cost-effective method. The defined risk factor for surgical site infection (SSI) after LA is the contamination of trocar entry area by inflamed appendicitis. This study aims to compare the single and double knot technique for stump control and specimen removal methods in LA. METHODS: The data of patients who underwent LA between January 2015 and January 2017 were obtained from prospectively collected database. Single and double knot technique, specime

Peritoneal irrigation vs suction alone during pediatric appendectomy for perforated appendicitis: A meta-analysis.

BACKGROUND: There currently exists no substantial evidence reporting the efficacy of peritoneal irrigation in reducing the incidence of postoperative intra-abdominal abscess in pediatric patients. The purpose of our study was to perform a meta-analysis to compare rates of intra-abdominal abscess after appendectomy between irrigation and suction alone groups. METHODS: We identified studies by a systematic search in EMBASE, PubMed, Web of Science, and the Cochrane Library to recognize randomized controlled trials and case control studies from the 1950 to May 2019. We limited the English languag

Short-term and long-term outcomes after gastrectomy for gastric cancer in patients with chronic kidney disease.

BACKGROUND: The incidence of chronic kidney disease (CKD) is increasing, which might be an obstacle to various aspects of gastric cancer treatment, such as perisurgical management and postsurgical follow-up. The present study aimed to evaluate the short- and long-term surgical outcomes following gastrectomy in patients with CKD. METHODS: We retrospectively analyzed surgical complications and prognosis of patients with gastric cancer. These patients were divided into three groups according to the glomerular filtration rate (GFR): 49 patients with severe CKD (GFR < 29 ml/min/1.73 m(2)), 128 wit

Abdominal computed tomography for diagnosing postoperative lower gastrointestinal tract leaks.

BACKGROUND: Computed tomography (CT) is the most readily available imaging tool for diagnosis of postoperative lower gastrointestinal tract (LGIT) leak. The accuracy and sensitivity of CT for diagnosing a leak from a hollow viscous or anastomotic bowel leakage are still not well established. This retrospective study was conducted in order to define the role of CT in this setting. STUDY DESIGN: The medical records of patients who underwent early relaparotomy (within 30 days) due to LGIT leak following a previous surgery in our department between 1998 and 2006 were reviewed. The ones whose abdo

Rapid identification of microbial pathogens in intra-abdominal infections using multiplex PCR.

Intra-abdominal infections (IAI) resulting from the perforation of hollow viscera are associated with high morbidity and mortality. In addition to timely surgical source control, rapid and accurate pathogen identification is crucial to guide targeted antimicrobial therapy, particularly in polymicrobial infections involving potentially virulent pathogens. In this prospective study (ClinicalTrials.gov NCT07562204), we compared real-time multiplex PCR Unyvero A50 (Curetis®) as a point-of-care (POC) tool with conventional microbiological culture (cMB). 50 patients with suspected hollow organ perfo

Clinical evaluation of multiplex pathogen real-time PCR for early detection of pathogens and antimicrobial resistance genes in intra-abdominal infections.

Rapid pathogen identification is essential for the management of post-operative intra-abdominal infections. However, conventional culture is often time-consuming and has limited pathogen coverage. Our study aimed to evaluate the performance of multiplex pathogen real-time polymerase chain reaction (MP-PCR) in detecting pathogens and antimicrobial resistance genes (ARGs) from postoperative drainage fluids. A total of 63 drainage fluid specimens were prospectively collected from 52 patients who underwent abdominal surgery and were included in the final analysis. All specimens were detected in pa

[Application of the Alexis O wound protector/retractor ring as an alternative silo system in the management of gastroschisis].

Gastroschisis is a congenital abdominal wall defect associated with extraabdominal herniation of the bowels and, rarely, other abdominal organs. In cases of severe visceroabdominal disproportion, primary abdominal wall closure may not be feasible, therefore staged reduction using silo systems is required. The use of the Alexis wound protector/retractor as an alternative silo system in gastroschisis was first described by Kusafuka et al. in 2005. We present the case of a restricted neonate born at 38 weeks of gestation with prenatally diagnosed gastroschisis. Due to the hypoplastic abdominal ca

Clinical Characteristics and Outcomes of Hypertriglyceridemic Acute Pancreatitis Concomitant with Diabetic Ketoacidosis.

OBJECTIVES: Hypertriglyceridemic acute pancreatitis (HTG-AP) is the common cause of acute pancreatitis (AP) after gallstones and alcohol abuse, accounting for up to 10% of the cases of acute pancreatitis. Diabetic ketoacidosis (DKA) may potentially complicate the diagnosis, treatment, and clinical outcomes of HTG-AP. This study aimed to directly compare the clinical characteristics and outcomes of HTG-AP in patients with and without DKA. METHODS: A retrospective analysis was conducted on 289 patients diagnosed with HTG-AP, among whom 23 were also diagnosed with DKA. Epidemiologic characterist

Minor hepatectomy for hepatocellular carcinoma in a patient with portal hypertension: A case report and review of the literature.

INTRODUCTION: Curative modalities for early hepatocellular carcinoma (HCC) include liver resection (LR) and transplantation. For patients with portal hypertension (PH), liver transplantation (LT) is the preferred treatment but is oftentimes limited by organ shortage and can lead candidates to drop off due to disease progression, while hepatectomy has a higher risk of complications. This would pose a dilemma as to whether wait for donor organs or prioritize hepatectomy. PATIENT CONCERNS: The patient was a 56-year-old male, a case of liver cirrhosis due to hepatitis C with sustained virological

Percutaneous cholecystostomy as bridge to surgery vs surgery in unfit patients with acute calculous cholecystitis: A systematic review and meta-analysis.

BACKGROUND: Acute cholecystitis is one of the most common causes of acute abdomen. Early laparoscopic cholecystectomy is the gold standard treatment, still burdened by a risk of intraoperative biliary duct injury. An alternative strategy to manage patients with severe acute cholecystitis is the percutaneous gallbladder drainage (PGBD). METHODS: The Italian Society of Emergency Surgery and Trauma performed a systematic review and meta-analysis with the aim to clarify controversies about the preoperative use of PGBD. We extracted 32 studies: 9 Randomized Control Trial Studies (RCTs) and 23 no R
