Intraoperative examination (IOE) in pediatric extracranial tumors.

BACKGROUND AND OBJECTIVES: Intraoperative evaluation of surgical specimens by frozen sections (IOE) is required to distinguish benign and malignant lesions, assess surgical margins, and determine sample adequacy of biopsies. In the last years, it has been used also for therapeutic decisions, particularly in children, who may need other ancillary procedures, in case of malignancies. Our purpose was the evaluation of diagnostic accuracy, limits, and different role of IOE in pediatric pathology. PATIENTS AND METHODS: From 1990 to 2001, 416 IOEs were performed in 341 children, affected by lymph n

Clinicopathological pitfalls associated with benign uterine mesenchymal tumors: A single-center experience.

Diagnosis of uterine mesenchymal tumors continues to be challenging because of their nonspecific clinicopathological presentation. Several studies have focused on the underdiagnosis/undertreatment of hidden uterine sarcomas. However, few have examined the overdiagnosis/overtreatment of benign uterine mesenchymal tumors that masquerade as uterine sarcomas. We report 4 cases of benign uterine mesenchymal tumors that were preoperatively diagnosed as having malignant potential and underwent extensive surgery. The patients had cotyledonoid-dissecting leiomyomas, uterine tumors resembling ovarian se

Clinical, Diagnostic, and Intervention Approach on Yolk Sac Tumor: A Case Report.

Endodermal primitive tumors, also known as yolk sac tumors (YSTs), are rare tumors that usually develop in the gonads and are more common in females at premenarchal age. Thus, this report details a rare case of YST in a 13-year-old girl who presented to the Obstetrics and Gynecology outpatient department with complaints of pain and a mass in the abdominal region that had been gradually growing in size for two to three months. The patient also reported a history of weight loss, loss of appetite, intermittent fever, and frequent urination. Per abdomen examination resulted in a mass corresponding

Intra-articular migration of tibial suture button in pediatric full epiphyseal anterior cruciate ligament reconstruction. A case report.

We report a case of a 12-years-old boy that underwent anterior cruciate ligament (ACL) reconstruction and lateral meniscus repair. The all-epiphyseal, all-inside technique, with quadriceps tendon autograft and adjustable suspensory button fixation was utilized due to the open physes. Intraoperative fluoroscopy confirmed optimal position of the buttons, while arthroscopic evaluation of the graft showed proper tension, with full range of motion and knee stability. Nevertheless, routine radiographic evaluation of the knee, 6 h postoperatively, revealed tibial button migration through the tunnel

NON-COMMUNICATING HYDROCOELE OF THE CANAL OF NUCK: A RARE FINDING IN A RURAL-DWELLING NIGERIAN WOMAN.

BACKGROUND: The surgical condition termed hydrocoele of the canal of Nuck is one of the rarest clinical entities in the female group. It occurs due to the failure of obliteration of the derivative of the peritoneum, the processus vaginalis. It usually presents with painless inguinal swellings and is sometimes associated with features of intestinal obstruction if there is hernia coexisting with it that has become obstructed. Abdominopelvic ultrasonography and magnetic resonance imaging provide the basis for diagnosis, where these imaging modalities are available. However, a definite diagnosis m

Knot formation of a guidewire during subclavian venous catheterization: A case report.

Central venous catheters are used for a variety of purposes, including emergency care, parenteral nutrition, and chemotherapy, but their insertion can cause mechanical complications such as malposition, arterial puncture, and pneumothorax. Here, we describe a rare case of guidewire knot formation during subclavian vein catheterization. A 70-year-old woman presented to our facility for central venous port placement for chemotherapy after surgical resection of a retroperitoneal liposarcoma. The left subclavian vein was selected and punctured under ultrasound guidance, and a guidewire was introdu

Hyperthermia in a pediatric patient with neuroblastoma during anesthesia: a case report.

BACKGROUND: Neuroblastoma is the most common malignant extracranial solid tumor in pediatrics patients. Intraoperative hyperthermia is extremely rare in patients with neuroblastoma and can cause a series of complications. Here, we represent a case of neuroblastoma accompanied by hyperthermia during anesthesia, and propose a rational explanation and management options. CASE PRESENTATION: The patient had gait disturbance and sitting-related pain without fever. Magnetic resonance imaging revealed a soft tissue mass located in the right posterior mediastinum, paravertebral space and canalis verte

Surgical Excision of a Functional Carotid Body Tumor Presenting Intraoperative Hypertensive Crisis: A Case Report and Literature Review.

Carotid body tumor (CBT) is a rare neoplasm arising from the carotid bifurcation. Functional CBTs are extremely rare and are usually associated with elevated serum catecholamine and catecholamine-induced symptoms such as paroxysmal hypertension, palpitations, dizziness, flushing and tachycardia. We reported a 47-year-old female with a functional CBT that was biochemically silent and had no catecholamine-induced symptoms preoperatively, but experienced hypertensive crisis during surgical excision of the lesion. Postoperative pathology confirmed the diagnosis of CBT, and a functional tumor was c

Isolated Deep Ear Canal Pain: Possible Role of Auricular Branch of Vagus Nerve-Case Illustrations with Cadaveric Correlation.

UNLABELLED: Glossopharyngeal, nervus intermedius, and vagus neuralgias can all present with ear pain. However, to our knowledge, there have been no reports of otalgia as the only symptom of vagus neuralgia. The seventh, ninth, and tenth cranial nerves have many interneural connections, and the exact anatomy and pathophysiology of these neuralgias are often not clear. Moreover, symptoms due to involvement of any of these nerves can be difficult to attribute solely to 1 of them. The overlapping sensory innervation of the external auditory canal can lead to misdiagnosis in patients suffering from

Surgical correction of scoliosis in Rett syndrome: cord monitoring and complications.

AIM: Rett syndrome is a progressive neurodevelopmental disorder that predominantly affects females and is associated with a high incidence of scoliosis and epilepsy. There is scant published work about intraoperative spinal cord monitoring in these patients and little more regarding the rate of perioperative complications. We investigated our institutions' experience with both. METHODS: We retrospectively reviewed the records of 11 patients with Rett syndrome who underwent surgical correction of scoliosis at our institution between 2004 and 2010. RESULTS: Eleven patients underwent successful

Retroperitoneal cavernous hemangioma resected by a pylorus preserving pancreaticoduodenectomy.

A retroperitoneal hemangioma is a rare disease. We report on the diagnosis and treatment of a retroperitoneal hemangioma which had uncommonly invaded into both the pancreas and duodenum, thus requiring a pylorus preserving pancreaticoduodenectomy (PpPD). A 36-year-old man presented to our hospital with abdominal pain. An enhanced computed tomography scan without contrast enhancement revealed a 12 cm × 9 cm mass between the pancreas head and right kidney. Given the high rate of malignancy associated with retroperitoneal tumors, surgical resection was performed. Intraoperatively, the tumor was i

Successful implantation of a deep brain stimulator for essential tremor in a patient with a preexisting cochlear implant: surgical technique: technical case report.

OBJECTIVE: Deep brain stimulation (DBS) has become routine for the treatment of Parkinson's disease and essential tremor. Because both of these disorders are common in patients older than the age of 60, neurosurgeons are likely to encounter increasing numbers of patients who require DBS surgery but who already have another electronic medical implant such as a cardiac pacemaker/defibrillator or intrathecal infusion pump, raising the concern that one device might interfere with the performance of the other. CLINICAL PRESENTATION: Herein we report a modification of surgical technique resulting i

What's right when the gallbladder's left? A case report.

During laparoscopic cholecystectomy for cholelithiasis in a 40-year-old woman, we came upon a case of incidentally discovered left-sided gallbladder (LSG). Two anatomic variants of LSG are known: (a) "true LSG," in which, according to Gross, an accessory gallbladder originates from the left hepatic duct (LHD), the right embryonic bud is readsorbed, and the cystic duct joins either the CBD from the left or the LHD directly. Otherwise, a normal right-sided gallbladder adheres to the inferior left hepatic lobe, and the cystic duct joins the CBD from the right side (as in our case); (b) gallbladde

Evaluation of the association between self-reported pre-operative symptoms with surgically diagnosed endometriosis using the #ENZIAN classification in a multi-centre cohort.

STUDY QUESTION: Is there an association between pre-operative symptoms and intraoperatively described localization and size of endometriosis lesions as assessed by the #ENZIAN classification system? SUMMARY ANSWER: Dyschezia is associated with any deep infiltrating endometriosis (DE) lesions; severe dyspareunia is associated with adenomyosis. WHAT IS KNOWN ALREADY: Previous attempts to correlate the common symptoms of endometriosis to the size and localization of lesions have been of moderate success. STUDY DESIGN, SIZE, DURATION: This prospective, multicentre, non-interventional cross-sect

Radiologic and Neurosurgical Diagnosis of Arterial Neurovascular Conflict on Magnetic Resonance Imaging for Trigeminal Neuralgia in Routine Clinical Practice.

OBJECTIVE: The most common cause of trigeminal neuralgia is neurovascular conflict, particularly arterial compression of the trigeminal nerve (ACTN). It is possible to show this condition preoperatively on fine-cut constructive interference in steady state magnetic resonance imaging (MRI), supplemented by time-of-flight magnetic resonance angiography. We have noticed significant variability in the interpretation of these studies between radiologists and the treating neurosurgeon. We have assessed the sensitivity and specificity of these 2 styles of interpretation compared with the intraoperati

In vivo patellar kinematics during total knee arthroplasty.

Patellar maltracking after total knee arthroplasty often results in complications, including anterior knee pain, instability and impingement … This study demonstrated the feasibility of using a CAS system to measure patellofemoral kinematics. Intraoperative awareness of patellar tracking, including knowledge of the tibiofemoral joint line, could have an impact on the surgical plan and thereby improve the postoperative outcome.
