We report a Down's syndrome patient who underwent penetrating keratoplasty (PKP) surgery. The patient was a 62-year-old female who had Down's syndrome with severe bilateral keratokonus and cataract. Her visual acuity was very poor as she did everything by touch before the surgery. We performed a triple procedure surgery, i.e., PKP, extracapsular cataract extraction, and posterior chamber intraocular lens implantation in her left eye. Fifty patients with advanced degrees of keratoconus, requiring corneal transplantation, were screened for mitral valve prolapse by two dimensional echocardiography. The overall prevalence of 58% was found to be statistically higher than the prevalence of 7% found in a group of age and sex-matched controls. Cells of keratoconus corneas have been reported to produce higher levels of collagenolytic/gelatinolytic enzymatic activities than do cells of normal corneas. Contact lens intolerance in keratoconus may be due to the formation of a proud nebula at or near the apex of the cone that gives rise to contact lens related abrasions. These nebulae do not flatten when contact lens wear is discontinued. We describe a simple technique of superficial keratectomy to remove proud nebulae in which the resulting defect healed quickly under a therapeutic hydrogel lens. Soft contact lenses have been used in 41 patients (71 eyes) aged 10-50 years with uni- and bilateral keratoconus. In 46 eyes (64.8 p.c.) application of soft contact lens enabled visual acuity of 5/5 to 5/25, mainly in initial or intermediate phases of keratoconus. In these cases correction with soft contact lenses gave better results than after application of ++sphero-cylindrical spectacles. A 42-year-old man with Leber's congenital amaurosis, cataracts and keratoconus died following abdominal surgery. Postmortem pathological examination of the globes disclosed cone-shaped corneas with unusual central subepithelial scars and a retinopathy consistent with retinitis pigmentosa. A review of the family history revealed two siblings with congenital blindness.
