INTRODUCTION: Metatarsophalangeal (MTP) joint dislocations are uncommon due to the joint's inherent stability. Simultaneous dislocations of multiple lesser MTP joints with an associated fracture are extremely rare and typically result from high-energy trauma.

Twenty subtalar dislocations were reviewed in patients seen at the hospital complex of the University of Pennsylvania, 1950--1979. All patients were available for followup examination and roentgenographic review. These were 15 males and five females in the study. CASE: This is a case of a female patient born with thrombocytopenia-absent radius syndrome, with bilateral upper extremity phocomelia, bilateral hip dislocations, and congenital fusion of the right knee with progressively worsening flexion contracture. At age 3 years and 5 months, the patient was treated with excision of the knee ankylosis and Van Nes rotationplasty. This proved durable at age 20 years (final follow-up) without any need for further surgery and without complication.

Between 1999 and 2001 thirty knees underwent a semitendinosus tendon plasty to recreate the medial patellofemoral ligament for recurrent patellar dislocation. The mean follow-up was 38 months. The mean improvement of the patellofemoral congruence angle after surgery was 14 +/- 7 degrees. Between 1987 and 2006 we performed a modified Thompson's quadricepsplasty on 40 fracture-related stiff knees and followed the patients for a mean of 7.9 years (2 to 11.1). The factors affecting the final gain of movement were investigated. A total of 15 knees required lengthening of the rectus femoris. This report is based on two case stories. Traumatic knee dislocation is a rare but serious event. There is extensive damage to the ligaments of the knee, but the vascular lesions, with an incidence of approximately 29% are of primary concern. Signs of ischaemia indicate arteriography or exploration, and if the vascular lesion is repaired within eight hours, the majority can avoid amputation.
