A retrospective study of 55 patients with Meniere's disease suggested a subgrouping based on presentation of auditory symptoms. Patients were defined as "definite" (documented fluctuating hearing loss and episodic vertigo), "probable" (history of fluctuating hearing loss and episodic vertigo) and "possible" (history of fluctuating tinnitus and episodic vertigo). An in-depth discussion of the natural history, audiometric and vestibular findings is presented.

Idiopathic Sudden Sensorineural Hearing Loss (ISSHL) is defined and the literature on the etiology of ISSHL is reviewed. Existing hypotheses on the etiology of ISSHL are judged on experimental, clinical, laboratory, radiological and post mortem histological evidence. It is concluded that a viral pathogenesis of ISSHL needs further attention in therapy and research.

An alarming increase in the incidence of syphilitic labyrinthitis has prompted us to review the pathology of this condition. The temporal bones of two patients with congenital syphilis of the ear were studied histopathologically. They showed diffuse osteitic changes in the otic capsule associated with severe hydrops and degeneration of the membranous labyrinth. The posterosuperior wall of the external auditory canal as well as the auditory ossicles showed numerous connective tissue filled spaces surrounded by thin bony trabeculae. OBJECTIVE: Gadolinium-enhanced magnetic resonance imaging (MRI) is useful in assessing inflammatory and neoplastic lesions of the labyrinth and facial nerve. The following cases demonstrate the ability of MRI to differentiate neoplastic from inflammatory lesions within the labyrinth.
PATIENTS OR OTHER PARTICIPANTS: Nine patients were selected with enhancing lesions of the labyrinth and the facial nerve identified on MRI.
INTERVENTION: Acyclovir and prednisone were prescribed for herpes zoster oticus; surgical removal of neoplastic lesions was performed.
