VisuMax Flap 2.0: a flap plus technique to reduce incidence of an opaque bubble layer in femtosecond laser-assisted LASIK.

PURPOSE: To evaluate the incidence of an opaque bubble layer (OBL) in femtosecond laser-assisted in situ keratomileusis (FS-LASIK) flaps created with VisuMax Flap 2.0 as a result of a modification in the parameters of the flap programming. METHODS: This retrospective study was comprised of 1400 eyes of 715 patients who received FS-LASIK surgery. OBLs were measured and reported as a percentage of the flap area to identify the incidence and extent. Flap creation, which is a modification technique, was performed with 8.1-mm flap diameters plus 0.3-mm enlarged interlamellar photodisruption (group

Clinical results of topography-guided laser-assisted in situ keratomileusis using the anterior corneal astigmatism axis and manifest refractive astigmatism axis.

PURPOSE: To evaluate the clinical results of primary topography-guided femtosecond laser-assisted in situ keratomileusis (TG-FS-LASIK) using the Pentacam-measured anterior corneal astigmatism axis (ACA) or manifest refractive astigmatism axis (MRA). METHODS: In this prospective cohort study, all eyes were treated with primary TG-FS-LASIK using the manifest cylinder. Thirty-two right eyes were treated using ACA with axis disparity > 5° in the experimental group, and 32 right eyes were treated using MRA with axis disparity ≤ 5° in the control group. Visual, refractive outcomes, and corneal high

Corneal Epithelial Thickness Changes Following SMILE for Myopia With High Astigmatism.

PURPOSE: To evaluate the corneal epithelial thickness (CET) profile changes after small incision lenticule extraction (SMILE) surgery for myopic astigmatism correction of greater than 2.00 diopters (D). METHODS: This prospective observational study included 40 eyes (23 patients) treated with SMILE for myopia with cylinders of -2.25 to -4.50 D. Along with standard ophthalmic examinations, CET maps with a diameter of 9 mm were measured by high-resolution spectral-domain optical coherence tomography preoperatively and postoperatively. Correlations between the degree of residual astigmatism and t

Short term changes in corneal stress-strain index and other corneal biomechanical parameters post-laser in situ keratomileusis.

PURPOSE: To report the short-term changes in a corneal stress-strain index (SSI) and other corneal biomechanical parameters post-laser in situ keratomileusis (LASIK) surgery. METHODS: A retrospective study was conducted at a tertiary eye care center wherein patients who had undergone LASIK (microkeratome blade and femtosecond bladeless LASIK) between July and December 2019 were enrolled. Patients of age group 20-40 years, best-corrected visual acuity of 20/20, intraocular pressure (IOP) <22 mmHg, pre-LASIK pachymetry >500 microns, and corneal astigmatism ≤3.00 D were included. Subjects with a

[Enhancing the management and control of perioperative infection in small incision lenticule extraction].

Small incision lenticule extraction (SMILE) has been widely used in the treatment of myopia and astigmatism due to its small incision (2 mm), without open corneal flap, rapid recovery of the ocular surface function and good comfort. However, it is the microincision and the potential cavity under the corneal cap that provide a suitable environment for microbial nourishment. In this article, the characteristics of SMILE and the particularity of postoperative infection are stated, and the key points of risk management and control during the period of operation as well as the principles of identif

Management of cap striae following challenging small incision lenticule extraction surgery - A case report.

A 24-year-old female underwent small incision lenticule extraction (SMILE) for myopic astigmatism OU. In the left eye, cap-lenticular adhesion along with tearing of the cap occurred, resulting in a gaped incision and transverse striae involving the visual axis on the first post op day. Uncorrected distance visual acuity (UDVA) was 20/32. The case was managed with interface wash and stretching of the cap, in order to iron out the striae. Post intervention, the UDVA improved to 20/20, striae resolved, and interface remained clear through a follow-up of nine months, suggesting that cap striae in

Dry Eyes After SMILE.

Dry eyes is one of the most common complications after laser vision correction. Small incision lenticule extraction (SMILE) is a flapless procedure with a smaller corneal incision, less corneal nerves are transected during, making it theoretically less prone to dry eyes. Both SMILE and femtosecond-laser-assisted in situ keratomileusis (FS-LASIK) induce a transient worsening in dry eye parameters, but there is evidence showing that SMILE holds promises to have fewer negative impacts on the ocular surface parameters and allow an earlier recovery. SMILE-treated eyes may also have shown less corne

The use of femtosecond lasers in cataract surgery: review of the published results with the LenSx system.

PURPOSE: To review the published literature describing the use of the LenSx femtosecond laser technology (Alcon Laboratories, Inc., Fort Worth, TX) in cataract surgery. METHODS: Literature review. RESULTS: The LenSx system has been used in more than 200,000 cataract surgery procedures to date. Pre-clinical evaluations have shown that this system produces accurate and reproducible capsulorhexes, and that both the energy required for phacoemulsification and total phacoemulsification time are less than with conventional phacoemulsification. In comparative studies, femtosecond lasers have been s

Novel experience of laser-assisted 'inside-out' central venous access in a patient with bilateral subclavian vein occlusion requiring pacemaker implantation.

AIM: This new laser facilitated 'inside-out' technique was used for transvenous pacemaker insertion in a pacemaker-dependent patient with bilateral subclavian occlusion and a failed epicardial system who is not suitable for a transfemoral approach. METHOD AND RESULTS: Procedure was undertaken under general anaesthesia with venous access obtained from right femoral vein and left axillary vein. 7F multipurpose catheter was used to enter proximal edge of the occluded segment of subclavian vein via femoral approach, which then supported stiff angioplasty wires and microcatheters to tunnel into th

Targeted UV therapy in the treatment of psoriasis.

Ultraviolet (UV) light is an effective treatment for extensive psoriasis and some other inflammatory skin conditions. Because the predominant effect of UV is a local one (as opposed to a systemic effect on immunity), localized delivery of ultraviolet B radiation (UVB) may be a useful treatment for localized variants of psoriasis and other conditions. The article reviews the literature regarding use of localized UV therapy. A theoretical benefit of localized UV therapy is reduced toxicity compared with whole-body therapy. Practical benefits in psoriasis treatment include higher efficacy and mor

[Corneal complications after PRK for hyperopia].

We report the case of a patient with epithelial hyperplasia, peripheral haze and regression after transepithelial photorefractive keratectomy (PRK) for hyperopia outside the limits set by the committee for refractive surgery (KRC). Retreatment was performed on both eyes 1 year postoperatively and later an ablation on the left eye due to scarring. Regardless of these procedures haze, scarring and regression appeared again in both eyes. The patient will currently retain a permanent reduction of visual acuity as another treatment of hyperopia outside the KRC recommendations should not be performe

[Corneal dystrophies].

The article reviews modern clinical, morphological, and genetic aspects of corneal dystrophies based on the most recent international classification updated in 2015. Corneal dystrophies is a group of slow progressing, non-inflammatory corneal pathologies, most of which are characterized by variability of the associated traits. The existence of such pathologies makes important their differential diagnosis from acute inflammatory processes of various etiology, which require urgent therapy. Conservative treatment suitable for dystrophies is usually associated with disorders of the anterior cornea
