Environmental renal disease: lead, cadmium and Balkan endemic nephropathy.

The similarity of lead and cadmium nephropathy to Balkan endemic nephropathy warrants careful reevaluation of the possibility that these nephrotoxic metals contribute to the production of the endemic renal disease. Low-level environmental exposure may result in a relationship between the concentration of the metals in tissue storage sites and biological fluids that differs from that encountered after occupational exposure. Urine and blood concentrations may therefore be inadequate measures of exposure. Lead is accumulated in the skeleton and cadmium in the liver and kidneys with biological hal…

Observations on renal function in workers exposed to lead.

Renal function was studied in 40 workers chronically exposed to lead at a smelter. The geometric means of lead were 40 (range 24.0 - 63.0) micrograms/dl in blood and 47 (range 19 - 115) micrograms/g creatinine or 71 (range 20 - 250) micrograms/l in urine, respectively. There were no significant differences between the lead-exposed group and a control group in the concentrations of beta 2-microglobulin in serum, urinary total protein and IgG, whereas the level of beta 2-microglobulin in urine was significantly higher in the lead-exposed group (p less than 0.01). No individual lead worker showed…

Recurrent lead poisoning in a child with immobilization osteoporosis.

Lead poisoning associated with progressive osteoporosis in patients who have been previously lead poisoned has been described but poorly documented. We managed a 4-year-old child with a prior history of plumbism who developed recurrent blood lead elevations (as high as 70 mcg/dl), requiring multiple courses of EDTA, after acute paraplegia from transverse myelitis. The patient was hospitalized throughout these periods of chelation. No exogenous sources of lead were found. Calcium, phosphate, magnesium, alkaline phosphatase and parathyroid hormone levels remained normal while vitamin D levels we…

Does infarction of the medullary pyramid lead to spasticity?

A 60-year-old patient developed leg weakness leading to spasticity, secondary to a sharply localized infarction of the medullary pyramids. Within a month, spasticity had replaced the initial flaccidity. Several years later, rupture of an atherosclerotic aneurysm of the basilar artery led to subarachnoid hemorrhage and eventual death.

Lead poisoning in infancy--unusual causes in the U.A.E.

Six infants, three of them neonates, were diagnosed as having acute lead poisoning; four of them had acute encephalopathy. All had been given an indigenous preparation, 'Bint Al Zahab' (Daughter of Gold), for abdominal colic and early passage of meconium after birth. Chemical analysis of this powder revealed a lead content of 82.5%. The index case had anaemia with punctate basophilia, dense metaphysial lines on X-ray and markedly raised blood lead levels, arousing a strong index of suspicion for the early diagnosis of subsequent cases. Computerized axial tomography (CAT) scan in three cases sh…

Chronic lead nephropathy in Queensland: alternative methods of diagnosis.

Indices of past lead absorption were measured and compared in patients with chronic renal failure from many causes, including some with chronic lead nephropathy. X-ray fluorescence (XRF) yielded finger bone lead concentrations by a new in vivo method. These correlated significantly with excess urinary lead following calcium di-sodium EDTA (ethylenediamine tetra-acetate) and erythrocyte lead concentration. Discriminant function analysis demonstrated that the patients in the study could be separated into two groups without any reference to the EDTA lead excretion test using the following variabl…

[Lead encephalopathy with thrombosis of the superior longitudinal sinus].

A 32 year old obese, diabetic and hypertensive woman presented with signs of increasing intracranial pressure: diplopia, partial lateralised Jacksonian seizures followed by alternating post-ictal hemiplegia. She was also anaemic. Investigation revealed lead poisoning and thrombosis of the superior sagittal sinus. She recovered rapidly with heparin and calcium EDTA therapy, except for persistent optic atrophy due to the papilloedema. The authors discuss the relation of lead encephalopathy and dural sinus thrombosis, and the contribution of each condition to this patient's clinical symptoms.

Lead encephalopathy. A case report and review of management.

Acute lead encephalopathy has become a rare syndrome in the United States. Early recognition of the disease, with institution of specific chelation therapy, is critical in order to minimize mortality and morbidity. Lead intoxication, however, may mimic other more common diseases. We report an 8-year-old child with known sickle cell anemia who presented initially with findings suggestive of vaso-occlusive crisis but who deteriorated rapidly and was found to have severe lead poisoning. We present her hospital course and review the management of lead encephalopathy in detail.

Analysis of lead in circumpulpal dentin of deciduous teeth.

Absorbed lead continuously adds to the lead deposited in the vascularized circumpulpal dentin of the teeth. Thus, this dental tissue is expected to contain a lead concentration which reflects the integrated lead exposure during the time from completion of tooth formation to tooth extraction or shedding. A method has been developed to assess the lead level in the dentin surrounding the pulp chamber in deciduous teeth. Variation within the tooth is minimal, but upper medial incisors show a slightly lower lead level than do other incisors, as assessed in 714 teeth from first-grade Danish school c…

Lead at low dose and the behavior of children.

Over 3,000 children attending first and second grade in public school were classified as to past lead exposure according to the concentrations of lead in their teeth. Those in the highest and lowest deciles for lead were evaluated by a broad panel of neuropsychologic outcome measures. Controlled either by matching or analysis of covariance were 39 other variables which could effect outcome. High lead children were significantly impaired on IQ auditory processing and reaction time under varying intervals of delay. Teacher's rating scales showed a dose-related increase in non-adaptive classroom …

On the absolute magnitude of the human lead vector.

Twelve cases with temporary artificial pacing were used to investigate on the efficacy of a bipolar current source within the heart to generate potential variations in Frank lead ECGs. A constant current, subthreshold square pulse was applied to the catheter electrodes and the artefact was recorded with the Frank system. Assuming that the Frank system was normalized and orthogonal, the lead vector magnitude was calculated as 0.58 ohm/cm on the average. The value of the lead vector was inversely related to the thorax size. The direction of the vector determined from the recorded potentials appr…

Neuropathological studies of experimental toxocariasis in lead exposed mice.

Binghamton Heterogeneous stock mice intubated with embryonated eggs from the common parasite of dogs, Toxocara canis, display dramatic alterations in behavior. Many of these behavioral changes are markedly attenuated if mice are maintained also on a drinking solution containing lead acetate. The data presented here, however, show that the extensive neural degeneration of the CNS occurs after T. canis infection and that the nature and extent of this degeneration is similar, regardless of whether mice had been administered parasites alone or in combination with lead. Thus the neuropathological d…

Blood lead concentrations: apparent reduction over approximately one decade.

Blood lead concentrations of male subjects presenting for pre-employment examination during the period 1967-1969 have been compared with those obtained during the period 1978-1980. A statistically significant reduction of the geometric mean value from 20.2 microgram Pb/100 whole blood to 16.6 microgram Pb/100 ml whole blood has been found.

Toxocara canis and lead alter consummatory behavior in mice.

Ingestion of palatable and unpalatable solutions was measured in adult mice in which had been administered the common parasite of the dog, Toxocara canis alone, or in combination with lead. In addition, response to hot plate and susceptibility to electroconvulsive seizure were also measured. Results from the palatability test indicated that either lead or Toxocara may alter the mouse's mode of interacting with its environment. However, the two agents in combination interacted in their effects on consummatory behavior. Results from the hot plate and ECS measures were less clear with respect to …

Behavioral effects of lead and Toxocara canis in mice.

Adult mice were administered the common parasite Toxocara canis or lead or both. The parasite clearly altered mouse performance on tests of exploration, activity, learning, and motor coordination; behavioral effects in mice receiving lead alone were less general. Consequence of Toxocara administration appeared attenuated in animals receiving both agents. Parasite larvae were found in the central nervous system in all infected mice.

Abiotic factors affecting the toxicity of lead to fungi.

The toxicity of lead (Pb) to fungi in pure culture was influenced by several abiotic factors: pH, inorganic anions, clay minerals, and particulate (humic acid) and soluble organic matter. The toxicity of Pb was potentiated under acidic conditions (pH 5 and 6), and phosphate or carbonate anions reduced the toxicity, apparently as a result of the formation of sparingly soluble Pb salts. Clay minerals (montmorillonite greater than attapulgite greater than kaolinite) and particulate humic acid protected against the toxicity of Pb, presumably as the result of sorption, by cation exchange of the Pb …
