Toward a better death: applying Burkean principles of symbolic action to interpret family adaptation to Karen Ann Quinlan's coma.

This article considers the manner that families come to accept the collapse of identity in a family member who has entered a medical crisis with no hope of returning from it. The transformation is regarded as a "right of passage" and is characterized in terms of both the conditions that bring about resistance to the passage as well as the sorts of symbolic activities that ultimately allow the transformation to occur. The theoretical source that is used to discuss both these issues is Kenneth Burke's (1969) theory of Dramatism, regarded herein as a template that guides both interpersonal action

Possessed and dispossessed youth: spirit possession of school children in northwest Madagascar.

Spirit possession is a common experience shared by many women in northwest Madagascar. In the town of Ambanja, possession by volatile and dangerous Njarinintsy spirits is an affliction which strikes young, adolescent schoolgirls. When Njarinintsy is contrasted with tromba, another well-known class of spirits found in this region, it becomes clear that Njarinintsy possession is a relatively recent phenomenon, and its victims form a discrete and unusual group. These girls are, in essence, young migrants, who have moved alone to town in order to attend school. Close examination of their experienc

Self-concept as a "BMT patient", illness intrusiveness, and engulfment in allogeneic bone marrow transplant recipients.

OBJECTIVE: The experience of cancer and its treatment by bone marrow transplantation (BMT) can enhance the salience of one's status as a patient in the evolution of self-concept. Illness and the patient role can come to dominate the sense of self, resulting in feelings of hopelessness, helplessness, and distress ("engulfment"). Illness-induced lifestyle disruptions ("illness intrusiveness") introduce adaptive demands, challenging preexisting conceptions of self. Illness intrusiveness and engulfment may interact, leading affected individuals to construe themselves as highly similar to a prototy

Childhood Trauma, Recent Stressors, and Risk of Depression Relapse in Late-Life Depression.

BACKGROUND: Recent stressors have been linked to poor outcomes of late-life depression (LLD), including relapse. Emerging evidence has linked adverse childhood experiences (ACEs) to the occurrence of LLD. We sought to examine the relationships between recent exposure to stressful life events and ACEs with relapse following remission among older adult. METHODS: In this cohort study, depressed older adults (age ≥65) were provided standard antidepressant medications by a study psychiatrist. Each participant provided responses to the ACEs Questionnaire and to a list of questions regarding stresso

Temporal dynamics of psychosocial risk and resilience pathways in adolescent depressive and anxiety symptoms: a three-wave longitudinal study.

BACKGROUND: Adolescence is a critical neurodevelopmental stage marked by heightened sensitivity to psychosocial influences and increased risk for emotional disorders such as depression and anxiety. Although multiple psychosocial risk and protective factors have been identified, their interrelations and temporal patterning remain insufficiently understood, especially in Chinese populations. METHODS: We analyzed three-wave longitudinal data from 907 junior high school students in Northern China to examine associations among psychosocial stressors, contextual resources and mental health across ad

The Effect of Stress on All-Cause Dementia: A Longitudinal Analysis From the Australian Longitudinal Study on Women's Health.

OBJECTIVES: Perceived stress has been linked to dementia, however the duration of perceived stress, as well as the timing of this exposure, on dementia has not been explored. This study aimed to assess the effect of transient versus prolonged versus persistent perceived stress and exposure to stressful life events (SLE), as well as time windows of exposure, on all-cause dementia. DESIGN: Cohort study. SETTING: The Australian Longitudinal Study on Women's Health (ALSWH). PARTICIPANTS: The oldest cohort (born 1921-26), and the middle cohort (born 1946-51), from the Australian Longitudinal Stu

Life events in pregnancy, early childhood, adolescence and adulthood predicting adult mental illness: A 40-year prospective study.

BACKGROUND: Life events (adversities) represent widely prevalent experiences that have been implicated as causes of mental illnesses. There is little known about whether the experience of multiple life events over the early life course predicts depression, anxiety and delusional thoughts in adulthood, independently of life events in adulthood. METHODS: We take data from a long-running birth cohort study which follows mothers from pregnancy and their children to 40 years of age. Life events are measured in pregnancy, shortly after the birth, and at 5 and 14 years (mother report) and are also r

Evaluating psychological distress associated with life events under the traumatic experience threshold in patients with major depressive and bipolar disorder.

Patients with bipolar disorder (BD) and major depressive disorder (MDD) experience psychological distress associated with daily events that do not meet the threshold for traumatic experiences, referred to as event-related psychological distress (ERPD). Recently, we developed an assessment tool for ERPD, the ERPD-24. This tool considers four factors of ERPD: feelings of revenge, rumination, self-denial, and mental paralysis. We conducted a cross-sectional study between March 2021 and October 2022 to identify the differences and clinical features of ERPD among patients with MDD and BD and health

Immersive virtual reality as a novel approach to investigate the association between adverse events and adolescent paranoid ideation.

PURPOSE: Paranoid ideation is common among adolescents, yet little is known about the precursors. Using a novel immersive virtual reality (VR) paradigm, we tested whether experiences of bullying, and other interpersonal/threatening events, are associated with paranoid ideation to a greater degree than other types of (i) non-interpersonal events or (ii) adverse childhood experiences. METHODS: Self-reported exposure to adverse life events and bullying was collected on 481 adolescents, aged 11-15. We used mixed effects (multilevel) linear regression to estimate the magnitude of associations betw

Association of life course adiposity with risk of incident dementia: a prospective cohort study of 322,336 participants.

Cohort studies report inconsistent associations between body mass index (BMI) and all-cause incident dementia. Furthermore, evidence on fat distribution and body composition measures are scarce and few studies estimated the association between early life adiposity and dementia risk. Here, we included 322,336 participants from UK biobank to investigate the longitudinal association between life course adiposity and risk of all-cause incident dementia and to explore the underlying mechanisms driven by metabolites, inflammatory cells and brain structures. Among the 322,336 individuals (mean (SD) a

Integration of other services with human papillomavirus vaccination; lessons from earlier in the life course highlight the need for new policy and implementation evidence.

Integration of vaccination against human papillomavirus (HPV) with other essential health services for adolescents has been proposed in global strategies and tested in demonstration projects in low- and middle-income countries (LMIC). Published experiences, global guidance, and one key example, the implementation of "HPV Plus" in Tanzania, all demonstrate the need for greater operational evidence to guide future implementation and policy. Review of experiences earlier in the life course, integrating post-partum family planning with infant immunization, show lessons from 13 LMICs that can apply

Dialogical Integration of Life Experiences and Successful Aging.

This article examines the hypothesis that the dialogical integration of life experiences is related to successful aging. Life story interviews with 93 older Czech adults were sorted into categories characterized by specific patterns of life experience integration: (i) without dialogical processes, (ii) with differentiated I-positions, (iii) with dialogical relationships, (iv) partially integrated, and (v) completely integrated. The results indicated that the categories were ordered, yielding low-level correlations with scales of successful aging in predicted directions. A comparison of the cat

Recovery Capital and Symptom Improvement in Gambling Disorder: Correlations with Spirituality and Stressful Life Events in Younger but Not Older Adults.

Although age-related differences have been reported in gambling disorder, prior studies have not examined how age may influence recovery in gambling disorder. Recovery may be influenced by positive factors (e.g., spirituality and recovery capital) and negative factors (e.g., depression, anxiety, and stressful life events). The current study examined associations between these positive and negative factors and gambling disorder DSM-5 symptom improvement in younger and older adults. Younger (less than 55 years of age; n = 86) and older (55 years or older; n = 54) adults, with lifetime gambling d

Healthy women with severe early life trauma show altered neural facilitation of emotion inhibition under acute stress.

BACKGROUND: Across psychopathologies, trauma-exposed individuals suffer from difficulties in inhibiting emotions and regulating attention. In trauma-exposed individuals without psychopathology, only subtle alterations of neural activity involved in regulating emotions have been reported. It remains unclear how these neural systems react to demanding environments, when acute (non-traumatic but ordinary) stress serves to perturbate the system. Moreover, associations with subthreshold clinical symptoms are poorly understood. METHODS: The present fMRI study investigated response inhibition of emo

Causal attributions of potentially traumatic life events in fibromyalgia patients.

AIM: In common clinical practice, potentially traumatic life events and the possible attribution of such events to fibromyalgia often go undetected. Having demonstrated in a previous study that hypnosis facilitates the exploration of such events, we now aim to explore and classify the content of these events and to determine whether there is an association between the type of event, the state of consciousness in which the patient verbalized it, and possible causal attributions of the disorder. METHOD: Thirty-two women (age 52 ± 7 years) suffering from fibromyalgia and receiving care in a prim

Prolonged grief disorder: Its co-occurrence with adjustment disorder and post-traumatic stress disorder in a bereaved Israeli general-population sample.

BACKGROUND: Prolonged grief disorder (PGD) is a new disorder included in the WHO International Classification of Diseases 11th version (ICD-11). This study is the first to use these new ICD-11 PGD guidelines to examine prevalence rates, predictors of PGD and disorder co-occurrence with other stress-related disorders in a survey of 544 bereaved Israelis. METHODS: Descriptive statistics, correlation, linear regression and mediation analysis examined the validity of the ICD-11 diagnostic algorithm. RESULTS: Prevalence of PGD in the Israeli population sample is low (2%). The prevalence rate of p

Stressful life events among homeless people in León (Nicaragua): Quantity, types, timing, and perceived causality.

Nicaragua is one of Latin America's least developed countries and has a lack of data on homeless people, a stigmatized group living in extreme poverty. We conducted structured interviews with homeless people living in the city of León (Nicaragua; n = 41) to obtain data on the quantity, types, timing, and perceived causality of stressful life events (SLEs) experienced during their childhood and adolescence as well as throughout their adult lives. The findings showed that our participants had experienced a high number of extremely severe SLEs at very early ages and most of these SLEs occurred be
