Beyond the Body: Women's Experiences of Life-Limiting Gynecological Cancer and Implications for Feminist Palliative Social Work.

Gynecological cancers are serious, potentially life-limiting illnesses affecting women's physical health, bodily identities, family roles, relationships, and psychosocial care needs. Although psycho-oncology literature has addressed cancer-related distress, the gendered, embodied, and relational dimensions of gynecological cancer remain insufficiently examined within feminist and palliative social work scholarship. This qualitative phenomenological study explores women's lived experiences of gynecological cancer and their psychosocial and palliative care needs from a feminist social work persp

Self-reported care needs of Dutch homeless people with and without a suspected intellectual disability: a 1.5-year follow-up study.

Cognitive impairment is a prevalent problem among the homeless and seems related to more psychosocial problems. However, little is known about the care needs of the subgroup of homeless people with an intellectual disability compared to those without an intellectual disability and how their care needs develop over time. This study explores self-reported care needs within a broad range of life domains among Dutch homeless people with and without a suspected intellectual disability to gain insight into the transition of self-reported care needs from baseline to follow-up in both subgroups. This

Stereotactic ablative radiotherapy or best supportive care in patients with localized pancreatic cancer not receiving chemotherapy and surgery (PANCOSAR): a nationwide multicenter randomized controlled trial according to a TwiCs design.

BACKGROUND: Significant comorbidities, advanced age, and a poor performance status prevent surgery and systemic treatment for many patients with localized (non-metastatic) pancreatic ductal adenocarcinoma (PDAC). These patients are currently treated with 'best supportive care'. Therefore, it is desirable to find a treatment option which could improve both disease control and quality of life in these patients. A brief course of high-dose high-precision radiotherapy i.e. stereotactic ablative body radiotherapy (SABR) may be feasible. METHODS: A nationwide multicenter trial performed within a pr

Places of end-of-life care and death in health policies of four countries (EOLinPLACE Project).

BACKGROUND: Place of death and its concordance with patient preference is a key indicator for end-of-life care, studied cross-nationally and flagged as a priority by the OECD. However, it is unclear if and how 'place' is considered in health policy in relation to end-of-life care. This study aims to examine if and how health policies in different nations consider places of end-of-life care and death. METHODS: We conducted a comparative qualitative study across the US, the Netherlands, Portugal, and Uganda, of health policy documents following the READ (i.e., Ready materials, Extract data, Ana

Healthcare self-management support of stroke patients after discharge: A conceptual analysis using Rodger's evolutionary approach.

AIM(S): To explore the meaning of healthcare self-management support for post-discharged stroke patients. METHOD: Rodgers' evolutionary approach was used to identify antecedents, attributes, related terms, surrogate terms and consequences. DATA SOURCE: Literature from 2012 to 2022 was searched from MEDLINE, CINAHL, PsycINFO and Google Scholar. RESULTS: Three antecedents preceded healthcare self-management support for post-discharged stroke patients: loss of inpatient support, preparedness for self-management and presence of self-management support. Healthcare self-management support for pos

Social care for disabled elderly women in urban China: The roles of the community.

This article analyses the provision of community care for urban disabled elderly women. With the emergence of an aging society in China and the empty nest syndrome in Chinese families, the number of elderly people who cannot take care of themselves is increasing. With the reduction in family size and the weakening of the home care function, traditional family care in China is facing immense challenges. On the one hand, a growing number of disabled elderly women are in urgent need of care; they encounter many difficulties in daily life, including poor health status, the loss of their spouse and

Maximum conservative management for patients with chronic kidney disease stage 5.

Following the expansion of dialysis services for patients with chronic kidney disease, an increasing number of elderly patients with varying degrees of frailty and additional comorbidities have been offered treatment. Life expectancy is somewhat limited in this group of patients, and initiation of dialysis may not necessarily improve quality of life. As such, an increasing number of centers are offering conservative care for patients who have made an informed decision not to have dialysis. As conservative care includes active treatment of anemia, volume overload, blood pressure control, and ma

Care burden for people with palliative needs in rural Nepal: mixed method study.

OBJECTIVES: To explore the level and nature of burden among primary caregivers of people with palliative care needs living in rural Nepal and to understand the sources of support available to them. METHOD: An explanatory sequential mixed-method design was used. A house-to-house survey in two rural districts identified people with palliative care needs. Primary caregivers were invited to complete a structured questionnaire including a modified Caregivers' Burden Scale in End-of-Life Care. A purposive subsample then took part in semi-structured interviews exploring caregiving experiences. Quant

Homecare amyotrophic lateral sclerosis (ALS): A multidisciplinary, home-based model of care for patients with ALS and their caregivers.

INTRODUCTION/AIMS: Multidisciplinary care for patients with amyotrophic lateral sclerosis (ALS) is recommended in international guidelines, but reaches its limits when immobility increases. This pilot project addresses this gap by delivering home-based, specialized, multiprofessional support to ALS patients who are not able to attend outpatient care. The study assessed the feasibility of this model of care and the satisfaction of both patients and caregivers. METHODS: This was a longitudinal cohort study of patients with ALS and their caregivers in the surroundings of Munich, Germany. Patient

Structures of survival: Syndemic pathways to care among people with HIV involved with the criminal legal system.

BACKGROUND: Criminal legal system (CLS) involvement intersects syndemically with HIV, mental illness, substance use, and exposure to violence. Mass incarceration remains both a cause and consequence of racialized health inequities in the U.S. The post-release period is particularly vulnerable for people living with HIV (PLWH) who experience CLS involvement, underscoring the need for care models attuned to their structural and psychosocial contexts. This study examines how syndemic pathways and cumulative life experiences shape HIV care trajectories among people with HIV involved with the CLS.

The value of uncertainty in critical illness? An ethnographic study of patterns and conflicts in care and decision-making trajectories.

BACKGROUND: With increasingly intensive treatments and population ageing, more people face complex treatment and care decisions. We explored patterns of the decision-making processes during critical care, and sources of conflict and resolution. METHODS: Ethnographic study in two Intensive Care Units (ICUs) in an inner city hospital comprising: non-participant observation of general care and decisions, followed by case studies where treatment limitation decisions, comfort care and/or end of life discussions were occurring. These involved: semi-structured interviews with consenting families, wh

Screening for prostate cancer. A decision analytic view.

OBJECTIVE: To determine the clinical and economic effects of screening for prostate cancer with prostate-specific antigen (PSA), transrectal ultrasound (TRUS), and digital rectal examination (DRE). DESIGN: Decision analytic cost-utility analysis comparing four screening strategies with a strategy of not screening. We assumed that the cancer detection rate and stage distribution were predicted by each combination of tests and that localized cancer was treated with radical prostatectomy. For each strategy, we calculated life expectancy, quality-adjusted life expectancy (QALE), and cost-utility
