TRUST: an large language model-based dialogue system for trauma understanding and structured assessments.

OBJECTIVES: While large language models (LLMs) have been widely used to assist clinicians and support patients, no existing work has explored dialogue systems for standard diagnostic interviews and assessments. This study aims to bridge the gap in mental healthcare accessibility by developing an LLM-powered dialogue system that replicates clinician behavior. MATERIALS AND METHODS: We introduce TRUST, a framework of cooperative LLM modules capable of conducting formal diagnostic interviews and assessments for post-traumatic stress disorder (PTSD) following the Clinician-Administered PTSD Scale

Motivated by identity: A social identity analysis of why people become foster carers.

Understanding why individuals are motivated to take on new identities, particularly why people choose to become foster carers, is critical to addressing persistent shortages in foster care systems. Despite policy consensus that foster care is the preferred alternative when children cannot remain with birth families, shortages persist across many countries. Research has traditionally emphasised individualistic motivations, such as altruism, neglecting broader social and identity-based factors. This study applies a Social Identity Approach, drawing on the Social Identity Model of Identity Change

When health changes everything: The disruptive side of identity shifts in people with CF.

Cystic fibrosis (CF) is a severe genetic disorder historically associated with low life expectancy. The introduction of CF transmembrane conductance regulator (CFTR) modulator drugs like Trikafta has dramatically improved health outcomes for many people with CF (pwCF), shifting their lived experiences in unexpected ways. This qualitative study explores the psychological and social consequences of such improvements, focusing specifically on identity disruption and emotional adaptation. Utilizing a phenomenological approach, in-depth interviews were conducted with CF patients who had used Trikaf

Antarctic nematodes survival in Martian and Lunar regolith simulants under terrestrial conditions.

The exploration of extraterrestrial environments has become a focal point of scientific inquiry, driven by advancements in technology and a growing interest in the potential for life beyond Earth. This study investigates the adaptability of Antarctic nematodes, known for thriving in extreme cold and isolation, to simulated Martian (MGS-1) and Lunar (LMS-1) soils. The experiment revealed differential responses in nematode survivability to the two simulants, with Lunar soil demonstrating better adaptability compared to Martian soil. Our study provides valuable insights into the potential surviva

Management of Sideline Medical Emergencies.

Sideline medical care is typically provided by musculoskeletal specialists and orthopaedic surgeons with varying levels of training and experience. While the most common sports injuries are often benign, the potential for catastrophic injury is omnipresent. Prompt recognition of sideline emergencies and expeditious medical management are necessary to minimize the risk of calamitous events. Paramount to successful sideline coverage are both preseason and game-day preparations. Because the skillset needed for the sideline physician may involve management of injuries not commonly seen in everyday

Obstacle Segmentation with Encoder-Decoder Architectures in Low Structured Environments for the Navigation of Visually Impaired People.

Orientation and mobility of visually impaired people usually requires intensive training with mobility aids (e.g. white canes). Assistance systems capture information from the environment, process sensor data and provide the results to the impaired user. The paper presents an approach for efficient segmentation of obstacles in low-structured outdoor environments using encoder-decoder deep learning architectures and depth images. Therefore, an efficient method for generating training data using the v-disparity method is presented. Based on an extensive dataset of RGB and depth images and the co

Methodological Recommendations for Studies on the Daily Life Implementation of Implantable Communication-Brain-Computer Interfaces for Individuals With Locked-in Syndrome.

Implantable brain-computer interfaces (BCIs) promise to be a viable means to restore communication in individuals with locked-in syndrome (LIS). In 2016, we presented the world-first fully implantable BCI system that uses subdural electrocorticography electrodes to record brain signals and a subcutaneous amplifier to transmit the signals to the outside world, and that enabled an individual with LIS to communicate via a tablet computer by selecting icons in spelling software. For future clinical implementation of implantable communication-BCIs, however, much work is still needed, for example, t

A preliminary examination of child well-being of physically abused and neglected children compared to a normative pediatric population.

Federal mandates require state child welfare systems to monitor and improve outcomes for children in three areas: safety, permanency, and well-being. Research across separate domains of child well-being indicates maltreated children may experience lower pediatric health-related quality of life (HRQL). This study assessed well-being in maltreated children using the Pediatric Quality of Life Inventory (PedsQL 4.0), a widely used measure of pediatric HRQL. The PedsQL 4.0 was used to assess well-being in a sample of children (N = 129) receiving child welfare services following reports of alleged p

Quality of life and broader experiences of those with acoustic neuroma: a mixed methods approach.

Background Acoustic neuromas (ANs) are consistently associated with decreased quality of life (QOL) related to the physical and psychosocial impacts of symptoms experienced from the tumour and its treatment. This study explored patient-reported experiences of ANs in New Zealand, with a focus on the impact on QOL and the provision of information, support and services. Methods A mixed methods approach was taken, conducting an online community survey that included the Penn Acoustic Neuroma Quality of Life Scale (N  = 52). Those who indicated interest were offered semi-structured interviews after

Extracorporeal membrane oxygenation versus counterpulsatile, pulsatile, and continuous left ventricular unloading for pediatric mechanical circulatory support.

OBJECTIVES: Despite progress with adult ventricular assist devices, limited options exist to support pediatric patients with life-threatening heart disease. Extracorporeal membrane oxygenation remains the clinical standard. To characterize (patho)physiologic responses to different modes of mechanical unloading of the failing pediatric heart, extracorporeal membrane oxygenation was compared to intra-aortic balloon pump, pulsatile-flow ventricular assist device, or continuous-flow ventricular assist device support in a pediatric heart failure model. DESIGN: Experimental. SETTING: Large animal

Thoracic trauma. Nursing assessment and management.

The importance of the respiratory system to total body functioning cannot be overemphasized. It is imperative that the nurse realize that the first priority in trauma care is to maintain and support the respiratory system. The nurse must be able to rapidly and effectively perform a primary survey, recognize the clinical manifestations of life-threatening thoracic injuries, and intervene in the care of the patient to help stabilize and maintain the patient's respiratory function. The nurse must not only be able to asses, but must also be able to technically intervene rapidly and effectively. Ot

Impact of total dissolved solids on the environment and recent strategies for the complex industrial effluents treatment.

High-TDS wastewater discharged from various industries, including textile, dye & dye intermediate, pharmaceuticals, pulp and paper, tannery, dairy, food, brewery etc., pose a serious impact on environment. Generally, TDS effluents shows concentration considerably higher than their general regulatory discharge limits. Release of high TDS effluent with other contaminants harms aquatic life and makes it difficult to use such aquatic systems for human and animal consumption. A partial reduction of TDS can be achieved in the range of 15-35% by physicochemical treatments like coagulation-flocculatio

Drug-Induced Sleep Endoscopy in Pediatric Obstructive Sleep Apnea: Clinical Applications, Classification Systems, and Anesthetic Considerations.

Background/Objectives: Drug-induced sleep endoscopy (DISE) enables dynamic, real-time visualization of the level, severity, and configuration of upper airway collapse during pharmacologically induced sleep, providing anatomical information that polysomnography cannot. It is increasingly used to guide individualized surgical planning in obstructive sleep apnea (OSA), particularly in children with persistent OSA after adenotonsillectomy or at elevated risk of surgical failure. However, the absence of a universally accepted pediatric-specific classification system and standardized protocol contin

Compound Testing of Human-Induced Pluripotent Stem Cell-Derived Cardiomyocytes Using Multi-Well Microelectrode Arrays.

The cardiac action potential is a highly orchestrated process that depends on the precise timing of activation and inactivation of various ion channel subtypes. Any deviation from this carefully regulated sequence, such as the blockage of specific voltage-gated potassium channels, can disrupt the electrical balance and lead to life-threatening arrhythmias. Standard cellular assays, which typically rely on cells engineered to express only a single ion channel subtype, have significant limitations in accurately predicting a compound's effects on the heart. These limitations often result in prema

Intimate Partner Violence Among Postpartum Women in Low-Income Developing Rural Areas: A Cross-Sectional Study in China.

Intimate partner violence is a global issue with severe impacts on women's health, especially during pregnancy and postpartum. Living conditions for mothers in rural areas are characterized by transportation isolation, low literacy levels, lower economic status, limited access to healthcare, and social norms that condone violence. These specific cultural, social, and economic factors have contributed to the worsening of intimate partner violence in rural areas. However, the current situation of intimate partner violence among women during pregnancy and the postpartum in low-income rural areas
