APOLT has been developed both to enable the native liver to regenerate in acute liver failure and to avoid the risks of long-term immunosuppressive therapy. We present our experience with APOLT in 4 patients with acute liver failure. The patients were 33, 18, 5, and 34 years of age, respectively. The causes of hepatic failure were: one HELLP syndrome, one paracetamol intoxication, and 2 causes which remained undetermined. All patients were in coma before transplantation. First symptoms had occurred 13, 2, 30, and 20 days prior to APOLT, respectively. In all cases, segments II and III of the re…

Hepatorenal syndrome (HRS) is a severe complication of liver failure with high mortality. The pathogenesis of this reversible functional renal failure is not yet clearly understood. Diagnosis is based upon the association of clinical and biological criteria. A patient was admitted to our institution for severe liver failure secondary to an exacerbation of cirrhosis, where he developed a fulminant hepatorenal syndrome. Both, the renal and hepatic failure were successfully treated by orthotopic liver transplantation. Special attention was paid to the immunosuppressive treatment with Cyclosporine…

Fifty three patients with liver cirrhosis and other chronic liver disease were divided into three different groups according to severity (group I: non-cirrhotic group, group II: compensated cirrhotic group, group III: decompensated cirrhotic group) and were studied with regard to their autonomic nervous and general nervous activity. To estimate the patients' autonomic nervous activity, they were examined on the following items: 1) their subjective symptoms, 2) orthostatic dysregulation, 3) coefficient of variation in R-R interval in ECG (CVR-R) at rest, 4) minimum heart rate (MHR) at night, CV…

The authors report the first case, to their knowledge, of disseminated coccidioidomycosis occurring in a liver transplant recipient. The case is also interesting in that the diagnosis of disseminated coccidioidomycosis was made fortuitously, only after finding the characteristic endosporulating spherules on a percutaneous liver biopsy. In addition, the authors reviewed the literature on post-transplant infection with particular emphasis on fungal pathogens. All studies concurred that Candida species was the most prevalent infecting fungal organism when both localized and disseminated forms of …

The examinations have involved 177 subjects (80 of these were regularly examined for a year), 148 of them suffering from acute or chronic diseases of the liver and 29 normal subjects. Circulating immune complexes (CIC) containing specific liver lipoprotein (SLL) were detected by the enzyme-binding immunosorption complement-fixation test. SLL CICs are most frequently detectable in the patients with acute hepatitis B (in 19 of 48), with chronic active autoimmune hepatitis (in 8 of 9), and with primary biliary cirrhosis of the liver (in 18 of 20 patients).

The Liver Cancer Study Group of Japan statistically analyzed 2396 cases of primary liver cancer diagnosed from January 1, 1978 to December 31, 1979 in over 500 hospitals throughout the country. They comprised 1047 cases of hepatocellular carcinoma, 93 of cholangiocarcinoma, 9 of mixed carcinoma, 16 of hepatoblastoma, and 33 others. In 1198 cases (50%) a histologic diagnosis was available. The survey and analysis, based mostly on the histologically proven cases, describe the gross anatomic and histologic features of the tumors, grade of anaplasia and growth patterns of the tumor cells, patholog…

The characterisation of lymphocytes from liver biopsies indicates that 'activated' T lymphocytes are present in the liver in alcohol induced hepatitis, chronic active hepatitis (HBS+ve and -ve), and in primary biliary cirrhosis but not in inactive cirrhosis, chronic persistent hepatitis, extrahepatic and drug induced cholestasis. A greater percentage of lymphocytes bear Fc-receptors in chronic active hepatitis than in alcohol induced hepatitis or cholestatic liver disease. The concentration of 'activated' T cells in the peripheral blood in all groups studied was within the normal range, sugges…

Acute liver injury is commonly caused by overdose-drug, toxins, or ischemia. The liver possesses a remarkable regenerative capacity, wherein metabolic remodeling plays a crucial role in restoring hepatic function and homeostasis. However, the specific metabolic shift that initiates hepatocyte regeneration remains unclear. Here, we employed multi-time-point multi-omics analyses to demonstrate that phosphatidylethanolamine undergoes a dynamic metabolic reprogramming, with phospholipid phosphatase 2 (PLPP2) exhibiting synchronous changes among the enzymes regulating PE metabolism. Liver biopsy sa…

BACKGROUND AND AIMS: Excessive alcohol consumption poses a significant global health concern, ranking as the world's third-largest risk factor for diseases and disabilities, contributing to 5.9% of all deaths worldwide. Among various disorders linked to alcohol misuse, alcohol-associated liver disease (ALD) is the most prominent. ALD patients often exhibit increased intestinal permeability, systemic inflammation, gut dysbiosis, and hepatic steatosis. No FDA-approved therapies are available to treat ALD or to resolve the pathological domains of alcohol-induced gut barrier dysfunction, inflammat…

A cold environment is a significant risk factor for cardiovascular diseases (CVDs), and its pathophysiological mechanisms involve complex interactions across multiple organ systems. However, the role of the liver as a central organ for metabolism and energy homeostasis in cold-related CVDs remains unclear. In this study, we systematically investigated the role of the liver in the pathogenesis of cardiac damage in cold regions by integrating clinical cohort studies, bioinformatic analysis, and animal experiments. The results demonstrated that in a cold-exposed population, serum alanine aminotra…

The membrane unsaturation in the liver is associated with abnormal biosynthesis and accumulation of cholesterol and triglycerides, which is a hallmark of metabolic dysfunction-associated steatotic liver disease (MASLD). The lysophosphatidylcholine acyltransferase 3 (LPCAT3) incorporates arachidonic acid (AA) into the membrane phospholipids and increases the degree of unsaturation. The metabolites of AA are involved in the lipogenesis under high nutrient conditions. However, the mechanistic insights by which LPCAT3 contribute to lipogenesis and MASLD remain unknown. This study found that LPACT3…

The interplay between adipose tissue and liver becomes dysregulated during liver fibrogenesis. Despite its importance, the adipose-liver axis remains overlooked, limiting the efficacy of current liver-centric therapies. In this study, we developed a dual-tissue metabolic modulation strategy to reprogram interorgan communication between adipose tissue and liver. To reduce adipose lipid overload, adipocyte-targeting peptide-modified nanoparticles loaded with rosiglitazone (P-Lip/RGL) were designed to induce white adipocyte browning and enhance lipid oxidation, thus reducing the metabolic burden …

BACKGROUND & AIMS: Most literature on paediatric autoimmune liver disease (P-AILD) comprises single-centre, retrospective studies. This study aims to describe robust, real-world data for P-AILD during the first year following diagnosis, utilising data from the prospective European Reference Network (ERN) R-LIVER registry.
METHODS: All patients younger than 18 years with autoimmune hepatitis (AIH) or autoimmune sclerosing cholangitis (ASC) enrolled in the ERN R-LIVER registry from January 2017 to October 2023 and with >12 months of follow-up were included. Each participating centre recorded da…

BACKGROUND: Conventional indicators for diagnosing liver injury in cholestatic liver diseases (CLD) include a combination of biochemical, imaging, and clinical assessments, which are critically limited by non-specificity, delayed detection windows, and poor correlation with cholestasis-specific mechanisms, underscoring the need for novel biomarker-driven strategies. Emerging biomarkers (e.g., vitamin D metabolites) are being explored to improve early diagnosis and specificity in liver injury assessment.
METHODS: Liquid chromatography-tandem mass spectrometry (LC-MS/MS) was employed for simult…

Steatotic liver disease (SLD), which encompasses alcohol-related liver disease (ALD), metabolic dysfunction-associated steatotic liver disease (MASLD), and MASLD with increased alcohol intake (MetALD), is now the leading cause of liver transplantation (LT) worldwide. ALD and MASLD have become the first and second leading indications for LT (41% and 12% in Europe, respectively), with MetALD accounting for a rapidly increasing proportion of transplants (8-10%). Candidate evaluation must be multidisciplinary and account for the complex interplay between alcohol use, metabolic syndrome, cancer, ca…
