STUDY OBJECTIVE: This study estimates trends in impairment before and after the 1992 California Mandatory Helmet Use Law using the injury impairment Scale (IIS).
METHODS: We linked medical records and police reports for a cohort of 4,790 nonfatally injured motorcycle riders who crashed between January 1, 1991, and December 31, 1993, and were treated for injuries in 1 of 18 hospitals in 10 California counties. Traumatic abdominal wall hernia (TAWH) is a rare type of hernia that occurs after an impact of the abdominal wall against a blunt object. The first case described was reported in 1906 and ever since, 46 cases have been reported. CT scan is mandatory in the diagnosis of TAWH but also because of a frequent association with significant intra-abdominal injuries. The universal donning of masks during the coronavirus disease 2019 pandemic led to a decreased ability to read people's facial expressions. In this context, it is prudent to think about how diagnoses can be missed or worsened by the practice of mandatory masking. We report the case of a 62-year-old man who developed Guillain-Barré syndrome (GBS) following Hantavirus infection. Only three similar cases have been described in the literature so far. GBS is an autoimmune disease characterized by progressive symmetrical weakness of lower limbs extending to upper limbs and face and low or absent tendon reflexes. Prompt diagnosis is mandatory as GBS is a potentially life threatening disorder and needs timely treatment to ensure fast recovery and fewer complications.

Jejunojejunal intussusception and jejunojejunal lipomas are both very rare clinical entities. We report a 24-year-old man with a known Behcet's disease who was lost to follow-up for a year. The patient was admitted for the association of scrotal ulceration and inguinal folliculitis, suggesting a Behcet's disease flare-up. Necrotizing course of the folliculitis led to the diagnosis of skin infection caused by a community-acquired methicillin-resistant Staphylococcus aureus strain, carrying Panton-Valentine leukocidin genes. Bacteriological analysis should be mandatory in the absence of specific criteria for the diagnosis of Behcet's disease.

In cases of severe atherosclerosis of the ascending aorta, alterations in the standard surgical technique are mandatory. We report mitral valve replacement and coronary artery bypass grafting in a patient with a severely atherosclerotic aorta. Cardiopulmonary bypass was conducted via an arterial cannula in the femoral artery and two single venous cannulas. Coronary artery bypass grafting was performed using bilateral internal thoracic arteries with beating heart in normothermia. The mitral valve was replaced with a mechanical prosthesis during hypothermic fibrillatory arrest.

This case report presents a patient with Leopard syndrome, with multiple lentigines all over the body and face, ocular hypertelorism, delayed secondary sexual characteristics, mild cardiac abnormalities and supernumerary teeth. Clinical relevance of this syndrome lies in its early recognition and precautions to be taken during any invasive dental procedure, which if not performed under antibiotic prophylaxis and premedication, could lead to infective endocarditis. Additionally, a multidisciplinary approach with pediatric and medical consultants is mandatory during the management of such cases.

This is the first case report on the diagnosis of spinal cord injury due to hemorrhage during the neonatal period using magnetic resonance imaging (MRI). Somatosensory evoked potentials are also helpful in the functional demonstration of this lesion. When discrepant signs, alert consciousness and intact cranial nerves are observed in newborn babies with flaccid extremities and respiratory disturbance, the immediate carrying out of MRI is mandatory in order to differentiate spinal cord injury from hypoxic-ischemic encephalopathy, cerebral hemorrhage, and neuromuscular disease.

An 11-year-old boy with septic arthritis of both knees presented with an anterior mediastinal abscess extending suprasternally. This was drained through a suprasternal incision and the mediastinal cavity was intermittently irrigated with povidone iodine solution and packed with gauze. A 68-year-old woman was hospitalized complaining of right flank pain. The excretory pyelogram revealed that the right kidney was hydronephrosis, and a retrograde pyelogram showed stringy filling defects in the middle portion of the right ureter. Suspecting a right ureter tumor, a right ureteronephrectomy was performed. The pathological diagnosis was ureteral cholesteatoma which is a rare condition with reports of only 9 cases in the Japanese literature. Although the possibilities of malignant change has been debatable, long term follow up would be mandatory.

We present 3 cases of a genital abnormality associated with unilateral renal aplasia in which the single kidney was abnormal. Renal aplasia associated with unicornuate uterus or uterus didelphia is rare but it has been reported previously. However, except for vesicoureteral reflux the single kidney usually is normal. Modern methods of evaluating renal function and urodynamics will be helpful when abnormal kidneys are found and will avoid unnecessary surgery. It is mandatory to suspect abnormalities of the urinary tract when genital maldevelopment is encountered and vice versa.

Two cases of traumatic bilateral locked facets in the thoracic spine are reported. Both patients had only minor neurological signs. They both made a full neurological recovery after surgical reduction of the locked facets. Bilateral locked facets are very uncommon in the thoracic spine. The diagnosis should be considered in any patient with a fracture-dislocation of the thoracic spine. In these cases additional lateral tomographs are required. Early open reduction of bilateral locked facets and internal fixation are mandatory.

A fatal case of infusion of a fat emulsion (Intralipid) into the pericardium is reported. Perforation of the anterior wall of the right ventricle of the heart by a central venous catheter had occurred 3 days after insertion via the basilic vein. Local myocardial inflammation and necrosis along the puncture wound through the myocardium was a feature. The incident underlines the necessity for prompt assurance of correct positioning of central venous catheters. A 31-year-old woman who had undergone bilateral ureterosigmoidostomy at age five had a moderately differentiated adenocarcinoma at the anastomotic site that was not detected by preoperative studies or intraoperative palpation. The authors review other reports of anastomotic carcinomas occurring after ureterosigmoidostomy for benign disease and recommend a diagnostic protocol. Whenever the urine is diverted away from the sigmoidostomy (rediversion), the anastomotic site should be resected. INTRODUCTION: Aortobronchial fistula is a rare but potentially fatal condition. We report our experience in the endovascular treatment of acute haemoptysis in high risk patients with aortobronchial fistula.
REPORT: Between May 2002 and December 2004 five patients presenting with acute haemoptysys due to aortobronchial fistula were treated by endovascular stent grafting of the descending thoracic aorta.
