Does a Short Period of Maxillomandibular Fixation Decrease Complications in Open Reduction Internal Fixation of Mandibular Angle Fractures?

PURPOSE: A short duration of postoperative maxillomandibular fixation (MMF) has the potential to reduce complications following open reduction and internal fixation (ORIF) of mandibular angle fractures. The purpose of this study was to determine if a short duration of MMF is associated with a reduced rate of postoperative inflammatory complications (POICs) in patients with mandibular angle fractures undergoing ORIF. METHODS: The authors conducted a retrospective cohort study consisting of patients treated with ORIF for mandibular angle fractures from August 1, 2015 to May 31, 2020 at an urban

Pediatric Mandibular Condylar Fractures With Concomitant Cervical Spine Injury: A Treatment Protocol for Prevention of Temporomandibular Joint Ankylosis.

In pediatric patients with mandible fractures and concomitant cervical spine injury (CSI), treatment strategies may occasionally compete: condylar fractures require functional therapy with range of motion exercises of the temporomandibular joint (TMJ), while CSI requires neck immobilization. The authors report the case of a child presenting with bilateral condylar head fractures, a right parasymphyseal fracture, and concomitant CSI treated with cervical collar immobilization, who ultimately developed TMJ ankylosis. Here, the authors review mandibular condylar fracture management and suggest an

Three-dimensional virtual-surgery simulation-assisted asymmetric bilateral mandibular distraction osteogenesis for a patient with bilateral condylar fractures.

INTRODUCTION: Our objective was to report a patient treated with 3-dimensional virtual-surgery simulation-assisted asymmetric bilateral mandibular distraction osteogenesis. METHODS: A boy (age, 9.5 years) had mandibular hypoplasia and facial asymmetry, induced by bilateral condylar fractures at 4 years of age. The asymmetric bilateral mandibular distraction osteogenesis was planned to correct facial asymmetry and mandibular hypoplasia. The 3-dimensional virtual-surgery simulation results were 11 mm of horizontal distraction on the right side and 4.5 mm of horizontal and 18 mm of vertical dist

Temporomandibular disorders in patients with mandibular fractures: a preliminary comparative case-control study between South Australia and Oman.

The status of temporomandibular disorders (TMD) in subjects with previously treated mandibular fracture was evaluated in two centres: South … ls. For SA injured subjects who reported clinically significant TMD symptoms, assault and bilateral mandibular fractures were predominant features. The study shows that most mandibular injuries fully recover and the associated TMJ trauma usually has low clinical significance in the long term.

Magnetic resonance imaging interpretation of temporomandibular joint disc position in mandibular condylar fractures: A fossa-based reference approach.

This study aimed to evaluate temporomandibular joint disc position on magnetic resonance imaging after mandibular condylar fractures using a fossa-based reference system, and to compare it with conventional condyle-based assessment. Patients with mandibular condylar fractures who underwent magnetic resonance imaging were retrospectively reviewed. Disc position was assessed using both a condyle-based reference and a fossa-based reference system. With the fossa-based approach, disc position was described using continuous spatial parameters, including anterior displacement angle, horizontal displ

Overcoming the absence of pre-traumatic imaging: a multiplanar three-dimensional virtual reduction workflow for mandibular condylar fractures.

BACKGROUND: A fundamental challenge in condylar fracture reconstruction with patient-specific implants is the absence of a pre-traumatic baseline, making direct intra-individual comparison impossible. Several virtual reduction workflows for mandibular condylar fractures have been introduced, but all currently available approaches rely on mirroring the healthy, unaffected condyle as a template for the affected side. However, contralateral mirroring is not feasible in bilateral fractures or cases with contralateral compromise. To address this limitation, the present study introduces a three-dime

Comparison of Pediatric and Adult Mandibular Fractures: Identifying Differences in Presentation and Management Using a National Trauma Database.

BACKGROUND: Facial fractures in children are less common than in adults but can lead to significant long-term complications, including growth abnormalities and functional issues. Mandibular fractures are the most common facial fractures in both age groups, yet there is no consensus on optimal management strategies. This study investigates the epidemiology, presentation, treatment, and outcomes of mandibular fractures in pediatric versus adult populations. METHODS: The authors conducted a retrospective, population-based study using 2016 to 2019 data from the National Trauma Data Bank. Patients…

Correlation between Malocclusion and Mandibular Fractures: An Experimental Study Comparing Dynamic Finite Element Models and Clinical Case Studies.

Mandibular fractures are very common in maxillofacial trauma surgery. While previous studies have focused on possible risk factors related to post-operative complications, none have tried to identify pre-existing conditions that may increase the risk of mandibular fractures. We hypothesized, through clinical observation, that anatomical conditions involving poor dental contacts, such as malocclusions, may increase the risk of mandibular fractures. This work was subdivided into two parts. In the first part, Digital Imaging and Communications in Medicine (DICOM) data of four healthy patients cha…

Anatomical position of the mandibular condyle after open versus closed treatment of unilateral fractures: A three-dimensional analysis.

This study aimed to compare open and closed treatment for unilateral mandibular condyle neck and base fractures by final three-dimensional (3D) condylar position at 6 months follow-up. 3D position was associated with mandibular functioning and pain. A total of 21 patients received open (n = 11) or closed (n = 10) treatment. 3D positions were assessed on cone-beam computed tomography scans. Volume differences, root mean square, translations, and rotations were obtained related to the pursued anatomical position and compared between treatment groups by the Mann-Whitney U test. Impact of patient-related factors on complications when treating mandibular fractures by load-sharing and load-bearing osteosynthesis.

The decision about the choice of load-sharing (LS) or load-bearing (LB) osteosynthesis is determined by an interplay of fracture-related and patient-related factors. In some situations a similar fracture in two different patients may be treated successfully by either of these methods. Our aim was to identify preoperative patient-related factors that may assist in deciding which form of osteosynthesis is employed. All adult patients who underwent open reduction and internal fixation of mandibular fractures (excluding condyle) between 1 October 2018 and 1 June 2021 were retrospectively identifie…

Comparative evaluation of bite force in patients treated for unilateral mandibular condylar fractures by open and closed methods.

BACKGROUND/AIM: Condylar fractures account for a large proportion (20% to 52%) of mandibular injuries. The goal of treating these fractures is to restore the occlusion, thereby re-establishing the masticatory function as close as possible to the patient's pre-trauma state. The aim of this study was to compare the bite forces following unilateral condylar fractures treated by open reduction and internal fixation (ORIF) with those managed by the closed method. METHOD: Patients with unilateral mandibular condylar fractures were divided into two groups (n = 20) using a simple randomization method

The radiographic and functional outcome of bilateral mandibular condylar head fractures: a comparison between open and closed treatment.

UNLABELLED: Bilateral mandibular condylar fractures accounts for 24% to 33% of condylar fractures but the indications of open reduction of bilateral condylar fracture are still controversial. It is generally accepted that displaced subcondylar fractures are indicated for open reduction, but the proper treatment of condylar head fractures are still variable. This retrospective study compares the radiographical and functional outcomes of bilateral condylar head fractures between open and closed reduction groups. MATERIALS AND METHODS: From February 1994 to June 2012, a total of 85 patients with
