Static reconstruction of malar region in facial paralysis: a new alternative technique for plasty of symmetric mouth appearance.

Static suspension using fascia lata graft is used as a reconstructive procedure against drooping of the mouth corner for treating longstanding facial paralysis. Although it achieves symmetry at rest, movement of the mouth corner at mouth opening is restricted to some extent because it is fixed with fascia lata to the immovable temporal fascia, the parotid fascia, or bones. This was overcome by suspending the mouth corner to the mandibular coronoid process with fascia lata, which enabled a shift of the mouth corner with mouth opening and closure. The nine patients discussed in this study were o

Total lower lip functional reconstruction with a prefabricated gracilis muscle free flap.

Total lower lip reconstruction was performed in an 18-month-old boy following a dog bite. In order to obtain an optimal functional result and to avoid any additional facial scarring, a prefabricated gracilis muscle free flap was used in a two-stage procedure. Firstly, the muscle was delayed on its main pedicle, a 'tendinous' strip was inserted along its free border and a silicone sheet was slid under its predicted intraoral side. At the time of reconstruction, the neomucosal lining obtained in this way reconstituted the labial vestibule. Furthermore, the muscle, reinnervated by the mandibular

Bioaccessibility, composition, and anti-inflammtory activity of bound polyphenols from burdock (Arctium lappa L.) root insoluble dietary fiber.

INTRODUCTION: Reconstruction of mandibular disarticulation defects is a challenging area of head and neck surgery, with a variety of options available for replacement of the condylar head. The gold standard is autogenous reconstruction of the condyle-ramus unit. The use of a prosthetic condylar head is controversial, but in challenging cases, and those with a likely poor prognosis it may be considered and can achieve a good functional result. The objective of this study is to evaluate the outcomes of its use in a high volume head and neck reconstructive unit. MATERIALS AND METHODS: A retrospe

Relevance of Cone-beam computed tomography on diagnosis and surgical planning of the cementoblastoma.

The cementoblastoma is a rare odontogenic tumor occurring in the mandibular molar and premolar of the patients in the second and third decades of life. Despite its typical benign behavior, this tumor may promote local destruction by perforating the cortical bone and displacing the mandibular canal. This case report shows a 31-year-old man with an aggressive cementoblastoma attached to the apex of the mandibular first molar. Cone-beam computed tomography revealed a hyperdense mass connected to the root of mandibular molar surrounded by a hypodense area. Multiplanar reconstructions showed ruptur

[Aneurysmal bone cyst of the mandibular condyle].

INTRODUCTION: Aneurysmal bone cyst (ABC) is a pseudotumor of bone consisting of a septated cystic cavity filled with blood. CASE: We report a case of aneurysmal bone cyst of the left mandibular condyle discovered in a 10-year-old child with a history of mandibular trauma which had occurred one year earlier. The treatment consisted in surgical excision of the lesion and reconstruction using a chondro-costal graft. The post operative development led to a chondral hyperplasia. DISCUSSION: We discuss the pathogenesis of the ABC centered on three theories, traumatic, vascular and tumoral of prima

A Giant Aneurysmal Bone Cyst in the Mandibular Condyle.

Aneurysmal bone cyst (ABC) is a rare, rapidly expanding, locally destructive, and easily misdiagnosed lesion. An ABC of the condyle is rare. This report presents a 25-year-old female with a giant ABC in the left mandibular condyle. This patient was treated with surgical resection of the affected bone and immediate mandibular reconstruction using autologous bone. Follow-up to date showed no signs of recurrence. The clinical feature, imaging finding, pathogenesis, and treatment methods of ABCs are discussed.

Predictors and Implications of Bony Union of Osseous Free Flaps for Oromandibular Reconstruction.

BACKGROUND: Optimal contouring of osseous free flaps for mandibulectomy often requires osteotomies. The impact of malunion or nonunion on hardware extrusion is not well-documented. This study assesses postoperative bony union, its correlation with plate extrusion, and related demographic and surgical predictors. METHODS: A retrospective study of 127 patients who underwent segmental mandibulectomy and osseous free tissue transfer from 2004 to 2019. Bony union was assessed via computed tomography (CT) scans at 6-9, 9-15, and 15-24 months using a callus scale. Logistic regression analyzed the re

A Comparative Study of Speech and Swallowing Postoperative Outcomes in Oral Cancer: Impact of Lateral Mandibular Continuity Restoration.

BACKGROUND: Surgical resection with mandibulectomy in advanced oral cancer results in complex defects. The influence of reconstruction of mandibular continuity on postoperative speech and swallowing function, though crucial, is not widely studied. PURPOSE: The purpose of study was to compare the speech and swallowing outcomes of oral squamous cell carcinoma (OSCC) patients with lateral mandibulectomy, with or without the restoration of mandibular continuity. STUDY DESIGN, SETTING, SAMPLE: This retrospective cohort study was performed at the Malabar Cancer Centre, Kerala, India, on patients w

Double-Flap Elevation From the Ipsilateral Lower Extremity: The Anterior Approach to Fibula Osteo-Cutaneous Flap Elevation.

The free fibular flap has been elevated by a "lateral approach" from the posterior edge of the peroneal muscle for more than 40 years. However, in this approach, the surgical view is limited because flap elevation in mandibular reconstruction is performed simultaneously with tumor resection in the supine position, even when using positioning pillows. We herein propose an "anterior approach" as a new surgical method. We retrospectively investigated free fibular flap surgeries performed using the anterior approach, which consists of three anterior approaches, over a seven-year period. First, to …

Management strategy in patient with familial gigantiform cementoma: A case report and analysis of the literature.

RATIONALE: Familial gigantiform cementoma (FGC) is a rare benign autosomal dominant fibrocemento-osseous lesion generally limited to the facial bones, typically in the anterior portion of the mandible; it is often associated with abnormalities of the long bones and prepubertal pathologic fractures. Owing to the small number of such patients, a uniform treatment criterion has not been established. This paper presents a patient with FGC who was treated in our department, and offers a systematic review of the patients reported in the literature. Our aim was to explore the treatment strategy for p

Open Dislocation of the High Ankle Joint After Fibular Graft Harvesting.

The free microvascular fibula and soft tissue transfer has become a widely used method for reconstruction of different regions. Donor site morbidity for free fibula microvascular flaps has generally been reported to be low, or at least acceptable. We describe the case of a patient who underwent vascularized free fibula graft harvest for mandibular reconstruction. After 21 months, he had sustained an open dislocation of the left high ankle joint during recreational sports activity. We did not found such case in the published data.

Three-dimensional evaluation of craniofacial asymmetry: an analysis using computed tomography.

3D image technology provides a very effective tool for evaluating, characterising, and drawing up the surgical treatment plan for potential orthognathic surgery patients. Patients with dysmorphic syndromes or incorrect jaw positions frequently show facial asymmetry. The objective of this cross-sectional survey is to evaluate facial asymmetry by means of three-dimensional computed tomography (CT) reconstructions. Twenty one consecutive patients were diagnosed using a CT scan. 3D reconstructions of the patients' skulls were made and then measurements taken of different craniometric landmarks and

Considerations for the Use of Alloplastic Temporomandibular Joint Replacement in Irradiated Patients: Report of an Off-Label Indication.

BACKGROUND: Custom-made alloplastic temporomandibular joint replacement (ATMJR) is not validated in irradiated patients. However, in specific situations, after previous reconstructive surgical failures, the authors hypothesized the role of a customized ATMJR after radiotherapy. METHODS: A 65-year-old male patient was referred to Instituto Português da Face-Lisbon, Portugal-after failed attempts of mandibular reconstruction secondary to oral carcinoma resection and partial hemi-mandibulectomy plus radiotherapy of 60 total Grays. Primary reconstruction was performed with fibula free flap. Due t…

Efficacy of Standard Costochondral Grafting in Patients With Bilateral Craniofacial Microsomia.

Craniofacial microsomia remains the second most common craniofacial deformity after cleft lip and palate. Mandibular pathology has been classically scored from type I to type III by the modified Pruzansky-Kaban classification. The authors report a case of a 5-year-old patient with Goldenhar syndrome and bilateral type III craniofacial macrosomia. The patient had absence of bilateral glenoid fossas, condyles, coronoids, and rami as well as hypoplasia of the symphysis, parasymphysis, and mandibular body. Reconstruction was performed using 2 costochondral rib autografts to reconstruct a ramus and

[Applications of autogenous outer table of the calvarial and mandibular bone in posttraumatic deformities of the nose].

OBJECTIVE: To study the applications of bone grafts from the cranium or mandible in the repair of posttraumatic deformities of the nose. METHODS: From 1997 to 2002, a total of 44 patients, who presented with secondary nose deformity as well as facial fractures, were reconstructed by autogeneous outer table grafts of the calvarial or mandibular bone after facial skeleton reposition. In this series, 32 cases used the calvarial bone, 12 cases used the mandible bone, which was harvested through a coronal incision and an oral approach respectively. RESULTS: The surgical incisions healed primarily

Resection of a giant ossifying fibroma through an intraoral approach in a 9-year-old child: immediate reconstruction and 6-year cephalometric follow-up.

An unusual opportunity was afforded to study the growth and development of the facial structures of a 9-year-old child who underwent major mandibular reconstruction. The longitudinal studies confirmed the present concepts of the factors responsible for mandibular growth as well as their repercussions on other facial structures. An intraoral subperiosteal resection of a major portion of the right hemimandible sparing the upper part of the mandibular ramus was required to eradicate a large ossifying fibroma. An iliac bone graft consisting of the outer table of cortical bone and cancellous bone w…

Sagittal split osteotomy of the fibula for modeling the new mandibular angle.

Fibula modeling techniques for mandible reconstruction carry a high risk of bone perfusion impairment and low predictability of osteotomy angles. To restore the parabolic shape of the mandible, the number of osteotomies should be as small as possible to preserve both periosteal and endosteal perfusion. We report our approach with sagittal split osteotomy (SSO) technique for mandibular angle reconstruction.Obwegeser-Dal Pont SSO of fibula flaps was performed on 10 patients who underwent mandibular angle reconstruction. Bone segments were tilted according to stereolithographic template and fixed

Plate fixation strategies for fibular reconstruction following segmental mandibulectomy: A comparative study of double-barrel and single-barrel techniques.

PURPOSE: Mandibular defects following oncological resection present significant aesthetic and functional challenges due to the loss of bone and soft tissue. Immediate reconstruction is crucial to prevent complications such as malocclusion, mandibular deviation, temporomandibular joint dysfunction, and impaired mastication, swallowing, and speech. Although the fibula flap is widely used, its limited vertical height complicates alveolar arch restoration in dentate mandibles. The double-barrel fibula flap addresses this limitation; however, the biomechanical stability of various fixation methods
